Acetylcholine and aromatic amine systems in postmortem brain of an infant with Down's syndrome.

Adult cases of Down's syndrome often show histologic and biochemical changes comparable to those seen in severe Alzheimer's disease, but it is not known whether these are congenital or acquired defects. Cell counts of the basal forebrain cholinergic system innervating the cortex in a 5.5-month-old male infant with Down's indicated about 50% of the number of cells expected at birth but this is in the range of cell numbers found in healthy middle-aged normals. The noradrenergic system of the locus ceruleus has the expected complement of cells for normal newborns. The activities of choline acetyltransferase (ChAT), acetylcholinesterase (AChE), glutamate decarboxylase, and tyrosine hydroxylase in a number of brain regions are reported for this infant, two cases of crib death, and a group of normal adults. The regional distributions of the enzymes in the infants were generally as expected from adult control data except for that of ChAT in one of the two cases of crib death; the AChE activities seemed extraordinarily high, especially in the case of Down's syndrome. Data on the concentrations of the catecholamines, serotonin, and their metabolites are also given but, like the enzyme data, are difficult to interpret in the absence of controls for the neonatal period.

Pontine haemorrhage: a clinical analysis of 26 cases.

Correlation with the size of the haematoma, neurological findings and prognosis was studied in 26 cases of hypertensive pontine haemorrhage, using CT. A good prognosis was found with no or mild disturbance of consciousness, normal pupils, and transverse diameter of the haematoma 20 mm or less. Most of the patients with poor prognosis became comatose within two hours of onset. Their pupils were dilated bilaterally, pin-point or anisocoric and the transverse diameter of the haematoma was over 20 mm.

CT-guided stereotactic aspiration of intracerebral hematoma--result of a hematoma-lysis method using urokinase.

CT-guided stereotactic aspiration was performed in the CT room on 97 patients with hypertensive intracerebral hematomas, using a standard ventricular cannula. Residual hematomas were liquefied by urokinase and aspirated through the drainage tube. Major and minor rebleeding were seen in 7 cases. Two out of the 4 major rebleeding cases were followed by craniotomy, while the other cases were treated conservatively. More than 80% of the hematomas were aspirated in 68 cases, 50-70% in 19 cases and 30-40% in 6 cases. Operation in the CT room and hematoma lysis with urokinase is very useful for the aspiration of intracerebral hematomas.

Recovery of vestibulospinal balance function after unilateral labyrinthectomy in patients with Ménière's disease.

The influence of unilateral labyrinthectomy on vestibulospinal balance function in patients with Ménière's disease is studied with a gravicorder. Our data disclosed that vestibulospinal balance recovery was better when tested in the light, including the total length and area. Moreover, this quantitative test which measures changes in the center of gravity in standing subjects revealed that total length recovery was almost achieved within 5 weeks when measured in the light but required 12 months when measured in the dark; total area was almost in the normal range within 1 week when measured in the light but required 5 weeks when measured in the dark.

Mouse granuloma pouch induced by Freund's complete adjuvant with croton oil.

A mouse inflammation model of chronic proliferative type was newly produced in this study. Among a great many methods examined in mice, only an air-pouch method with Freund's complete adjuvant with 0.1% croton oil obviously induced the granuloma pouch with well-developed vascularization, which a little differed from that seen in rats. Both granuloma formation and fluid exudation in this adjuvant pouch increased until one or two weeks after treatment, respectively, followed by reducing gradually to become nearly normal at 12 weeks after. The pathohistological study exhibited liquefaction necrosis in the earlier stage, followed by coagulation necrosis accompanied with inflammatory cells infiltration. The nascently sprouted capillaries were well-developed and then the fibrous tissue formation proceeded along with the vascularization. The granuloma finally changed to scars and callous tissues. Intrapouch injection of hydrocortisone and indomethacin decreased the pouch fluid and the pouch wall weight. In conclusion, the adjuvant pouch in mice may be a feasible experimental model for the pharmacological study on the vascularization in the acute and chronic inflammatory processes.

Pharmacokinetics of nipradilol (K-351), a new antihypertensive agent. II. Influence of the route of administration on bioavailability in dogs.

The pharmacokinetic parameters of nipradilol (NIP), a new potent antihypertensive and antianginal agent, and propranolol were determined after oral, intravenous and intraportal administration to the beagle dog implanted with cannula in portal vein at a dose of 1 mg/kg. Orally administered NIP underwent extensive first-pass metabolism leading to low bioavailability (11%), despite of complete gastrointestinal absorption. On the constant infusion for 30 min into the portal vein, hepatic extraction ratio was 0.71. The reduction in the systemic availability of orally administered NIP could partly be attributed to the fact that denitration and glucuronidation of NIP occur primarily in the intestinal tract and liver, respectively. Following oral administration of NIP, smaller amount of unchanged drug (1.9%) was excreted into the urine than that of intravenous administration (5.8%). However, in the qualitative and the quantitative aspects on urinary metabolic patterns, there was no appreciable influence of the route of administration. On the other hand, the systemic availability and the hepatic extraction ratio of propranolol were 11% and 0.86, respectively, suggesting that the first-pass metabolism through the liver actually contributes to the reduced availability.

Pharmacokinetics of nipradilol (K-351), a new antihypertensive agent. I. Studies on interspecies variation in laboratory animals.

The pharmacokinetics, plasma protein binding and metabolism of nipradilol (K-351: NIP), a new potent antihypertensive and antianginal agent, were compared in dogs, monkeys, rabbits and rats. In all species studied, NIP did not appreciably bind plasma protein (less than 30%) and was extensively distributed in tissues. There was a good correlation between the volume of distribution at steady state (Vss, 1) and body weight (B, kg) of the animal species as follows: Vss = 4.42 B0.805. In addition, Vss increased as a function of the plasma free fraction. Intrinsic clearance of unbound drug (CLuint, 1/h) also correlated with body weight as follows: CLuint = 4.78 B0.722, but blood clearance in rabbits exceeded hepatic blood flow, suggesting extrahepatic metabolism. Following oral administration, the systemic availability for all species increased with the oral dose, while the half-life was about 2 h, and was independent of dose. The apparent threshold dose (ATD, mg/kg) was observed to vary inversely with body weight of the animal species as follows: ATD = 4.33 B-0.472. Less than 2% of the dose was excreted into the urine as unchanged NIP in all species. The metabolic profile for all species was similar, but pronounced quantitative differences among species was observed for aliphatic and aromatic hydroxylation of the 3,4-dihydro-2 H-1-benzopyran ring.

Effect of forskolin on prostaglandin productions in isolated dog renal arteries.

Forskolin (1 to 100 microM), a direct activator of adenylate cyclase, did not have any effect on prostaglandin E2 and I2 productions in isolated dog renal arteries. However, forskolin at the lower concentrations (10 and 100 nM) markedly stimulated only prostaglandin E2 production. 8-Bromo-cyclic AMP (0.5 and 1 mM) failed to stimulate prostaglandin E2 and I2 productions. The results suggest that 1) forskolin stimulates only prostaglandin E2 production, not through the activation of adenylate cyclase and 2) the prostaglandin production system may be independent of the cyclic AMP-generating system in isolated dog renal arteries.

Determination of urine neuron-specific enolase levels in neuroblastoma patients.

Urine levels of neuron-specific enolase were determined in 3 neuroblastoma patients (1 in an advanced state and 2 in remission), 25 control children, 37 control adults and 4 children with hematuria by means of the double-antibody inhibition radioimmunoassay specific to the gamma subunit of enolase isozymes. The levels of neuron-specific enolase mean +/- S.D. ng/creatinine mg in an advanced neuroblastoma patient were elevated (1.25 +/- 0.29 before or after treatment and range 1.61-74.2 during treatment) when compared with those of control subjects (0.51 +/- 0.26 in children and 0.36 +/- 0.17 in adults). The levels in 2 neuroblastoma patients in remission were within normal range. Urine samples with hematuria were not used for the assay.

Diffuse form of primary leptomeningeal gliomatosis. Case report.

Primary leptomeningeal gliomatosis is rare, and the diffuse form is even rarer with only three cases reported in the literature. A fourth case is described in this report. Computerized tomography (CT) findings showed hydrocephalus with enhancement of the cerebral cisterns, and analysis of cerebrospinal fluid obtained by lumbar puncture showed many atypical cells. Based on these findings, a diagnosis of leptomeningeal tumor was made. There was some improvement in neurological and CT findings following radiotherapy and chemotherapy.

Skull metastasis of thyroid carcinoma. Study of 12 cases.

Over the past 33 years the authors have treated 12 cases of skull metastasis from thyroid carcinoma, accounting for 2.5% of a total of 473 cases of thyroid cancer. A study of these 12 cases revealed the following characteristics. The mean age of the patients was 60.4 years and the predominant incidence was in the seventh decade of life. The incidence of skull metastasis from thyroid cancer was higher among women than among men. The primary sign was a soft hemispheric tumor resting on the skull. The tumors were rich in vascularity with osteolytic changes in the skull. The average period from diagnosis of the thyroid tumor until thyroidectomy was 14.3 years, and until discovery of the skull metastasis was 23.3 years. The clinical course was thus very long. The most frequent histopathological presentation was follicular adenocarcinoma. Such lesions were morphologically well differentiated, with little pleomorphism and atypism, but detailed examination showed infiltration into the vascular lumen or capsule. The mean survival time in these patients was 4.5 years from the time of diagnosis. The prognosis in this lesion was relatively poor, considering its long clinical course.
