Pemphigus vulgaris localized to the nose.

Pemphigus often begins with localized oral lesions. Initial localized pemphigus lesions elsewhere are less common. A 65-year-old man who presented with one crusted erosion on his nose was diagnosed as having pemphigus and was treated with local and intralesional steroids. Seven months after the onset of the first lesion, others occurred elsewhere on his skin.

Global approaches to quantitative analysis of gene-expression patterns observed by use of two-dimensional gel electrophoresis.

A major difficulty in the use of two-dimensional protein maps to identify and classify cell types is the problem of acquiring, selecting, and analyzing quantitative data on hundreds of protein spots. Here we use methods of multivariate statistics to analyze the differences among a panel of human cell lines, in some cases involving quantitative data on more than 250 proteins. Principal-component and cluster-analysis techniques show that the lines can be easily distinguished, even by using the subset of proteins present in all cells. A preliminary analysis of the protein changes brought about by phorbol ester-induced differentiation of the line U937 is included.

Lactic acidosis, neurological deterioration and compromised cellular pyruvate oxidation due to a defect in the reoxidation of cytoplasmically generated NADH.

Two patients, one dying at 25 days and one at 20 months had 'chronic' lactic acidaemia with a high lactate to pyruvate ratio. Both showed EEG abnormalities and seizure activity and both died of respiratory failure. Investigation of cultured skin fibroblasts from these patients revealed normal pyruvate dehydrogenase and pyruvate carboxylase activities but the cells showed a decreased ability to oxidase pyruvate which was returned to normal on the addition of methylene blue. Subsequent investigations revealed that the mitochondria from the patients' cells could oxidase pyruvate normally but that the cells had an abnormal NAD to NADH ratio under standard conditions of incubation. It was concluded that both children had a redox disequilibrium in the cytoplasmic compartment due to a problem in transporting reducing equivalents from the cytoplasmic to the mitochondrial compartments.

Effects of cigarette smoking on fasting triglyceride, total cholesterol, and HDL-cholesterol in women.

We examined the relationships of cigarette smoking with fasting triglycerides, total cholesterol, and high-density lipoprotein cholesterol (HDL-C) levels among a group of 191 white women aged 20 to 40 years. The mean triglyceride level among current smokers was 100.0 mg/100 ml and among nonsmokers was 68.4 mg/dl (p less than 0.005). Mean total cholesterol values among current smokers and nonsmokers were, respectively, 197.0 and 189.1 mg/dl (p less than 0.1). Mean HDL-C levels were 45.0 mg/dl among women who were smoking and 52.1 mg/dl among nonsmokers (p less than 0.005). Simultaneous adjustments for the effects of age, weight, height, blood glucose, resting pulse, and oral contraceptive use did not materially alter these relationships. A modest portion of the effect of cigarette smoking on risk of coronary heart disease may be explained by an adverse effect of cigarette smoking on blood lipids.

Variation of receptor status in cancer of the breast.

One hundred and nineteen patients with breast cancer had 2 or more lesions removed for oestrogen (REc) or progesterone receptor (RPc) assay, either synchronously (on 38 occasions) or after an interval (on 91 occasions). In all but 7 both receptors were assayed for each lesion. The assays did not agree on the presence or absence of REc alone, RPc alone or the combination of both receptors in 11, 13 and 16% respectively of the synchronous samples, compared with 23, 30 and 43% of the asynchronous samples. The differences between the synchronous and asynchronous samples were significant for the combined receptors (P = 0.007) but not for REc (P = 0.176) or RPc alone (P = 0.077). Variation between asynchronous biopsies was greater when the earlier lesion contained RPc (18/37 disagreed) than when it did not (8/50) disagreed, P = 0.0023). This was not true for oestrogen receptor. In those remaining receptor positive there was only a weak correlation between the first and second values (Spearman rank correlation coefficient, rho = 0.39 for REc, P less than 0.02, and 0.45 for RPc, 0.05 less than P less than 0.1). Receptor levels and receptor status may change with time. Biopsy is most appropriate at the time when systemic treatment is proposed.

Patients' priorities for medical care.

Relatively little research has been done on the importance patients attach to various aspects of their medical care when their options are constrained. We studied priorities for care among 225 patients attending the medical clinics of a university teaching hospital. Eight attributes of medical care were considered: continuity, coordination, comprehensiveness, availability, convenience, cost, expertise, and compassion. Priorities were established by the method of paired comparisons. Continuity of care was the highest priority for these patients, while cost and convenience were lowest. Priorities varied in subgroups of patients defined by demographic, illness, and utilization characteristics. Patients with acute problems preferred coordination and expertise, while those with chronic problems ranked continuity higher. Patients younger than 30 years old valued coordination most; older patients preferred continuity and comprehensiveness. Since all aspects of medical care cannot be provided to all people, and choices are necessary, patients' priorities should be considered when planning health services.

Endogenous auxin and ethylene in pellia (bryophyta).

The occurrence of endogenous indole-3-acetic acid and ethylene in bryophyte tissue was tentatively demonstrated using gas chromatography, high performance liquid chromatography, and double-standard isotope dilution techniques. Rapidly elongating stalks (or setae) of Pellia epiphylla (L.) Corda sporophytes contain approximately 2.5 to 2.9 micrograms per gram fresh weight of putative free IAA. Ethylene released by setae increases during growth from 0.027 to 0.035 nanoliter per seta per hour. Application of 5 microliters per liter ethylene inhibits auxin-stimulated elongation growth of this tissue, a result which suggests that both endogenously produced compounds act in tandem as natural growth modulators.

An evaluation of the Technicon H6000 haematology system.

The Technicon H6000 haematology system combines haemoglobinometry with cell counting including differential counts of leucocytes. Evaluation confirmed it to be safe, precise and free from carry-over of practical significance. Comparability with standard reference methods showed a difference with mean cell volume (MCV) and packed cell volume (PCV) but the haemoglobin, red cell count and mean haemoglobin concentration (MCH) were comparable. Total white cell counts and differential counts were comparable apart from eosinophils and basophils. Platelet counts were higher at both ends of the range. Throughput was confirmed at 90 samples per hour. The system was shown to be cost-effective when taking into consideration staff costs, materials and maintenance but expensive with regard to materials alone.

Specific neurotoxin lesions of median raphe serotonergic neurons disrupt maternal behavior in the lactating rat.

Impairments in lactation after electrolytic lesions of the median raphe (MR) nucleus have been corrected by treatment with PRL. Specific serotonin neurotoxin lesions were used in the present study to determine whether decrements in litter growth after electrolytic lesions could be attributed to serotonergic neuron damage at the MR locus, and whether MR lesions (MRL) disrupted suckling-induced PRL release. Intracerebral microinjection of 5,7-dihydroxytryptamine (5,7-DHT) into the MR nucleus produced dose-related decrements in litter growth after either 4 micrograms (sham, 1.35 +/- 0.05; MRL, 1.04 +/- 0.05 g/pup X day; P less than 0.001) or 8 micrograms 5,7-DHT (sham, 1.35 +/- 0.06; MRL, 0.87 +/- 0.11 g/pup X day; P less than 0.001). Despite hypothalamic serotonin depletions of 15% and 55%, respectively, for the two doses of 5,7-DHT, there was no difference between sham and MRL animals in either basal or suckling-induced PRL release. When lesions were placed on day 1 of lactation (L) so that killing on day 7-L corresponded to the early maximal neurotoxin effect, MRL mothers still showed litter growth decrements (0.37 +/- 0.07; sham, 0.98 +/- 0.08 g/pup X day; P less than 0.001) and normal PRL values. When maternal behavior was examined, MRL animals exhibited a higher incidence of abnormal behaviors (failure to retrieve pups, cannibalism, and failure to initiate suckling during a 1-h test period; Fisher's exact P, Sham vs. MRL, less than 0.01, less than 0.05, and 0.15, respectively) than sham animals or animals with 5,7-DHT lesions in the dorsal raphe nucleus or superior colliculus. In addition, suckling behavior scores, determined from daily suckling behavior observations, were lowest in the MRL group and correlated with litter growth only in this group (r = 0.789; P less than 0.01). These data suggest that serotonergic elements in the MR nucleus play an obligatory role in maintaining normal maternal behavior during lactation, but they are not involved in suckling induced PRL release.

Dorsal raphe-hypothalamic projections provide the stimulatory serotonergic input to suckling-induced prolactin release.

The present study was carried out to determine whether dorsal raphe serotonergic neurons are involved in the regulation of suckling-induced PRL release. Neurotoxin lesions were placed stereotaxically in lactating rats on day 1 of lactation by 5,7-dihydroxytryptamine microinjection into the dorsal raphe (DR), median raphe (MR), or superior colliculus (SC), an area devoid of serotonergic perikarya. Litters were adjusted to eight pups each and weighed daily to determine litter growth rates. On day 7 of lactation, litters were separated from mothers for 8 h, after which six healthy foster pups were provided for a 30-min suckling stimulus. Animals were killed by decapitation immediately after suckling, plasma was collected for RIA of PRL, and brains were frozen and dissected for determination of hypothalamic, caudate, and hippocampal serotonin (5-HT) using the enzymatic-isotopic assay procedure. Litter growth rates from days 1-7 of lactation were significantly different among lesion groups (P less than 0.005), with litters from SC-lesioned animals (SCL) growing similarly to the sham group (sham, 0.924; SCL, 0.941 g/pup . day). In contrast, growth rates of litters from both DR-lesioned (DRL) and MR-lesioned (MRL) animals were significantly depressed (DRL, 0.596; MRL, 0.449 g/pup . day; P less than 0.05 and P less than 0.01, respectively). 5-HT levels in hypothalamus, caudate nuclei, and hippocampus were similar in the sham and SCL groups, whereas hypothalamic 5-HT was depleted by 63% and 55%, respectively, in the DRL and MRL groups. Despite impairments in growth rate and litter survival in both the DRL and MRL groups, only DRL animals showed significant decrements in suckling-induced PRL release (DRL, 288 +/- 107; sham, 837 +/- 134 ng NIAMDD rat PRL RP-1/ml; P less than 0.05) after 5-HT-depleting lesions. The results suggest a specificity of function within the raphe system during lactation; DR 5-HT neurons which project to the hypothalamus provide stimulatory inputs to suckling-induced PRL release, whereas MR 5-HT neurons influence litter growth and survival via their role in maternal behavior.

Age changes in the bone density and structure of the lumbar vertebral column.

Old age is associated with a decline in bone density in lumbar vertebral bodies in both sexes, although the rate and amount of the decline is greatest in females. The bone translucency index method, described in this study, is a sensitive method of estimating bone density. The primary reason for this decline is the significant decrease in the number of transverse trabeculae of lumbar vertebrae in old age. It is postulated that the increase in vertebral end plate concavity and the increased horizontal dimensions of lumbar vertebral bodies in old age follows as a direct consequence of the selective loss of the transverse trabeculae.

C-reactive protein and serum amyloid P component in the plaice (Pleuronectes platessa L.), a marine teleost, are homologous with their human counterparts.

C-reactive protein and serum amyloid P component were isolated from serum of the plaice (Pleuronectes platessa L.), a murine teleost. The isolation was based on their calcium-dependent binding affinity for pneumococcal C-polysaccharide and for agarose, respectively. These specificities are the same as those of human C-reactive protein and serum amyloid P component, respectively, and we have previously reported that the plaice molecules resemble human C-reactive protein and serum amyloid P component in their electron microscopic appearance. We describe here estimation of the molecular weights of plaice C-reactive protein and serum amyloid P component and their subunits, and analysis of their amino acid composition, glycosylation and partial amino-terminal amino acid sequences. The results establish that plaice C-reactive protein and serum amyloid P component are homologous with each other and with their human counterparts and indicate that there has been stable conservation of this protein family throughout vertebrate evolution.
