Hormone serum levels and hormone receptor contents of endometria in women with normal menstrual cycles and patients bearing endometrial carcinoma.

Serum levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), prolactin (HPRL), 17 beta-estradiol (E2) and progesterone (P) were estimated in 46 subjects with normal menstrual cycles in whom hysterectomies were performed. Estrogen (ER) and progesterone receptor (PgR) levels in endometrial samples of these patients were estimated, and histological dating of the cycle day was carried out. Similarly, hormone serum levels and ER as well as PgR were estimated in 17 patients with endometrial carcinoma. No correlation between LH, FSH, HPRL and ER as well as PgR was noted in the normal subjects. Correlation between P and ER was observed in this group. Parallel variations between E2 and PgR were recorded in the normal females. In the carcinoma group no correlations between hormone serum levels and receptor contents were found, but ER and PgR correlated with each other. Receptor levels was highest in the well-differentiated group of endometrial carcinoma. The present experiments provide a rationale for progestagen therapy of carcinoma of the endometrium.

The influence of burn trauma on the hypothalamus-pituitary axis in normal female subjects.

In 5 severely burned women (greater than 25% TBS, II degrees and III degrees burns) the following hormones were investigated by RIA. (1) LH-releasing hormone test in the initial phase of burn shock with 100 gamma 3 LH-RH. (2). Follow-up controls of luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin (HPRL), oestradiol (E2), progesterone (P) and testosterone (T) in the catabolic and healing phase of burn disease. All women were before onset of burn trauma regularly ovulating. The following preliminary results can be presented: If the burn trauma occurs in the preovulatory phase of the menstrual cycle, there was no ovulation during the time of observation. As a sign of the normal function of the anterior lobe of the pituitary gland all subjects responded with an increase of plasma LH to the stimulation with LH-releasing hormone. The burn trauma interferes with the female endocrine equilibrium by involving hypothalamic centres. Negative feedback mechanisms by the secretion of adrenal hormones and temporary hyperprolactinemic states as a further result are discussed.

Transformation and translocation of 17 beta-estradiol receptor in rat pituitary.

The association of 17 beta-(3H)-estradiol with its pituitary cytosol receptor is described. The transformation products of this 8 S receptor complex at high salt concentration was analyzed by 5-20% linear sucrose gradients. KCl was found to reversibly convert the 8 S moiety into a 4-5 S species. The transformation by CaCl2 and trypsin was found to be irreversible. In addition, the nuclear translocation of the estrogen-receptor complex was investigated in vitro. Translocation was found to be temperature-dependent, since a 6 S nuclear receptor complex could be extracted from crude nuclei only after incubation of whole pituitaries at 37 degrees C and not at 4 degrees C incubation temperature. Similarly, a 6 S species was found upon sucrose gradient analysis of nuclear extract after a cell free incubation of isolated nuclei and cytosol with 17 beta-(3H)-estradiol. Maximal translocation was observed in this in vitro system within 60 to 90 min, resulting in a concomitant loss of cytosol receptor. The present data combine to suggest that nuclear translocation resembles one of the early events involved in the molecular action of estrogen at the pituitary level.

[Hormonal and functional parameters in hyperprolactinaemia (author's transl)].

58 women with raised serum prolactin levels and normal hypophyseal X-ray parameters were subdivided into three groups according to the serum prolactin level (16 to 40, 41 to 80, greater than 80 ng/ml). In addition to the determination of LH, FSH, oestradiol, progesterone and testosterone in the serum and thyroid diagnostic procedures the following hormonal tests were performed: 1. response to gestagen; 2. response to clomiphene; 3. Gn-RH-test for hypophyseal function; 4. ovarian response to administered gonadotropins (only in cases with prolactin levels administered gonadotropins (only in cases with prolactin levels greater than 80 ng/ml). The grade of menstrual cycle disorders depends on the severity of the observed hyperprolactinaemia. Slight disorders like luteal phase insufficiency, anovulatory cycle and oligomenorrhoea are associated with low- or medium-grade hyperprolactinaemia. Mainly secondary, but also primary amenorrhoea is found in cases with higher serum prolactin levels (greater than 80 ng/ml). In these cases the hypophyseal response to Gn-RH is frequently found (61%) to be negative and, moreover, the ovarian response to administered gonadotropins seems to be diminished. Increasing HPRL levels often appear to be associated with a negative response to the other above-mentioned tests.

Changes of serum testosterone and of LH-RH test after treatment of cryptorchidism by intranasal LH-RH.

The aim of the present investigation was to study, in a collaborative double-blind study, the treatment with intranasal LH-RH application of boys aged 1 1/2 to 12 years, who suffered from unilateral or bilateral cryptorchidism. A total of 88 subjects were randomly and blindly allocated to LH-RH and placebo therapy. Before and after 4 weeks of treatment basal testosterone serum levels were estimated and LH-RH tests were performed. Intranasal treatment with LH-RH resulted in partial or complete descents of testes in 23 out of 88 patients, whereas the position of testes remained unchanged in 17 subjects. 42 boys did not respond to placebo therapy, but complete descents were observed in 6 boys of the placebo group. Hormone analysis data of 4 different laboratories were recorded and statistically evaluated. No changes of LH, FSH and testosterone were found in the placebo groups. Only patients of the Frankfurt group responded to LH-RH test with augmented LH release after therapy (P less than 0.05). All patients treated with intranasal LH-RH showed a significant decrease of FSH release after therapy (P less than 0.05 to P less than 0.01). Basal testosterone serum levels were found to be increased after therapy only in the patients treated at Zürich (P less than 0.05). Data of the present study combine to suggest that chronic application of LH-RH may result in an overstimulation phenomenon, and that LH-RH test as well as basal testosterone levels cannot be used as prognostic index of therapy efficacy.

[Serum oestriol: a parameter of the function of the feto-placental unit (author's transl)].

The determination of serum oestriol is known to be a useful parameter of the function of the feto-placental unit. In order to derive the normal range on application of the radioimmunoassay technique, the serum oestriol level was determined in 242 gravidae during the second half of pregnancy. The pregnancies were all uncomplicated and ended with the delivery of a healthy baby at term. The statistical mean and 4 confidence intervals were calculated for each gestational week individually. During the second half of pregnancy there was a continuous gradual increase in serum oestriol; between the 34th and 38th gestational week there was a further steep increase, followed by a decrease from the 39th gestational week onwards. The usefulness of this method is illustrated on the basis of case reports of 10 high-risk pregnancies in which oestriol determination is compared with the clinical findings and HPL determination.

The 'LH/HCG test', a valuable aid in the diagnosis of tubal pregnancy.

174 patients with suspected tubal pregnancy were thoroughly examined. The diagnosis was confirmed in 102 women. As the usual noninvasive and invasive examination methods proved to be rather inaccurate, a new procedure, the 'LH/HCG test', was evaluated. Results were positive in all our cases. It was therefore concluded that this assay should receive primary consideration when tubal pregnancy is in question.

Maturation of 17 beta-estradiol binding protein in the hypothalamus and anterior pituitary of male rats.

Levels of estrogen binding proteins in the cytosol of hypothalamic and pituitary tissue of male rats at different ages were estimated. Two classes of estrogen binding sites were detected in 13 and 20 days old animals. The number of high affinity estrogen binding sites (K = ca. 2 x 10(-10) mol) of the hypothalamus was slightly larger in 13 days old animals, then decreased to constant levels at the age of 20 days and no changes were observed thereafter. Levels of low affinity binding sites of the hypothalamus (K = ca. 5 x 10(-9) mol) decreased with age and were no longer detectable in 30 days old animals. Similarly, a decrease of low affinity estrogen binding sites with age was recorded for pituitary cytosol samples and this class of receptor sites was no longer observable in 30 days old male rat. At 13 days high levels of high affinity 17 beta-estradiol binding protein was registered and minimal numbers of binding sites were found at 20, 30 and 40 days. Thereafter, the levels remained constant until 100 days and were similar to values observed in the 13 days old male animals. Apparent affinity constants of two classes of binding sites in hypothalamus and pituitary were similar...

[Comments on the diagnosis of tubal pregnancy (author's transl)].

51 women with suspected tubal pregnancy were admitted to our hospital during the past two years. Tubal pregnancy was present in 31 women. Diagnosis is rarely accurate with the conventional examination methods. The "LH/HCG" test was positive in all of our patients. Hence it appears resonable, whenever tubal pregnancy is suspected, to combine several noninvasive examination methods, special preference being given to the LH/HCG test.

[Contraceptive efficiency of triphasic inhibitors (author's transl)].

The effects of two newly developed triphasic preparations with a sequence of 6/5/10 and a reduced content of the total dose of oestrogen per cycle on the parameters of cycle function were studied in 10 young women with normal phasic cycles using a randomized trial design. Cervical function, spinnbarkeit and crystallisation of cervical secretion and the karyopyknotic index were monitored and LH,E2 and progesterone levels in serum radioimmunologically determined in a pretreatment control cycle, the immediately following 1st treatment cycle, the 3rd treatment cycle and a subsequent treatment-free cycle. Both preparations brought about suppression of ovulation already in the 1st treatment cycle with the periphery (cervical barrier) greatly reduced but still reactive. The results of the study show that by adapting the ratio of active substances to the phases of the normal cycle, not only good cycle control and tolerance are achieved but also inhibition of ovulation.
