Erectile dysfunction caused by venous leakage: treatment with detachable balloons and coils.

Impotence can be caused by leakage of venous blood from the penis during erection. Of 135 patients studied with erectile dysfunction, 40 had venous leakage, and these patients were classified into three groups according to findings on cavernosograms. Thirty-one patients were treated with implantation of balloons and coils. Twenty-six of these patients now have normal sexual function. The authors believe this technique is simple, safe, and effective for the treatment of impotence caused by venous leakage.

Transcutaneous angioplasty of vertebral artery atheromatous ostial stricture.

The authors report experience of intravascular angioplasty of atheromatous ostial strictures of the vertebral artery in 24 patients. After reviewing the technique, they outline the indications provided through Doppler velocimetric data. The results seem to be encouraging as symptoms disappeared completely in 21 patients.

[Endoluminal angioplasty for atheromatous stenoses of supra-aortic trunks. The brachiocephalic arterial trunk, subclavian arteries].

The authors are reporting their experience about percutaneous transluminal angioplasty in atheromatous stenosis of proximal supra-aortic arteries. In this series of 12 patients, with an 6 months follow-up, vertigo was the main clinical symptom (6 cases), and it must be pointed out that 5 of these 6 patients also underwent a proximal angioplasty of one or the two of their vertebral arteries at the same time. Two subclavian steal syndromes, one arterial insufficiency of an upper limb, one transient cerebral ischaemia, and two asymptomatic cases were also treated by the same procedure. For diagnosis as well as for follow-up, the authors are emphasizing the value of hemodynamic results of Doppler recording. As for the technics itself, it mainly derives from the PTA of the lower limbs arteries. The results in this series appear to be good from an hemodynamic point of view, with neither morbidity, nor mortality.

[Chemical sympathectomy in the treatment of arteriopathies of the lower limbs].

Phenol injected transcutaneously into the lumbar sympathetic chain appears to be effective in ischemic arterial disease of the lower limbs. Among the other procedures, such as angioplasty, belonging to the field of therapeutic radiology, this technique seems to have it's specific indications. A preliminary study in fifty patients is reported.

[Angioplasty of the arteries of the extremities. Early and mid-term results (up to 3 years) in 492 cases].

Results of iliac, femoral and/or iliac artery percutaneous transluminal angioplasty (P.T.A.) in 492 cases are presented, and the latest technical improvements in the procedure reviewed. Short and medium-term results in iliac artery stenoses were excellent (98 and 96% respectively), obstructions in this region not being treated. The advantage of P.T.A. over surgery is the lack of exposure of patients to the risk of circulatory insufficiency. Very good results were obtained in stenosis of the distal femoral artery, the method being much less effective in cases of long thrombosis. The essential indications are all types of stenosis of limb arteries, while contraindications can be summarized as iliac artery obstructions (because of possible severe retroperitoneal hematomas) and massive atheromatous calcifications. Aortic and graft stenoses are particular indications. Complications are mainly hematomas and thromboses, but these can be reduced in incidence by increasing experience of radiologists in the use of P.T.A. and by improved selection of patients. Progress will also depend on improved control of anticoagulant and thrombolytic treatments. The question is raised as to the need to treat early claudication due to femoral artery stenosis following detection by the Doppler effect test. The essential argument in favor of P.T.A. is the simplicity and safety of the method to the compared reconstructive surgery, a method involving too high a risk.

[Cervical neurenteric cyst with paroxysmal symptoms].

Description of a cervical intradural extramedullary neurenteric cyst in a fifty eight years old female. After a sudden onset of right brachial paresis, spinal hemiplegia during the following few days. The tumor was situated at C6 and C7 in front of the spinal cord. It could be completely removed by means of laminectomy.

[2 cases of post-radiation stenosis of the main cerebral artery trunks].

Two cases of common carotid stenosis as a late complication of radiotherapy are reported. One of them was treated by subclavian-carotid by-pass. Delayed lesions of medium and large arteries after radiotherapy are reviewed, with emphasis on the neck vessels--and the surgical possibilities.
