[The use of clomiphene and menotropins as inducers of multiple follicular growth in a program of in vitro fertilization].

In our IVF & related techniques program, clomiphene 50 mg was used from days 2 to 6 of the cycle in 20 patients and 20 cycles. (17 IVF patients and 3 GIFT patients) menotropins were also used in 2 different schemes: a) 2 ampules in alternated days (12 cases) and b) one ampule daily (8 cases). The patients with the daily scheme required lesser total dose of menotropins and developed higher estradiol levels, although this had none statistic significance, and they also developed a greater number of follicles. There were 16 laparoscopies for ovum capture and 53 oocytes were retrieved (30.2% mature) 10 oocytes were from the GIFT patients. 16 oocytes were fertilized and 8 cleaved. There was only one pregnancy from and IVF patient with the alternated scheme that ended with a first trimester abortion.

[Down's dyndrome and congenital heart disease (author's transl)].

Out of a group of 113 cases of Down's syndrome 50 were associated with congenital heart disease, confirmed by haemodynamic, angiographic and surgical means. The mean age was 36.6 months. Twelve were cyanotic, six of them because of an inverted previous left-toright shunt. The endocardial cushion defect, alone or in association, accounted for 40% of these anomalies, and ventricular septal defect for 26%. If the cases with right ventricle outflow obstruction are excluded, 50% presented pulmonary pressures over 70 mmHg. In addition, 80% of endocardial cushion defects presented pulmonary hypertension, being the three fourths under one year of life. On the basis of the data presented, authors remark the need of an early approach to specialized diagnosis of these children, in order to allow an earlier surgical correction or palliation than in children without Down syndrome.

Clinical significance of nuclear matrix-estradiol receptor complex in human sperm.

Estrogen receptor levels were determined in nuclear matrix of human spermatozoa of normospermic fertile men and idiopathic infertile men. In noncapacitated sperm cells, the distribution of the estrogen receptor in normospermic fertile men was present in 50% of the cases, whereas in idiopathic infertile men was present in 19% of the cases. In capacitated sperm cells, the distribution of the estrogen receptor in normospermic fertile men was present in 80% of cases, whereas in idiopathic infertile men was present in 31% of the cases. The values of estrogen receptors in capacitated and noncapacitated sperm cells of normospermic fertile men were 91 +/- 21 and 26 +/- 7 fmol/10(8) sperm cells, respectively, whereas in capacitated and noncapacitated sperm cells of idiopathic infertile men, the estrogen receptor values were 50 +/- 17 and 22.5 +/- 9 fmol/10(8) sperm cells, respectively. The diminution of the estrogen receptor levels in the nuclear matrix could be a biochemical indicator of male factor infertility.

[Variant angina pectoris with demonstration of coronary spasm. Diagnostic and therapeutic considerations].

Eight patients with typical Prinzmetal's angina and angiography proven coronary artery spasm are reviewed. In all cases the angina pain appeared at rest and was accompanied by a simultaneous ST-segment elevation. The coronary angiography results varied from severe multivessel disease (3 cases) to single vessel disease (3 cases), and normal coronary arteries (2 cases). In all patients a coronary spasm was documented, in five cases induced by ergonovine and in three spontaneously; in all patients but in one, the localization of the spasm was concordant with the ST-segment elevation localization. One case with a severe proximal left anterior descending stenosis was grafted; however the pain was not relieved despite treatment with large amounts of propranolol. This patient died after an episode of severe chest pain. Treatment with nifedipine was given to six patients, with excellent results in four cases, and regular response in two. The main physiopathologic diagnostic and therapeutic aspects of the variant angina syndrome are reviewed.
