[Desmoid tumor of parotid gland].

Desmoids are rare tumors originating from musculo-aponeurotic soft tissues. Their biologic features placed them between benign fibromas and malignant fibrosarcomas. The rare of parotid gland desmoid tumor is presented. The patient was treated surgically--the tumor was resected with the safety margins. Recurrence was not observed during 12 month follow up.

Determining the feasibility of diagnosing meningiomas using static teleneuropathy images transmitted electronically.

The feasibility of using still images sent via the Internet for remote neuropathological consultation was examined. We assessed the diagnostic agreement between three groups of neuropathologists and a reference laboratory. All groups independently evaluated pictorial data exchanged by e-mail or ftp server. In the group of benign meningothelial meningioma agreement was reached in 100%, 100%, 92% of cases. In three cases of malignant meningioma agreement was achieved in 100%, 33%, 33% of cases, and in ten cases of atypical meningioma agreement was achieved in 50%, 30%, 40% of cases. Average concordance for all three groups was 83.33%, 54.33%, 57.67%. Our experiment showed that there were no discrepancies in the typical cases of benign meningothelial meningiomas. Disagreement related to the grade II and III lesions i.e. atypical and malignant meningioma. Atypical as well as malignant meningioma seems to need more direct discussion to achieve consensus. The main problem was the subjectivity of the local pathologist who overstated the informational content of the images, and an insufficient number of pictures. This may be prevented in the future by employment of dynamic teleneuropathology using a very high-resolution camera.

Real-time teleneuropathology for a second opinion of neurooncological cases.

A teleneuropathology system directed by Java programs through a standard Internet browser was evaluated in the present study. Assessment of neurooncological cases was done by remote microscope through the "Case Study" Web page at the Department of Pathology Web site (http:¿ampat.amu.edu.pl). The site was used to control a remote automatic microscope, the Axioplan 2 (Zeiss), which was connected to a computer that acts as an Internet server. The Java program for the control of the microscope server is automatically downloaded and started if the user selects the Web site of the corresponding microscope server. The microscope server receives microscope operation commands from the telemicroscopy clients, executes them, and distributes the new microscope image to all of the connected telemicroscopy clients. Fifteen cases were evaluated over several weeks. The percentage of correctly classified cases sent by remote consultation was 100%. Since the system does not require specialized software for the remote side and since the number of possible discussion partners is unlimited, this system may help to overcome obstacles to the practice of teleneuropathology.

Technological requirements of teleneuropathological systems.

Teleneuropathology is the practice of conducting remote neuropathological examinations with the use of telecommunication links. Because of a limited number of expert neuropathologists, some, especially smaller departments have the equipment to conduct the examination but do not have a specialist who would be able to evaluate material from the central nervous system. In case of teleneuropathology, a neuropathologist examines tissue fragments taken during an operation by means of a telemicroscope connected with the computer through a telecommunications network. It enables the neuropathologist to operate the microscope and camera remotely. Two basic systems exist for performing remote neuropathological examination: static and dynamic. Both have different needs in medical, computing and telecommunication aspect. Depending on the type of service the public telephone network, the integrated services digital network, or optical fibre should be used. Conditionally Internet can be used as a link for teleneuropathological system. However, for the newest developments in teleneuropathology such as teleconference and remote operation on robotized microscope only transmission over the integrated service digital network, which guarantees high speed of transmission gives a possibility to communicate. Because images are basic information element in teleneuropathological systems the high capacity of acquisition, processing, storing, transmission, and visualization equipment is necessary. The farther development of telecommunication as well as standardization of recording and transmission procedures of pictorial data is necessary.

Remote microscopy through the internet.

A fully robotic microscope Axioplan2 (Zeiss) and CCD camera (RGB-Sony) mounted on this microscope have been connected to a computer dedicated as an Internet server. This server is available by means of an Internet browser with an interpreter of the Java programming language. We have used Java technology for remote control this full-motorized telemicroscope. The Java program for microscope control is automatically downloaded and started when the user selects the Web site of the corresponding microscope server with his Internet browser. At the client's side, there is no need for any additional software except for what is included in Netscape Navigator 3.0, Microsoft Explorer 3.0 or its later versions. Our remote microscope can be remotely controlled from any place in the world through the Internet. The user can move the microscope stage, change the magnification or can execute any other microscope operations by pressing the related buttons of the downloaded telemicroscopy client program. The microscope server receives the commands from the client program, executes the operation on the microscope and after every new command distributes the next microscope image to the connected telemicroscopy clients. There are also software modules which allow marking a field of interest in the image and discuss. Every Internet user can control the remote microscope and discuss the images with others who have direct access to the Internet on-line, or are connected by modem or ISDN. The quality of the transferred microscopic images and response time is sufficient for practical use. No complicated installation processes are necessary and the system is easy to use. The system design makes every pathologist, who is the Internet user, to be a possible consultant.

Intelligent decision support in pathomorphology.

This paper presents a novel approach to computer-supported diagnosing based on microscopic images of histological sections. A method of extraction of textural feature is presented, which is in a sense complementary to the texture-based segmentation. The textural feature is obtained by tracing the process of image segmentation. For classification, a n2-classifier oriented to multi-class problems has been used. The paper presents also an empirical verification of the proposed approach on 700 microscopic images representing 14 classes of CNS neuroepithelial tumours, in which case an encouraging accuracy of classification on the testing set (70.6%) has been obtained.

[Sarcoidosis presenting as isolated nodules of the parotid and submandibular glands].

The authors documented the case of healthy, 50 year old women with a sarcoid lesions of the parotid and submandibular gland without any other manifestations of the disease. The presence of noncaseating granulomas in salivary tissue is a specific feature of sarcoidosis. Primary involvement of the intraglandular lymph nodes with a secondary involvement of the salivary gland parenchyma was detected. Etiopathogenesis, diagnostics and clinical features of sarcoidosis were presented.

[Diagnostic significance of percutaneous needle biopsy in metastatic neoplasms of the spine].

A retrospective review of percutaneous needle biopsy due to the vertebral metastatic neoplasms performed in 92 patients treated from 1975 to 1992 at Department of Neurosurgery, K. Marcinkowski University of Medical Sciences. Thoracic biopsies constituted 45 per cent, lumbar-32 per cent, cervical-18 per cent and sacral ones 5 per cent of all procedures. An accurate diagnosis was made in 70 per cent of all cases. The highest accuracy rate was noted in lesions compromising the entire vertebral body (84 per cent) and posterior elements of the vertebrae (89 per cent). Tomographic X-rays, CT and MRI were helpful in visualization of concomitant paravertebral mass. In these cases accuracy rate of the biopsy was 82 per cent. The technique of percutaneous biopsy and histological results were also discussed.

Diagnostic immunohistochemistry of tumors of the central nervous system.

The hopes of the early eighties for detecting a marker which would be specific for tumor cells as such or for a tumor of specific tissue or organ differentiation line have not been fulfilled. Thus, a tumor marker is a substance the level of which increases but which is not unique for tumor cells. The marker may be localized within the neoplastic cell or on its surface. The marker may also be produced by the tumor-surrounding tissue under the influence of the tumor. Therefore, the results of identifying the markers should be interpreted with great caution and should be always confronted with the routine results of light microscopy as well as other studies. This is so even if the results greatly enrich the diagnosis. The diagnostic procedures should be based also on fully standardized laboratory techniques (Taylor 1992).

NEURO multimedial data base and management system for neuropathology.

Implementation of multimedial data base and management system for cytological, biopsy, immunological, electronmicroscopy and autopsy examinations are described. Packet NEURO was implemented on IBM compatible computers runs under NetWare network system. The packet allows to realize all basic data base operations such as: entering, search, replace and deleting of records. Adding new records uses operations of projection, selection and merging relations include the formalized and coded SNOMed topography and morphology fields. The description fields, macro- and microscopic images are collecting and managing.

Multimedial data base and management system for self-education and testing the students' knowledge on pathomorphology.

The composition assumed our experience in creating and using multimedial data base of examination questions and management system, which is used for. This system is implemented on microcomputers compatible with IBM PC and works in network system Net Ware 3.11. The test questions exceeded 2000 until now. The packet consists of the two functionally individual programs: ASSISTANT, which is the administrator for the databases, and EXAMINATOR which is the executive program. This system enables to use text files and add images to each question, which are adjusted to display on standard graphics devices (VGA). Standard format of the notation files enables to elaborate the results in order to estimate the scale of answers and to find correlations between the results.

[PATHOLOGIST --software package for database management systems in pathology].

This article summed up 4 years experiences in the creation and working with software package for collection, processing and storage of data in the form of standard database. Software package--PATHOLOGIST--can be implemented on microcomputer compatible with IBM PC. This package manages 92 relative databases in the order to gather all possible information about performed histological, cytological, immunological examinations as well as autopsy.

[Clinical analysis of 27 cases of primary lymphoma of the central nervous system].

Clinical analysis of 27 patients with primary central nervous system lymphoma--Out of 4397 patients with intracranial neoplasms treated in the Department of Neurosurgery Medical Academy in Poznań between years 1956-1990, 27 patients i.e. 0.6% had primary central nervous system lymphoma. The patients age ranged from 15 to 63 years. In most patients the tumour was located in the temporo-parietal region. All patients were treated surgically, and then some received radio and/or chemotherapy. In spite of combined therapy the results of treatment are unsatisfactory, the median survival time is between 2 and 4 months.

[Traumatic and spontaneous cerebral hematoma].

The authors analyzed 231 patients--14% of all intracranial haematomas treated in the Department of Neurosurgery in Poznań between 1962 and 1990. The patients were divided into 3 following subgroups: I--cases of spontaneous haematomas (identified and unidentified)--30%, II--haematomas of unknown aetiology, but after mild head trauma--27%, III--traumatic haematomas--43%. Increased risk of mortality was observed in cases with intracerebral posttraumatic haematomas. These patients demonstrated diffuse cerebral oedema and multifocal contusions. In the subgroup of patients with spontaneous cerebral haematomas vascular malformation was present during operation or/and in histopathological study--65%.

[Multifocal oligodendroglioma. A case report].

A case of multifocal oligodendroglioma was described in a 51-year-old woman. Tumours were located in the parietal lobe of the right cerebral hemisphere and in the frontal lobe of the left cerebral hemisphere. Multifocal character and type of the neoplasm were determined by using computerized+ tomography and fine-needle biopsy with the patient alive. Histologic examinations of the surgical and autopsy materials confirmed multifocal development of oligodendroglioma.
