The partially implantable middle ear implant, case reports.

The partially implantable middle ear implant (MEI) is clinically useful for bilateral deafness when it is applied to the mixed deafness with a bone conduction threshold of 20-40 dB. Two out of five successful cases are reported. It should be noted that 1.5 years have passed without changes for the first successful implantation. The next step for application of the MEI to more cases is discussed.

Subarachnoid hemorrhage of unknown cause.

After the institution of computed tomography (CT), 814 cases of spontaneous subarachnoid hemorrhage (SAH) were treated during a period of 6 years and 9 months (April 1978 through December 1984). In 9 (22.0%) of 41 patients whose cause of SAH was not determined by the first four-vessel study (cerebral panangiography), ruptured aneurysms were found by repeated four-vessel study. Thus, of 814 cases, only 32 (3.9%) were diagnosed as cases of unknown etiology at discharge. The 32 cases were monitored by follow-up examination for 5 to 67 months (median, 27.1 months) after onset. No recurrence of SAH was reported, and all patients were rehabilitated except 2 who suffered terminal carcinoma. A third follow-up four-vessel study was performed 8 to 44 months (median, 22.0 months) after the second study in 14 of the 16 patients with SAH initially demonstrated by CT. In 1 of these cases, an aneurysm found 9 months after the initial SAH was treated surgically. Ultimately, 31 cases (3.8%) were diagnosed as cases of SAH of unknown cause. This incidence is low when compared with those in previous reports. Because of the strict examination schedule including repeated angiography, the incidence is lower and the prognosis is relatively favorable.

Computed tomography-guided stereotactic aspiration of posterior fossa hematomas: a supine lateral retromastoid approach.

We report a supine lateral retromastoid approach for computed tomography-guided stereotactic aspiration of hematomas of the posterior fossa. The method involves horizontal insertion of the aspiration needle at the suboccipital region with the patient in a supine position with the head turned 30 to 40 degrees away from the lesion and with slight forward flexion of the neck. Eleven hematomas have been removed through this approach thus far, nine cerebellar and two pontine. With this technique, general anesthesia is not needed and operative invasion is minimal, allowing hematoma aspiration in patients in whom operation would previously not have been indicated. An additional merit is the suitability of this method for elderly patients.

Involvement of glucocorticoid in insulin-induced angiogenesis of adjuvant pouch granuloma in diabetic mice.

The effects of insulin and glucocorticoid on granuloma formation and angiogenesis were studied using an adjuvant pouch in mice and a carrageenin pouch in rats. Carrageenin-induced granuloma formation was suppressed in the diabetic state induced by alloxan in rats. The suppression was restored by adrenalectomy. Corticosterone counteracted the restoration, whereas epinephrine did not, suggesting that the effects of adrenalectomy are due to the lack of adrenocortical hormones rather than epinephrine. The blood corticosterone levels in alloxan mice increased, following the increase of glucose level, to more than 20 micrograms/dl after 4 weeks. The increase of corticosterone levels disappeared after adrenalectomy. In the mouse adjuvant pouch, corticosterone dose-dependently decreased granuloma formation and angiogenesis. The values of these two parameters obtained with a dose of 20 micrograms/pouch corticosterone agreed with those of the serum levels in diabetic mice. Insulin dose-dependently reversed the suppressed angiogenesis of 20 micrograms corticosterone-treated mice and the dose-response curve approximated the curve of alloxan-treated mice. The effects of diabetes were concluded to involve insulin deficiency as well as glucocorticoid enhancement and the counteraction between the two may control granuloma formation and angiogenesis.

Comparative effects of two forms of gamma-oryzanol in different sterol compositions on hyperlipidemia induced by cholesterol diet in rats.

Hypolipidemic effects of the usual gamma-oryzanol (gamma-OZ) and a new gamma-OZ (N-gamma-OZ) with a different sterol composition from gamma-OZ were investigated on the hyperlipidemia induced by ingestion of a high cholesterol diet (HCD) containing 1% cholesterol for 12 days in male Sprague-Dawley rats. Treatment with gamma-OZ for 6 days significantly inhibited the increase in serum total cholesterol (TC) and phospholipids (PL) induced by HCD, while the treatment with gamma-OZ for 12 days did not inhibit the increase of TC and PL. Treatment with N-gamma-OZ at 100 or 1000 mg/kg for 6 days slightly inhibited the increase of TC by HCD. The decrease of TC in high density lipoprotein (HDL-TC) was markedly inhibited by treatment with N-gamma-OZ for 12 days, but N-gamma-OZ for 6 days and gamma-OZ for 6 and 12 days did not inhibit the decrease of HDL-TC. Treatment with N-gamma-OZ for 12 days significantly inhibited the increase of PL and free cholesterol (FC) by HCD. gamma-OZ at 1000 mg/kg for 12 days also inhibited the increase of FC. N-gamma-OZ significantly reduced the atherogenic index using TC and HDL-TC by affecting the HDL-TC increase. gamma-OZ at 100 mg/kg and N-gamma-OZ at 100 mg/kg for 6 days reduced the atherogenic index using TC and HDL-TC by the inhibition of TC increase. The atherogenic index using PL and HDL-PL was only reduced by the treatment with N-gamma-OZ at 1000 mg/g for 12 days. The increase of triglyceride (TG) by HCD was inhibited by the treatment of N-gamma-OZ for 6 days (all doses) and 12 days (500, 1000 mg/kg), and gamma-OZ at 500 mg/kg for 6 and 12 days also inhibited the increase of TG by HCD. gamma-OZ and N-gamma-OZ had no effects on liver lipid contents. The hypolipidemic effect of N-gamma-OZ was slightly more potent than that of gamma-OZ.(ABSTRACT TRUNCATED AT 250 WORDS)

Insulin resistance of adjuvant-induced granuloma pouch formation in genetically diabetic KK-CAy mice.

The characteristics of adjuvant-induced pouch granuloma in genetically diabetic KK-CAy mice with hyperinsulinemia were investigated. Both the dose-response relationship and the time-course experiments showed that the wet weight of pouch granuloma in diabetic KK-CAy mice was lower than in ddY normal mice. Insulin treatment enhanced granuloma formation in KK-CAy mice, and it restored the suppressed DNA content in the granuloma tissue to the level in ddY mice. Although the DNA content was dose-dependently increased by insulin, the ratio of DNA content to granuloma weight was constant. In severely diabetic mice, the granuloma weight was not different from that in normoglycemic mice, despite significantly higher blood insulin levels and greater body weight. Insulin stimulated granuloma formation in severely diabetic KK-CAy mice only when higher doses (1 mg/kg) were given. This evidence suggests that suppression of granuloma formation in diabetic KK-CAy mice is due to insulin resistance and that restoration requires pharmacological doses of insulin.

Insulin-induced granuloma tissue formation and angiogenesis in alloxan-treated diabetic mice.

Pouch granuloma formation induced by Freund's complete adjuvant containing 0.1% croton oil was studied in both normal and alloxan-induced diabetic mice, and was found to be significantly suppressed in the diabetic group. Insulin repeatedly injected into the pouch facilitated granuloma formation dose-dependently, especially in the diabetic mice. The topical insulin treatment did not affect blood glucose levels. The suppression of granuloma formation by diabetes and its reverse by insulin treatment were verified by histological findings in the granuloma pouch wall. Characteristic changes in neovascularization occurred in the pouch wall. The hydroxyproline content in the granuloma tissue in diabetic mice was not significantly enhanced by the insulin treatment. This indicates that differences in collagen production in normal and diabetic mice were not the critical factor affecting granuloma formation. It was concluded that the decreased granuloma formation in the diabetic state was due to the lack of insulin as a growth factor, and that angiogenesis induced by insulin preceding collagen fiber formation may play an essential role in granuloma formation.

A clinicopathologic correlation study of thallium-201 myocardial scintigraphy in diagnosis of myocardial infarction.

In a series of 1,000 consecutive autopsy cases, we evaluated the clinical utility of thallium-201 (Tl-201) myocardial scintigraphy and electrocardiography (ECG) in 101 patients who had been studied while alive. Fifty-five cases had myocardial infarctions (MI) at autopsy. The Tl-201 scintigram and ECG in diagnosis of MI showed sensitivities of 68% and 60%, specificities of 87% and 83%, and diagnostic accuracies of 76% and 70%, respectively. The sensitivity of the Tl-201 scintigram was 70% in anterior MI, 80% in postero-inferior MI, 25% in lateral and subendocardial infarction. The sensitivity was 88% for large massive MI, but was low in scattered (50%) or middle-sized MI (17%). The diagnostic limit of the resolution of Tl-201 scintigrams was 4.5 cm in long diameter. All 8 cases with MI of less than 4 cm could not be diagnosed with the technique. There were 48 cases of large MI (more than 5 cm), but 8 cases could not be diagnosed by scintigraphy because of non-transmural or scattered MI. A comparison of the Tl-201 scintigram and ECG showed that 27 cases out of 60 cases were diagnosed by both methods, 14 only by the Tl-201 scintigram, 9 only by ECG and 10 by neither method.

Colonic motility, autonomic function, and gastrointestinal hormones under psychological stress on irritable bowel syndrome.

Effects of an artificial mental stress on colonic motility, autonomic nervous system, and gastrointestinal hormones were examined in patients with irritable bowel syndrome (IBS). The subjects were 20 patients with typical IBS and 12 controls. A transducer was inserted to the sigmoid colon from the anus for measuring colonic intraluminal pressure, and mirror drawing test was loaded as psychological stress. At the same time, coefficient of variation of R-R interval on ECG (CV-RR) was measured and the levels of plasma catecholamines, gastrin, glucagon, and motilin were assessed. Colonic motility showed a significant increase in the IBS patients during the stress compared with that in controls (p less than 0.01). Motilin also increased significantly in the IBS patients after the stress (p less than 0.01). CV-RR and motilin revealed positive relationship with colonic motility alteration in the IBS patients although no significant change was detected in controls. These phenomena are thought to be due to autonomic nervous dysfunction and/or gastrointestinal hormonal derrangments induced by psychological stress. It is suggested that organ specificity of the alimentary tract for the stress exists in this disease.
