Twice-weekly dosing for thyroxine replacement in elderly patients with primary hypothyroidism.

Seven female patients (mean age 86 years) with proven biochemical primary hypothyroidism were enrolled in a single-blind randomized crossover study, of standard daily versus twice-weekly thyroxine therapy, with each phase of one month's duration. The median daily dose of thyroxine was 100 micrograms (range 75-100 micrograms). Serum levels of thyroid hormones and thyrotrophin were very similar during twice-weekly thyroxine therapy to those during daily therapy and there were no statistically significant differences between trough and peak serum total triiodothyronine, free thyroxine, or thyrotrophin levels or systolic time intervals during twice-weekly thyroxine. Administration of thyroxine twice-weekly to elderly patients with primary hypothyroidism gives effective biochemical thyroid hormone replacement, with no evidence from the systolic time intervals of tissue thyrotoxicosis at expected peak thyroid hormone concentrations. Supervised twice-weekly thyroxine should be considered in patients with primary hypothyroidism who comply poorly with daily dosing.

Meniscal repair in the young athlete.

Twenty-nine meniscal repairs in 26 patients 17 years of age or younger were performed using arthroscopic techniques. Clinical follow-up examinations were performed and the SF-36 Health Status Survey and International Knee Documentation Committee evaluation form were administered. A Lysholm score was determined for each patient. All 26 patients were seen for follow-up at an average of 5.0 years (range, 2.0 to 13.5). All patients had a full range of motion with no effusion, joint line tenderness, or McMurray sign present at the time of examination. No patient experienced symptoms of locking. No patient underwent repeat surgery for a nonhealed meniscal repair. The clinical healing rate in this group was 100%. The SF-36 data demonstrated an average physical functioning score of 91 and an average role physical score of 91. The average Lysholm score was 90. Twenty-two patients (85%) were performing level I activities based on the International Knee Documentation Committee rating. Excellent rates of healing, even higher than those obtained in the adult population, can be obtained with meniscal repairs performed in this young age group.

[Surgical interventions in the digestive tract during gynecological and obstetric operations].

At the University Hospital Department of Gynecology and Obstetrics in Novi Sad, from 1945 to 1975, there were 17,362 major gynecological (laparotomies and vaginal) operations and cesarean sections. During these operations 0.11% of patients (19 women) had accidental injuries of the digestive organs (small or large intestines, or the mesenterium). All these injuries were treated during one and the same surgical act (sutures or resections of the small or large intestines). In 39 (0.22%) women, owing to a wrong preoperative diagnosis, laparotomy revealed inflammatory changes or tumours of the digestive tract which required primary surgical treatment: resection of the small or large intestines or the treatment of the perforated or gangrenous appendix, etc. Among the operations performed there were also 944 appendectomies and 65 hernitomies (in the course of gynecological operations or, less frequently, as separate interventions), 13 intestinal injuries during legal and illegal abortions, as well as 9 other surgical interventions (splenectomies, surgical treatment of the ileus, etc). In a total of 1079 pelvic operations also operations of the digestive tract were jointly made, which makes 6.46% of all major gynecological operations and cesarean sections. Obviously, it is not rare that the gynecologist-obstetrician faces situations requiring surgical interventions in the abdominal organs along with those in the genital tract. This raises the question of how much the gynecologist-obstetrician should be qualified also for abdominal surgery and urology, which is particularly important from the point of view of postgraduate education of gynecologists-obstetricians. In this country there are still certain dilemmas concerning this question.

Introducing assessment criteria into patient training.

After discussion with other CAPD training centres, it became apparent that there was no formal evaluation of patients' ability to self care with CAPD before discharge. A comprehensive list of skills and knowledge was set out in a formal document. Daily assessment during training revealed that both patients and staff felt empowered with the implementation of this tool.

Effect of simulation gaming on attitudes toward mental illness.

Sixty nursing students were administered the Opinions about Mental Illness (OMI) scale. Students whose scores fell above the median on authoritarianism and social restrictiveness were randomly assigned to experimental and control groups. The experimental subjects' scores on authoritarianism and social restrictiveness decreased significantly (p greater than .001), wheras the control groups' scores on these two factors remained essentially the same.

Nursing implications of a vancomycin resistant enterococcus outbreak in a renal unit.

Vancomycin Resistant Enterococcus is a difficult organism to treat and is becoming more prevalent world-wide. When it was identified in a busy renal unit, radical and expensive measures were taken to try and control the outbreak. Although these had costs to both patients and staff, the organism was eradicated from the unit.

Pharmacists' readiness to assess consumers' over-the-counter product selections.

OBJECTIVE: To measure and explain pharmacists' readiness to adopt a proposed new standard for assessing consumers' over-the-counter product selections. DESIGN: Cross-sectional, descriptive study based on the theoretical underpinnings of the Transtheoretical Model of Change. A questionnaire was used for data collection. Sections included: (1) pharmacists' readiness to engage in a proposed standard of practice; (2) the positive and negative aspects of doing so; (3) pharmacists' agreement with current legislative status of pharmacist-only products; and (4) demographics. SETTING: Community pharmacy. PARTICIPANTS: Community pharmacists in one Canadian province. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Pharmacists' readiness to adopt a proposed new standard of care. RESULTS: Response rate was 70.6%. Most respondents were staff pharmacists in independent pharmacies. The majority were not ready to adopt the proposed new behavior--57.6% were in the precontemplation stage. Age, sex, year of graduation, workload, and size of town/city had no significant effect on readiness for change. As expected, precontemplators showed less overall support for keeping pharmacist-only agents behind the counter. Positive and negative beliefs about the proposed standard varied across stages. CONCLUSION: Any initiatives to increase pharmacist involvement in assessing consumer nonprescription product selections must acknowledge that pharmacists differ in their degree of readiness for change.

The TICAS-RTCIP real time cell identification processor.

A preliminary model of a real time processing system, the TICAS-RTCIP (for Taxonomic Intra-Cellular Analytic System--Real Time Cell Identification Processor) has been disigned and is being tested under operational conditions, proving the substantial advantages of employing a network of microprocessors operating in parallel. The system is designed only to demonstrate the feasibility, while more extensive development and biologic testing will be required to assess general applicability to automated uterine cytology.

Computer discrimination between benign and malignant urothelial cells.

Computer discrimination between benign and malignant cells by means of the TICAS routines and subroutines has been accomplished with a small error of classification, not greater than ten percent. One feature of major diagnostic significance, namely, nuclear texture was singled out for discussion. This study augurs well for the future of computerized cytology of the urinary tract.

Storytelling and violence against women.

Health professionals are frequently the first point of contact for many women who are abused and experience intimate male partner violence. Yet, practitioners often do not have the knowledge and/or feel prepared to address these issues with women. The authors propose the use of storytelling and literature as an educational strategy to challenge and change nurses' conceptualization and practices relative to abuse and violence against women.

Eating and smoking behaviours of school children in southwestern Ontario and Charlottetown, PEI.

We conducted in-class written surveys of fourth to eighth grade children's eating and smoking behaviours in southwestern Ontario (n = 870) and Charlottetown (n = 423) in order to facilitate local school boards' and health units' planning of health education initiatives. Using chi-square analysis, we examined sex, grade, provincial and rural-urban differences in behaviours. No rural-urban differences emerged. While students reported few gender differences in food group consumption, more PEI students consumed French fries, snack foods, cakes and cookies, and regular soft drinks daily. Only 60.1% (379/630) of girls and 70.0% (462/660) of boys (chi 2 = 13.8; p < 0.001) ate breakfast every day, falling to 53.2% (115/216) among eighth grade girls, and 32.9% (25/76) among students who smoked. The prevalence of smoking was 4.8% (n = 40) in Ontario and 8.5% (n = 36) in PEI (chi 2 = 6.8; p < 0.01). Compared to non-smokers, a higher proportion of students who smoked consumed snack foods and regular soft drinks daily. The results emphasize the need to promote healthy eating behaviours and anti-smoking messages among school children.
