Effect of domperidone on serum TSH and growth hormone in thyroid patients.

In euthyroid female patients, the release of TSH from the pituitary increases in response to domperidone, a dopaminergic-receptor blocking agent of peripheral action. The rate of increase varies with the functional state of the thyroid, as confirmed by results obtained in cases of euthyroidism, primary hypothyroidism, subclinical hypothyroidism and subclinical hyperthyroidism. Observations suggest that the more vigorous feedback-mechanism modulates the dopaminergic regulation of TSH-secretion at a high degree of sensitivity. Stimulation with 1-dopa intensifies the release of growth hormone from the pituitary which is, however, of lesser degree in hyperthyroid or hypothyroid than in euthyroid individuals. The GH-response to 1-dopa is enhanced by administration of propranolol, but the maximum serum GH levels in response to stimulation with 1-dopa are significantly lower in hyperthyroid than in euthyroid individuals. Administration of domperidone leaves the serum GH levels unaffected in euthyroid and hyperthyroid subjects, but causes a significant increase in a number of patients with primary hypothyroidism. The results suggest that the dopaminergic system plays a part in the regulation of TSH and GH secretion, asserting itself partly as a direct effect on the pituitary, and that the dopaminergic regulation may be affected by thyroid dysfunction.

Effect of ACTH-stimulated glucocorticoid hypersecretion on the serum concentrations of thyroxine-binding globulin, thyroxine, triiodothyronine, reverse triiodothyronine and on the TSH-response to TRH.

The responses of serum concentrations of TSH, thyroxine (T4), triiodothyronine (T3) and of reverse triiodothyronine (rT3) to i. v. administration of 0.4 mg THR were examined prior to (and after) i. m. administration of ACTH (2 mg Synacthen Depot) in 7 euthyroid women using estrogen-containing oral contraceptives and in 8 controls, with the following results: (1) an increase in endogenous glucocorticoid secretion is associated with a depression of the TSH response to TRH; (2) TSH formed in decreased amounts is still capable of stimulating thyroid secretion; (3) the increased serum corticoid levels fail to affect the secretory response of the thyroid to TSH; (4) control of the pituitary-thyroid axis remains normal in the presence of increased serum thyroxine-binding globulin (TBG) levels. In a further series the serum levels of TBG, T4, T3, rT3 and cortisol under the effect of ACTH-induced endogenous glucocorticoid hypersecretion were studied in 6 normal untreated controls, in 6 normal women using oral contraceptives and in 10 untreated hyperthyroid patients. During four days subsequent to treatment the serum TBG levels decreased, maximum decrease being found in the users of oral contraceptives, minimum decrease in the controls. Serum T4 was found to decrease during 2 to 4 days, serum T3 parallel with an increase in serum rT3, for 1 to 2 days, subsequent for ACTH loading. In the euthyroid cases also the serum TSH levels showed a transitory decline. It is concluded that in case of endogenous hyperproduction of glucocorticoids (1) T4 leads to T3 monodeiodination decreases and T4 leads to rT3 conversion increases parallel with the changes in the serum cortisol levels; (2) TBG synthesis is inhibited by endogenous glucocorticoids; (3) the changes in serum TBG levels are accompanied by a decrease in the serum T4 concentrations.

Adrenocorticotropic Hormone

[Long-range therapy of hypertension].

In 572 patients with hypertension a long-term therapy with alpha-methyldopa (Dopegyt) was carried out and this preparation was found to be highly effective. The preparation decreases the blood pressure in standing position as well as in lying position reducing mainly the peripheral vascular resistance. It is well absorbed orally. As a rule, the minute output of the heart is maintained without acceleration of the heart rate. An orthostatic hypotension was only rarely to be observed and only in a very small degree. With the help of the isotope renography could be proved that the treatment with alpha-methyldopa causes no reduction of the blood supply of the kidneys, wherefore alpha-methyldopa can be successfully applied in patients with hypertension and renal failure. Side effects appeared very rarely. During the application of 34,550 tablets of alpha-methyldopa only once a haemolytic anaemia was observed. In patients with a severe hypertension the combination alpha-methyldopa (Dopegyt) + etacrynic acid (Uregyt) evoked a distinct decrease of the systolic and diastolic blood pressure.

Drug Administration Schedule

Individual differences in the antidiuretic response induced by single doses of 1-deamino-8-D-arginine-vasopressin (DDAVP) in patients with pituitary diabetes insipidus.

Marked interindividual differences were found in the height and duration of the antidiuretic action induced by increasing single intravenous doses (0.5 mug and 8 mug) of DDAVP in patients with pituitary diabetes insipidus. It was assumed that differences in the duration of action reflected individual variations in the rate of removal of vasopressin.

Application of a simple fluorometric method on absorption of canrenone.

The absorption of canrenone, the major metabolite of spironolactone, was studied by a simple fluorometric method in 30 healthy subjects. Two different pharmaceutical formulations were compared on absorption, and only a negligible difference was found between the micronized and balled-milled form. Canrenone in both formulations was well absorbed, whereas spironolactone absorption was slower. Mattingly's fluorometric assay proved to be a simple method to evaluate the absorption of canrenone.

Canrenone

The antidiuretic action of 1-deamino-8-D-arginine vasopressin (DDAVP) in man.

1. After administration of a new vasopressin analogue (DDAVP), a marked and prolonged antidiuresis occurred in 10 patients with pituitary diabetes insipidus. 2. The antidiuretic effects of single intravenous doses of 0.04--24 mug DDAVP and single intranasal doses of 5--320 mug DDAVP were investigated. Time curves of the antidiuretic responses expressed in changes of urine osmolality (Uosm) and free water clearance per 100 ml GFR (CH2O X 100/GFR) are described. 3. Maximal "peak" response was obtained after an intravenous dose of 1 mug within the first 12 hrs (Uosm was 7--800 mOsm/KgH2O). Further increase of dosage resulted only in prolongation of duration of action (up to 48 hrs) and peak ("plateau") effect (up to 24 hrs). 4. There was a linear relationship between the log dose and log osmolality of urine collected in the second 12 hours after administration of single intravenous and intranasal doses of DDAVP. 5. Comparison of the effects of 1 mug lysine-vasopressin and 1 mug DDAVP revealed only slight differences in peak effects, but extreme differences in duration of action. 6. It is concluded that in the evaluation of a synthetic vasopressin analogue the maximal antidiuretic ability and the prolongation of action have to be analysed separately.

Absorption

Relationship between the dose of 1-deamino-8-d-arginine vasopressin (ddavp) and the antidiuretic response in man.

A definite relationship was found between the dose of DDAVP (1-24 mug intravenously and 5-320 mug intranasally) and the antidiuretic effects (expressed in changes in free water clearance per 100 ml GFR and in urine osmolality determined in 24 hour urine collection periods) in 7 patients with diabetes insipidus. The relationship was more conspicuous when the second 12 hour antidiuretic responses were considered, indicating a dose-dependent prolongation of the antidiuretic action. Time-curves of the antidiuretic responses proved the dose-dependent prolongation of the duration of antidiuretic action. Second 12 hour antidiuretic response increased more markedly when DDAVP was given divided in two doses a day as compared to the effects of the same quantity of the drug given as a single dose; only by the administration of excessive single doses the effects of the divided doses could be reproduced.
