Microvascular thermal equilibration in rat spinotrapezius muscle.

The current study investigates heat exchange in the thermally significant countercurrent paired vessels of the rat spinotrapezius muscle. Detailed tissue surface temperatures under normal (after the microvascular surgery) and pharmacologically vasodilated states were measured using high-resolution infrared thermography. During vasodilation, a measurable thermal disturbance was observed above the first-order feeding vessel pair. The measured tissue temperatures were compared with those predicted by modifying the theoretical model for two-dimensional muscle preparations given by Zhu et al. (Zhu, L., D. E. Lemons, and S. Weinbaum. Ann. Biomed. Eng. 24:109-123, 1996). They were found in good agreement. The Weinbaum-Jiji k(eff) theory (Weinbaum, S., and L. M. Jiji. J. Biomech. Eng. 107:131-139, 1985) for heat exchange between the paired vessels and their surrounding tissue was also examined in this muscle. A close agreement was obtained between the theoretically predicted k(eff) and the measured value calculated using a fin approximation for the tissue layer. This experimental study revealed for the first time the nonequilibration between blood vessels and the surrounding tissue, where the enhancement in k(eff) due to the incomplete countercurrent heat exchange is comparable to the tissue axial conduction.

Risk factors for infection and role of C-reactive protein in Korean patients with systemic lupus erythematosus.

To evaluate risk factors for infection and the role of C-reactive protein (CRP) in the diagnosis of infection, a retrospective case control study was performed among Korean systemic lupus erythematosus patients. Of 120 proven infections, 31 episodes (25.8%) occurred in patients taking no corticosteroids (CS). The risk of infection was lower in patients taking low-dose CS (< 300 mg prednisolone/month) than no CS (odds ratio (OR) 0.36). In patients receiving high-dose CS (> 1000 mg prednisolone/month), however, the risk increased (OR 2.9). In patients taking no CS, disease activity manifested as increased SLEDAI, anemia and active urinary sediment, was associated with infection. The CRP was higher in the patients with infection than controls and the CRP levels over 50 mg/l were observed only in infection. These results suggest that CS have a bimodal influence on infection depending on dose. Disease activity is an important risk factor for infection in patients taking no CS. Finally, CRP levels greater than 50 mg/l suggest the presence of infection.

Adrenal cortical nodule mimicking small round cell malignancy on fine needle aspiration.

The cases of two elderly female patients with benign adrenal cortical nodules that mimicked small round cell malignancies by fine needle aspiration cytology are reported. Electron microscopic examination of part of the aspirate revealed the cells to be aggregates of bare nuclei of broken adrenal cortical cells, leading to a correct diagnosis.

Hypermethylation of HpaII recognition sequences within the 5' coding region of the estrogen receptor gene is not associated with estrogen receptor negativity in primary breast tumours.

Estrogen receptor negative tumours are unable to respond to antiestrogen therapy. The underlying molecular mechanisms of estrogen receptor negativity are poorly understood. Cytosine mechylation is one of the mechanisms of gene control and previous studies, particularly on breast tumour derived cell lines have suggested that hypermethylation of HpaII recognition sequences within the 5' coding region of estrogen receptor may be responsible for gene inactivity. This study has examined the methylation status of HpaII recognition sequences in estrogen receptor positive and negative breast tumours taking into account a polymorphic HpaII site in the 5' coding region of the estrogen receptor gene. It is concluded that hypermethylation of one or more of the 4 or 5 HpaII recognition sequences in the 5' coding region of estrogen receptor is not associated with ER negativity in primary breast tumours.
