[Encysted gossypiboma of the lesser omentum: complementary aspects of echography and computerized tomography].

The authors report the fortuitous demonstration of an asymptomatic gossypiboma during the preoperative staging of a rectal carcinoma 13 years after abdominal surgery for perforated gastric ulcer. Ultrasonography disclosed a well-defined hypoechoic mass containing highly echogenic foci with a strong posterior shadow. The CT findings are reported, as well as the complementary aspects of sonography. Differential diagnosis from tumors of the lesser omentum is also presented.

Pulmonary sequestration-like anomaly presenting as a spontaneously resolving mass.

A premature baby boy presented at birth with respiratory distress. A right lower lobe opacity was found on chest X-ray. Pulmonary sequestration was the diagnosis suggested by ultrasound. The size of the mass decreased on subsequent examinations. 4 months later X-rays and sonography failed to demonstrate any remaining mass. This case illustrates another example of involutive pathology and suggests that not all pulmonary sequestration-like anomalies should be operated on.

Transverse geniculate ligament of the knee: appearance at plain radiography.

Visualization of the transverse geniculate ligament of the knee in four patients is described. This normal variant was seen on lateral plain radiographs as a small, rounded opacity surrounded by fatty tissue and located anterior to the knee joint. Magnetic resonance imaging helped confirm this image to be that of the transverse geniculate ligament.

[Desmoid tumor and Gardner syndrome].

A desmoid tumor was found in a patient with Gardner's syndrome. The tumor was visualized by CT before and after contrast injection, by radiographic examination of the small bowel, and by sonography. CT demonstrated a heterogeneous mass in the left lower abdomen with attenuation values ranging from 23 to 72 HU on native scans, enhancing to 123 up to 172 HU after injection of contrast medium. This examination also demonstrated the extent of the tumor and its exact dimensions. The radiographic examination of the small bowel completed the diagnosis and showed no involvement of the small bowel. This diagnosis was confirmed by laparotomy.

[The value of current imagery techniques in dentistry and stomatology].

Computed tomography, ultrasonography and magnetic resonance imaging are currently used in stomatology and dentistry. These recent imaging modalities have suppressed or reduced classic examinations (by example: conventional tomography and sialography). Efficient, exact and fast, they improve the diagnostic capacities and optimize the choice of the treatments. After some technical considerations, their applications in various stomatology and dentistry problems are evoked.

[Iatrogenic femoral pseudo-aneurysm and arteriovenous fistula. Non-surgical treatment].

Pseudoaneurysm and arteriovenous fistulae are classical but unusual complications of femoral catheterism. The use of large diameter catheters, heparinotherapy and fibrinolysis constitute risk factors. These local complications are becoming more frequent resulting from an increase in interventional techniques. Therefore, a non invasive management of these lesions becomes necessary. A simple manual percutaneous compression of the puncture site was successful as treatment of 3 arteriovenous fistulae and 4 of 7 pseudoaneurysms. That is the reason why the authors consider that this technique constitutes a good alternative of the surgical treatment.

[NMR evaluation of neoplastic invasion of the pulmonary artery].

Bronchial tumors that invade the mediastinum are not necessarily inoperable. Whether surgery is possible depends, among other things, on the extent of pulmonary artery invasion. The authors have studied the value of cardiac-gated MRI and compared it with CT and venous DSA for staging tumor invasion. CT demonstrated the areas of contact between tumor and mediastinum. The MRI planes were transverse and also in the main axis of the pulmonary arteries. Twenty-one patients were studied and in 16 the findings could be checked during surgery. In all cases the findings on MRI were confirmed. In eight patients MRI provided more information than CT and DSA combined and thereby showed its superiority for evaluating arterial invasion.

Magnetic resonance imaging of the normal prostate at 1.5 T.

Prostatic magnetic resonance images of 22 male volunteers less than 30 years old and with no known genito-urinary tract disease were obtained at 1.5 T. Normal anatomical features of the prostate were studied with spin-echo techniques. Different zones of the normal gland are shown by T2-weighted images: the anterior fibromuscular fascia, the central prostate, the peripheral prostate and the periurethral zone can be differentiated. The normal prostate gland is shown on T1-weighted images as a homogeneous appearance. It is important to recognize the normal zonal anatomy of the prostate since prostatic disorders arise in different anatomical zones.

Cysts of the prostate and seminal vesicles: MR imaging findings in 11 cases.

Cysts of the prostate and seminal vesicles are confusing abnormalities because they are uncommon and their origin is uncertain. Several approaches to diagnosis have been used, most recently CT and transrectal sonography. In this study, we investigated the value of MR imaging in the diagnosis of six cases of prostatic cyst and five cases of cysts of the seminal vesicles. All patients were symptomatic. T1- and T2-weighted spin-echo images were obtained in all cases; pathologic confirmation was available in four patients. MR images were analyzed prospectively without knowledge of clinical data or the results of other imaging procedures. In all cases, low- and high-signal masses were observed on T1- and T2-weighted images, respectively. The cysts were unilocular with a sharply defined margin, ranging from 0.5 to 3.0 cm in diameter. The thin wall was of low signal intensity on T2-weighted images. Our experience suggests that MR imaging is useful in demonstrating the liquid content of prostatic and seminal-vesicle cysts and in establishing their size and location.

Perinatal renal vein thrombosis. Sonographic demonstration.

The use of ultrasound (US) in the diagnosis of renal vein thrombosis (RVT) remains ill defined because the classical features lack specificity. The authors report three cases of renal vein thrombosis with a common US pattern: hyperechoic streaks in the interlobulary spaces confirming previous reports with the same pattern. The pattern has been observed in neonates as well as in utero. Associated vena cava thrombosis was present in two cases. This sign might be a specific sign of RVT.

[MRI of acute myocardial infarction with injection of Gd-DOTA (15 patients)].

We studied 15 patients 4 to 8 days after myocardial infarction by using ECG gated MR before and after administration of 0.2 mmol/kg Gd-DOTA. The diagnosis in each patient was confirmed by electrocardiographic criteria, elevated levels of fractionated creatine kinase (CK) isoenzyme, thallium scintigraphy, ventriculography and coronarography. T1-weighted, spin-echo images, were obtained before and immediately after injection of Gd-DOTA and were repeated 15 min later. The site of infarction was visualised in 10 patients as an area of high signal intensity after the injection of Gd-DOTA. Contrast between normal and infarcted myocardium was greatest 15 min after injection. Three patients were excluded because of failure to acquire adequate MR studies. In 2 other patients, the infarct were not detected. Before injection of Gd-DOTA, only 2 infarcts were detected. These results suggest that Gd-DOTA can improve MR visualisation and detection of acute myocardial infarction.

[Percutaneous transluminal coronary angioplasty: short- and long-term results].

We reviewed the results of percutaneous transluminal coronary angioplasty (PTCA) in 200 consecutive patients from January 1988 to January 1989. The mean age was 55.8 years. Twenty-two per cent had unstable angina, 66% had stable angina and the other 12% had atypical chest pain or were asymptomatic after a myocardial infarction. Five percent had left ventricular function less than 45%. The angioplasty procedure was angiographically successful in 184 patients (92%). There was no significant difference in success rate in the different vessels or indications. Coronary bypass surgery was required in 3% of patients as an emergency procedure; myocardial infarction occurred in 2% less than 24 hours after the procedure. There has been no in-hospital death. The recurrence rate of ischemic symptoms was 26.5%. Considering lesions treatment, the procedure was successful in 71% of the 200 patients over a long-term follow-up period.

Deep dorsal vein arterialisation in vascular impotence.

A series of 12 patients with vasculogenic impotence (4 arterial lesions; 8 arterial and venous lesions) underwent deep dorsal vein arterialisation after pre-operative assessment by a multidisciplinary approach. Cumulative graft patency was 58% (7 of 12 patients) up to 21 months but only 4 patients developed almost normal erections. Digital angiography, with and without the intracavernous injection of papaverine, was performed during follow-up to determine the vascular physiological status. At flaccidity, the corpora cavernosa were never opacified in the absence of a venocorporeal shunt. The penile glans was always visualised. Opacification of the deep dorsal vein and the circumflex system decreased with penile rigidity, resulting from their compression between Buck's fascia and the tunica albuginea. Intracavernous pressure recorded before and after the surgical procedure showed a marked increase when a caverno-venous shunt was performed. Hypervascularisation of the glans occurred in 2 cases. The relevance of this new surgical technique and its functional mechanism are discussed.

Percutaneous drainage of subphrenic abscesses.

Forty subphrenic abscesses were drained percutaneously in 37 patients. These abscesses were subsequent to abdominal surgery (28 patients), biliary obstruction and/or fistula (six patients), trauma (two patients) or acute pancreatitis (one patient). The procedures were usually monitored by duplex sonography and fluoroscopy (35 out of 40 cases), with an angled subcostal approach to the subphrenic space in over 85% of the cases. Forty-three drainage catheters were inserted using the Seldinger method; 37 (86%) had a size of 12 F or more, 21 were 16 F in size. Only two out of 43 catheters were double-lumen sump drains. The clinical condition improved after insertion of the catheter in all 37 patients. Definitive successful drainage, defined as hospital discharge without complementary surgery, was achieved in 32 patients (87%), without major complications. A temporizing effect was obtained in three additional patients (8%) who underwent curative surgery for the underlying process after complete drainage of the abscess. Two patients died from multiple organ failure before their abscess healed. Considerations for successful drainage of subphrenic abscesses include a good knowledge of the subphrenic space anatomy, the use of large-bore drainage catheters and the recognition and correct management of underlying enteric, biliary or pancreatic fistulas.

[Laser assisted angioplasty].

Percutaneous laser thermal angioplasty with a hot tip fiber was used in 26 patients to recanalize 20 femoropopliteal occlusions, 5 iliac thromboses and one occlusion of the left subclavian artery. Initial angiographic success was achieved in 85% for the femoropopliteal lesions and in 60% for the iliac occlusions. Eight complications did occur, but none necessitated emergency bypass surgery.
