Virilizing adrenal ganglioneuroma containing Leydig cells.

An adrenal medullary ganglioneuroma containing Leydig cells and revealed by a virilizing syndrome in a 56-year old woman is presented. The syndrome, associating with masculinization an elevated serum testosterone level and a normal urinary 17 ketosteroids, is uncommon in adrenal tumours. The tumour was located by computed tomographic (CT) scan and treated by right adrenalectomy. Microscopic examination showed typical features of an adrenal medullary ganglioneuroma containing Leydig cells with Reinke crystalloids. Positive immunohistochemical study confirmed the testosterone secreting nature of the cells. The presence of Leydig cells in the adrenal gland is discussed according embryogenic studies: 1. the common origin of the gonad and the adrenal cortex from the coelomic epithelium, 2. the vicinity of adrenocortical gland and gonad during embryogenesis, 3. the thecal metaplasia of mesenchymal cells, 4. the development of Leydig cells from Schwann cells. Fourth case published in the world literature.

[Vascular microanastomosis with diode laser. Control study and morphological results].

Direct diode laser carotid (0.7-1.2 mm) end-to-end microanastomosis versus contro-lateral manual suture microanastomosis (CMA) were performed in 70 Wistar rats. The laser beam-wavelength 830 nm- was transmitted through a micromanipulator and provided a focused spot of 300 microns in diameter. After placement of three 10.0 Ethilon stitches for edge coaptation, the laser anastomosis was performed using laser shots (average 3) of 500 mW power, 4.5 s duration and 700 W/cm2 irradiance each. CMA was performed by six 10.0 stitches. Good vascular flow was confirmed by macroscopic controls, Doppler spectral analysis (n = 548) and digitalized angiography (n = 18) performed from day 0 to day 210. The patency rate (93%) as well as the complication rate (5/66) were similar in both series. Nevertheless light and scanning electron microscopy (n = 82) showed the morphological superiority of laser anastomosis: reendothelialization by day 3 and medial collagen restructuration by day 10 whilst after CMA, the arterial wall repair was delayed and the medial scar occupied by fibrotic tissue. The other advantages of the laser anastomosis were, shorter operating time minimizing organ ischemia (13 min versus 22 min), and slight endothelial trauma reducing thrombogenic risk. The technical advantages of diode laser were pointed out: miniaturization, no special maintenance and decreasing price of diodes.

Health profiles of current and former smokers and lifelong abstainers. OXCHECK Study Group. OXford and Collaborators HEalth ChecK.

The aim of this study was to determine the extent to which smokers and smoking quitters differ in habits and risk factors from non-smokers. Subjects comprised 8,109 patients aged 35-67 years having health checks in British primary care. We compared lifestyle and measured cardiovascular risk factors in smokers, former smokers and lifelong abstainers in cross-sectional analyses, and in prospective data in quitters. Results were adjusted for confounding factors. Considering 25 aspects of lifestyle, smokers had significantly worse habits in 20 compared to abstainers, and in 17 compared to former smokers. These included eating more white bread, full cream milk, fried food and meat, and less fruit and vegetables, wholemeal bread and bran cereals. Smokers report drinking more alcohol and taking less exercise. Smokers' mean serum levels were higher for total and low density lipoprotein cholesterol and triglycerides and lower for high density lipoprotein cholesterol. Within five years ex-smokers' data became comparable to lifelong abstainers for most factors, with apparent attenuation over up to 20 years for triglyceride, body mass index and scores for fibre and polyunsaturated fat intake. Smokers who quit after initial examinations had better health profiles even before quitting (p = 0.016) and subsequently made more beneficial health changes (p = 0.039) than continuing smokers. Smoking is associated with relatively poor health choices and risk factor levels. Stopping smoking is associated with a wide range of improved health markers beyond avoidance of tobacco toxicity.

Morphological evidence for the location of calcitonin gene-related peptide (CGRP) immunoreactivity in rat lymphocytes.

Calcitonin gene-related peptide (CGRP) is widely distributed in sensory neurons and nerve fibers. It was shown recently in our laboratory that there was CGRP-immunoreactivity (CGRP-ir) in extract of rat lymphocyte of thymus and mesenteric lymph node by radioimmunoassay and reversed-phase HPLC. The aim of this study was to detect the CGRP-ir location in the rat lymphocyte by immunocytochemical method. Single cells isolated from thymus and mesenteric lymph node of male Wistar rat (200-250 g) were suspended in RPMI-1640 medium. After adhesion to plate wall and through nylon wool fiber columns, T cell-riched suspension was obtained. Immunocytochemical ABC method was performed in cell suspension, the specific antiserum (1:200 diluted) was rabbit anti-human CGRP. The cells were examined under light microscope after smeared on glass slide by Shandon Cytospin. The results showed that some lymphocytes were CGRP-ir positive. The positive granules were seen as ring, plaque, cap or spot distributing on the surface or inside the cell. The results indicate that CGRP-ir may be located in some rat T lymphocytes. This work provides new information about the interaction between the nervous and the immune systems. The functional significance of CGRP located in lymphocytes needs to be further studied.
