Template switching by reverse transcriptase during DNA synthesis.

The ability of reverse transcriptase to make template switches during DNA synthesis is implicit in models of retrovirus genome replication, as well as in recombination and oncogene transduction. In order to understand such switching, we used in vitro reactions with purified nucleic acids and enzymes. The assay system involved the use of an end-labeled DNA primer so as to allow the quantitation of elongation on a donor template relative to the amount of elongation achieved by template switching (by means of sequence homology) when an acceptor template RNA was added. We examined several variables that affected the efficiency of the reaction: (i) the reaction time, (ii) the relative amounts of acceptor and donor template, (iii) the extent of sequence overlap between the donor and acceptor templates, and (iv) the presence or absence of RNase H activity associated with the reverse transcriptase. The basic reaction, with RNA templates and normal reverse transcriptase, yielded as much as 83% template switching. In the absence of RNase H, switching still occurred but the efficiency was lowered. Also, when the donor template was changed from RNA to DNA, there was still switching; not surprisingly, this was largely unaffected by the presence or absence of RNase H. Finally, we examined the action of the RNase H on RNA templates after primary transcription but prior to template switching. We found that in most cases, both ends of the original RNA template were able to maintain an association with the DNA product. This result was consistent with the work of others who have shown that RNase H acts as an endonuclease.

5 alpha-reductase deficiency without hypospadias.

A boy aged 4 with penoscrotal hypospadias and his brother aged 12 with micropenis had typical changes of homozygous 5 alpha-reductase deficiency. After three injections of chorionic gonadotrophin there was a trivial rise in plasma dihydrotestosterone with a normal increase in plasma testosterone. Urine steroid chromatography showed abnormally high 5 beta: 5 alpha ratios and 5 alpha-reductase activity was appreciably reduced in genital skin fibroblasts. The results indicate that 5 alpha-reductase deficiency is not invariably associated with genital ambiguity.

Acute lobar atelectasis. A comparison of two chest physiotherapy regimens.

Fourteen cases of acute lobar atelectasis were alternately allocated to one of two chest physiotherapy regimens for treatment. Treatment in group 1 comprised positioning, vibrations, hyperinflation, and suction, and in group 2, treatment consisted of hyperinflation and suction alone. Treatment in either group was given hourly for six hours. Patients in group 1 had a significantly higher mean percentage resolution of their atelectasis (mean value, 60.1 percent), as seen on chest roentgenogram, after one treatment intervention than patients in group 2 (mean value, 7.6 percent; p less than .006). After the intensive six-hour treatment period, the difference between the groups was marginally statistically significant, still favoring group 1 over group 2 (p less than .055). Follow-up roentgenograms at 24 and 48 hours revealed no significant difference between the treatment groups (p greater than .10 and greater than .20, respectively). These results suggest that, at least initially in the course of acute lobar atelectasis, positioning and vibrations add to the efficacy of a treatment of hyperinflation and suction alone.

Factors affecting alcohol consumption in black women. Part I.

The purpose of this investigation is to evaluate the extent to which a general model for understanding and predicting Black mental health problems accounts for the particular problem of alcohol consumption in an urban sample of 289 African American women. The general model consists of eight variables: life events, social support, religious orientation, internalized racialism, physical health problems, marital status, socioeconomic status, and developmental status. In Part I expected interrelationships among variables are presented, from which a structural equation model for understanding and predicting alcohol consumption is formulated. Methods for evaluating the model are described in Part II (International Journal of the Addictions, Vol. 25, No. 12).

Factors affecting alcohol consumption in black women. Part II.

An eight-variable model for understanding and predicting alcohol consumption in a sample of 289 African American women is evaluated using a structural equation methodology. We found that life events, physical health problems, and internalized racialism played important roles in accounting for variance in alcohol consumption. Marital status did not have the predicted inverse effect on alcohol consumption. While religious orientation did not have the expected inverse effect on alcohol consumption, it had an unexpected direct effect on internalized racialism, which had a direct effect on alcohol consumption. We found that the effects of socioeconomic status and developmental status on alcohol consumption were mediated through other variables specified in the model. Overall the model, which provided partial to complete support for five of eight hypotheses, provided a statistically adequate fit.

Teacher-pupil talk: integrated vs segregated environments for children with severe learning difficulties.

The present study aimed to investigate the linguistic environment of children with severe learning difficulties attending nursery school. Teacher-pupil talk with three normal language learners and five children- with severe learning difficulties was examined in segregated and integrated settings. A cognitively based dialogue system analysis was used to ascertain the style and level of teacher and pupil interactions. Results revealed that teachers are able to adjust their speech when interacting with children of differing abilities but they do so by assuming a dominating role in dialogue through initiation and the use of obliges.

The reorganization of mine labor recruitment in Southern Africa: evidence from Botswana.

"The past decade has been one of unprecedented change in the pattern and organization of mine labor recruitment in Southern Africa. Using detailed data on recruitment patterns in Botswana, this article supports the view that recent changes have initiated a self-sustaining trend whereby certain flows of foreign labor into South Africa will decline unabated into the foreseeable future. This results from a shift in general recruiting policy from one of encouraging external migrant labor flows--by expanding recruitment networks and employing a variable and transient workforce--to one of retrenchment and labor stabilization biased in favor of internalized labor supplies."

Obstetric care in rural Missouri: the loss of rural general and family practitioners.

Family and general practitioners have historically provided a substantial portion of obstetric care in rural parts of the United States, including Missouri. The authors surveyed 328 rural general physicians to determine their participation in obstetrics. Their findings show a dramatic loss of physician obstetric services in rural Missouri and suggest that the dilemma is not likely to be easily remedied.

AMP deaminase from yeast. Role in AMP degradation, large scale purification, and properties of the native and proteolyzed enzyme.

Eukaryotes have been proposed to depend on AMP deaminase as a primary step in the regulation of intracellular adenine nucleotide pools. This report describes 1) the role of AMP deaminase in adenylate metabolism in yeast cell extracts, 2) a method for large scale purification of the enzyme, 3) the kinetic properties of native and proteolyzed enzymes, 4) the kinetic reaction mechanism, and 5) regulatory interactions with ATP, GTP, MgATP, ADP, and PO4. Allosteric regulation of yeast AMP deaminase is of physiological significance, since expression of the gene is constitutive (Meyer, S. L., Kvalnes-Krick, K. L., and Schramm, V. L. (1989) Biochemistry 28, 8734-8743). The metabolism of ATP in cell-free extracts of yeast demonstrates that AMP deaminase is the sole pathway of AMP catabolism in these extracts. Purification of the enzyme from bakers' yeast yields a proteolytically cleaved enzyme, Mr 86,000, which is missing 192 amino acids from the N-terminal region. Extracts of Escherichia coli containing a plasmid with the gene for yeast AMP deaminase contained only the unproteolyzed enzyme, Mr 100,000. The unproteolyzed enzyme is highly unstable during purification. Substrate saturation plots for proteolyzed AMP deaminase are sigmoidal. In the presence of ATP, the allosteric activator, the enzyme exhibits normal saturation kinetics. ATP activates the proteolyzed AMP deaminase by increasing the affinity for AMP from 1.3 to 0.2 mM without affecting VM. Activation by ATP is more efficient than MgATP, with half-maximum activation constants of 6 and 80 microM, respectively. The kinetic properties of the proteolyzed and unproteolyzed AMP deaminase are similar. Thus, the N-terminal region is not required for catalysis or allosteric activation. AMP deaminase is competitively inhibited by GTP and PO4 with respect to AMP. The inhibition constants for these inhibitors decrease in the presence of ATP. ATP, therefore, tightens the binding of GTP, PO4, and AMP. The products of the reaction, NH3 and IMP, are competitive inhibitors against substrate, consistent with a rapid equilibrium random kinetic mechanism. Kinetic dissociation constants are reported for the binary and ternary substrate and product complexes and the allosteric modulators.

Using nurses for preventive activities with computer assisted follow up: a randomised controlled trial.

OBJECTIVE: To assess whether an organised programme of prevention including the use of a health promotion nurse noticeably improved recording and follow up of cardiovascular risk factors and cervical smears in a general practice that had access to computerised cell and recall. DESIGN: Randomised controlled trial. SETTING: General practice in inner London. PATIENTS: All 3206 men and women aged 30-64 registered with the practice. INTERVENTION: The intervention group had their risk factors ascertained and followed up by the health promotion nurse and the general practitioner, whereas those in the control group were managed by the general practitioner alone. END POINT: Recording and follow up of blood pressure and cervical smears after three years. Recording of smoking, family history of ischaemic heart disease, and serum cholesterol concentrations were also examined. MEASUREMENTS AND MAIN RESULTS: When the trial was stopped after two years the measurements of blood pressure in the preceding five years were 93% (1511/1620) v 73% (1160/1586) (95% confidence interval for difference 17.5 to 22.7%) for intervention and control groups respectively. For patients with hypertension the figures were 97% (104/107) v 69% (80/116) (18.2 to 38.2%). For women the proportion who had had a cervical smear in the preceding three years were 76% (606/799) v 49% (392/806) (22.5 to 31.9%). Recording of smoking, family history of ischaemic heart disease, and serum cholesterol concentrations was also higher in the intervention group compared with the control group. CONCLUSION: An organised programme, which includes a nurse with specific responsibility for adult prevention, is likely to make an important contribution to recording of risk factors and follow up of those patients with known risks.

Cluster analytically derived MMPI profile subgroups of incarcerated adult rapists.

Sex offenders are known to be heterogeneous in their patterns of personality functioning and behavior. The present study conducted a multivariate cluster analysis on the MMPIs of 120 incarcerated adult male rapists. The analysis resulted in the identification of five MMPI profile subgroups. Additional differences among subgroups were identified in the circumstances of the crime and responses to the Multiphasic Sex Inventory. Levels of psychopathology, as indicated by the MMPI, ranged from slight to extreme. Results are discussed with reference to previous MMPI research with sex offenders. Suggestions for future research are offered.

The plasma catecholamine stress response is characteristic for a given animal over a one-year period.

The same male and female rats with indwelling jugular catheters were stressed (immobilization) on two occasions at the ages of 3 to 4 and 15 to 16 months. Plasma levels of the catecholamines (CA) norepinephrine (NE) and epinephrine (E) were determined before and during stress at each session. During stress, plasma NE and E levels increased markedly. At the younger age, female animals showed markedly higher CA levels than male animals. After one year, stress CA levels were higher for both sexes, but had increased markedly in male and little in female animals. A positive correlation was found between the plasma CA stress response of individual animals at both sessions; this correlation was stronger for males and NE. This indicates that "high" responders usually remain high responders, "low" responders remain low and intermediate responders intermediate. In conclusion, the plasma CA stress response increases markedly in male, but little in female rats over the period of 1 year and the relative magnitude of the individual stress response remains a characteristic of each given animal.

Effects of buspirone on plasma catecholamines, heart rate, and blood pressure in stressed and nonstressed rats.

The influence of buspirone upon plasma catecholamine levels, heart rate, and mean arterial blood pressure was studied in stressed and nonstressed rats. Measures were obtained directly via indwelling aortic catheters. Drug or vehicle were given acutely (10 mg/kg, IP) or twice a day for 10 days (10 mg and 20 mg/kg, SC). In nonstressed rats, a single dose of buspirone increased markedly plasma norepinephrine and epinephrine levels and decreased significantly heart rate with no effect on blood pressure. During stress, stress-induced increases in catecholamine levels were further elevated by the drug, whereas stress-induced increases of heart rate and mean arterial blood pressure were reduced. In chronically-pretreated rats, the effects of buspirone were similar to those observed after an acute injection. These effects of buspirone on plasma catecholamines are very different from those seen with other anxiolytics, whereas effects on heart rate and blood pressure are more similar.
