Prevention of postoperative thromboembolism by dextran 70 or low-dose heparin.

The efficacy of intravenous infusion of dextran 70 or subcutaneous administration of low-dose heparin in preventing postoperative thromboembolic complications has been investigated in a prospective randomized trial. During part 1 of the study, 232 patients over the age of 40 years who were undergoing major gynecologic surgery underwent a complete test protocol. The 125I fibrinogen uptake test (FUT) was used for the diagnosis of deep vein thrombosis (DVT) in 117 patients in the dextran and 115 patients in the heparin group. Heparin was significantly more effective than dextran for reducing DVT (P less than .001). During part 2 of the study no FUT was done but the incidence of clinical and fatal pulmonary embolism (PE) and the number of complications were studied. Of 444 patients (parts 1 and 2) 1 fatal and 2 nonfatal pulmonary emboli were diagnosed. All the emboli occurred in the dextran group. The benefit: hazard ratio appeared to favor heparin for the prophylaxis of DVT.

[Actinomycoma of the abdominal cavity].

Six months of gastrointestinal embarrassment in a 31-year old male patient were coped with by transversostomy and subsequent anti-tumour drug and irradiation treatment. However, histology tests of a pseudotumour removed from the abdominal cavity proved the presence of actinomycosis (actinomycoma). Once adequate treatment had been started, the intraabdominal process began to recede thus permitting the transversostomy to be discontinued.

Spectrum of total fatty acids in cerebrospinal fluid determined by gas chromatography.

A simple gas-liquid chromatographic method for the determination of the spectrum of fatty acids in a small volume of cerebrospinal fluid (CSF) is presented. In a group of 49 neurological patients it has been found that in the CSF of the controls (n = 12) there are the following main fatty acids: oleic (27.28%), palmitic (23.23%), stearic (12.21%), linoleic (7.66%), myristic (5.02%), and palmitoleic (4.51%). Altogether 28 fatty acid methyl esters (FAME'S) from 12:0 to 22:2 have been tentatively identified. The majority of them appeared irregularly, sometimes in less than in 10% of cases. The composition of FAME'S in the CSF of patients with lumbar discopathy and with acute ischaemic cerebrovascular accidents does not differ from the control group. A significant difference (P less than 0.01) has been found in the group of hypophyseal adenomas in which the amounts of practically all saturated FAME'S with an odd carbon number are elevated. The same applies to the 18:0 and 20:0 compounds. In the group of atrophic and degenerative CNS processes the palmitic and stearic acids predominated to the detriment of oleic and linoleic acids.

[Lipids in serum and cerebrospinal fluid in acute cerebrovascular disorders].

The cholesterol and lipoid phosphorus levels in the serum and liquor' cerebrospinalis were studied in 40 patients suffering from ischaemic cerebral lesions. A chromatographic determination was also performed for eight sub-fractions of polar and neutral fats (lysolecithin, sphingomyelin, lecithin, phosphatidyl-ethanolamine, non-esterified fatty acids, cholesterol, triglyceride and cholesterol ester). Furthermore, five sub-fractions of cholesterol esters were prepared according to the degree of unsaturation in the individual fatty acids. The results were compared with a group of 25 control persons. No significant changes in the lipide sub-fractions were found in the serum. Total cholesterol was lower for cerebral haemorrhages, lipoid phosphorus was higher. In the liquor, no increase was found in total lipides but only a shift in some lipide fractions, the most pronounced being a lowering in triglyceride and an increase in free cholesterol and sphingomyelin. It would seem that a determination of lipides in the serum and liquor is not decisive for the diagnosis of acute vascular cerebral haemorrhages. These questions require thorough research and experimentation.
