Joint debridement in the treatment of advanced hemophilic knee arthropathy.

Joint debridement by a meticulous surgical technique has been performed for advanced knee arthropathy in three hemophiliacs between the ages of 22 and 27. The results at follow-up evaluation, ranging from one to five years, have been surprisingly good. The procedure involves comparatively little surgical trauma and appears to have a low rate of complications. Knee joint debridement should be considered in the young hemophilic patient as a means of avoiding or at least delaying more extensive operations such as arthrodesis or arthroplasty. Joint debridement may give the patient years of pain-free, independent, and gainfully employed life with a minimum of surgical trauma.

Camptocormia.

Camptocormia is a conversion type of hysteria, found mainly in recruited soldiers. The condition is characterized by powerful hyperflexion of the trunk and pains in the lumbar region. Characteristically, the anthropoidal posture disappears entirely during sleep when the patient is lying on the back. Knowledge of the syndrome permits an easy diagnosis and treatment.

Treatment of patients with unstable fractures of the thoracic and lumbar spine: a follow-up study of surgical and conservative treatment.

A follow-up study was made to compare the effect of surgical reduction and stabilization of unstable vertebral fractures with the results of conservative treatment. Twenty patients treated by conservative methods and 18 patients subjected to surgical stabilization were analyzed with respect to period of immobilization, healing time, degree of residual deformity, neurological restitution and incidence of complications. The wide variety of initial impairment did not permit any definite conclusions with regard to neurological restitution. In all other respects, however, surgical stabilization produced better results as reflected by shorter periods of immobilization, hospitalization and rehabilitation, less residual deformity, and a lower incidence of complications. It may be concluded therefore that surgical treatment of unstable vertebral fractures may be of benefit to both patients and society.

Wagner resurfacing hip arthroplasty: preliminary results.

Resurfacing arthroplasty according to Wagner was used in 40 hips (31 patients) with coxarthrosis or arthritis. The severe, disabling preoperative pain was eliminated in all patients. There was a considerable increase in range of movement, and the functional results were very good. Of the 24 patients previously in employment but disabled by their hip condition, 22 have returned to their former occupations. Complications occurred in only four cases.

Arthropathy in children with severe hemophilia A.

Seven children with severe hemophilia A were studied. The radiological findings show that the patients developed joint changes despite our prophylactic treatment, even if no restriction of movement or deformity could be detected by physical examination. In contrast to earlier reports indicating that changes in the knee joints were most common, our findings show that changes in the ankle joints predominate. Such changes developed, even after less than 10 registered joint bleeding episodes. Prophylactic treatment must start early and should aim to eliminate subclinical bleedings also. Increased dosage and shorter intervals between treatments lead to less bleedings. The dosage must be so high and the time interval so short that the F VIII level in plasma is greater than 0.01 U/ml at the time of the next infusion. Prophylactic treatment must be regular and should be given even if bleeding requiring treatment with a therapeutic dose occurred in the previous day.

Aneurysmal bone cyst of the talus.

Aneurysmal bone cysts localized in the talus are extremely rare. Such a case is described and the histological findings are analysed in relation to those in aneurysmal cysts in general. The pathomechanism of the lesions is discussed.

Acetylsalicylic acid in a trial to diminish thromboembolic complications after elective hip surgery.

Fifty-one patients were investigated concerning thromboembolic complications in the operated leg after elective surgery of the hip. Suspicion of clinical deep venous thrombosis (DVT) arose in 11 patients, i.e. 21 per cent. Thirty-five patients were investigated with phlebography. The incidence of DVT among those patients was 15 cases, i.e. 43 per cent. It is concluded that acetylsalicylic acid, a drug which inhibits the platelet release reaction and thereby blocks the platelet aggregation, given orally in a dose of 2 gram per day from the first postoperative day, is not effective in diminishing the incidence of postoperative deep venous thrombosis.
