Educating child care providers in child health.

As demand increases for quality child care, less-than-adequate care has been provided to children due to, among other reasons, the lack of a national child care policy. The Childhealth Education Program (CEP) addresses the needs of child care providers in upstate New York using the services of nurse practitioners, who are well prepared to provide child education.

Pseudarthrosis of the radius associated with neurofibromatosis. A case report.

A seven-year-old-boy with neurofibromatosis developed pseudarthrosis of the radius. After treatment by external electrical stimulation of the forearm failed, the pseudarthrosis was excised and the proximal and distal ends of the radius were osteotomized. The bone segments were reversed to correct the deformity and to place bone adjacent to the pseudarthrosis in contact with normal bone. This operation has not been described previously for this condition and, as in all previous reports, failed to produce union. Surprisingly, the excised pseudarthrosis showed no histologic evidence of neurofibroma or impaired osteoblastic function. The surrounding soft tissues may play an important role in the establishment and maintenance of the condition.

Treating keratoconus.

Keratoconus is one of the most commonly seen corneal dystrophies. It manifests in the early teens, one eye becoming affected before the other. Treatment includes the use of glasses, contact lenses and, finally, penetrating keratoplasty (corneal graft). This care study highlights how an active young man with a future sporting career needed to come to terms with his eye condition.

Pulmonary mechanics generated by positive end-expiratory and continuous negative pressure.

Pulmonary function measurements were studied on equivalent levels of positive end-expiratory pressure (PEEP) and continuous negative pressure (CNP) while controlling for transpulmonary pressure (TPP). Four adult rabbits were anesthetized, instrumented, and ventilated with intermittent mandatory ventilation by using peak inspiratory pressure (PIP) of 16 cm H2O, PEEP 0 cm H2O, CNP 0 cm H2O, inspiratory time 0.3 seconds, rate 20/min, and fraction of inspired oxygen of 0.3. Subsequently, equal amounts of PEEP and CNP were alternated for 15-minute ventilation periods. PIP was changed to approximate the TPP in each PEEP/CNP pair. There was a significant decrease in PCO2 and increase in pH, mechanical tidal volume, minute ventilation, functional residual capacity, and total dynamic compliance on CNP. These differences could not be explained by changes in TPP.
