Comparison of 0.8% and 1.6% terconazole cream in severe vulvovaginal candidiasis.

Terconazole is the first topical triazole antifungal agent. The results of several European clinical trials have shown both terconazole cream and suppositories to be effective and safe in the treatment of vulvovaginal candidiasis. We conducted a comparative placebo-controlled, double-blind study of 0.8 and 1.6% terconazole creams in a group of patients with persistent vulvovaginal candidiasis. In short-term evaluations (1-3 days and 8-11 days after therapy), 75% of patients receiving 0.8% terconazole cream were asymptomatic and 83.3% were mycologically cured. By 30-35 days after therapy, 75% were still asymptomatic and 58.3% were mycologically negative. No significant difference was seen between the results obtained with 0.8 and 1.6% terconazole creams. Safety data collected on these patients demonstrated that the side effects were rare and mild. This study confirms the efficacy and safety of topical terconazole in patients with vulvovaginal candidiasis.

[The intragastric balloon: a logical and attractive idea, but probably ineffective].

Loss of post prandial satiety is often of limiting value in the success of a slimming training. Thus, the idea to introduce a balloon filled partially with air to occupy the stomach seems to be logic. A technic easy to handle had contributed to a large diffusion of this method before people asked about its real efficacy. Several short term (10 days) or medium term (6-10 months) studies show a significant weight loss, but there have been no controlled trials to determine the relative contribution to weight loss of the gastric balloon vs dietary and behaviour modifications. Furthermore, a great variability in the weight loss, some subjects even gaining weight, suggest that the compliance of the patients may be the most relevant factor. Adverse effects are effectively low and principally concern local gastric irritation (2-5%) and intestinal occlusion in some rare cases requiring surgical removal of the partially deflected balloon. Thus, if the implantation of a gastric balloon is relatively safe, it is difficult to recommend its use to the treatment of obesity in regard of the actual published results, even if most of the treated patients suffered of refractory obesity, difficult to modify with non surgical treatment.

Affective and social influences approaches to the prevention of multiple substance abuse among seventh grade students: results from project SMART.

Two drug abuse prevention curricula were tested to determine their efficacy in preventing the onset of tobacco, alcohol, and marijuana use among adolescents. The first program focused on prevention through social pressure resistance training. The second featured affective education approaches to prevention. Curricula were tested on seventh grade students. Subjects were pretested just prior to the program and were post-tested at 12 and 24 months. Post-test analyses indicated that the social program delivered to seventh grade subjects was effective in delaying the onset of tobacco, alcohol, and marijuana use. No preventive effect of the affective education program was observed. By the final post-test, classrooms that had received the affective program had significantly more drug use than controls.

Immunochemical characterization of fibrinogen, fibrin I, and fibrin II in human thrombi and atherosclerotic lesions.

Arterial thrombi and atherosclerotic lesions were analyzed immunochemically and examined histologically. The extent of in vivo proteolytic cleavage of the amino-terminal end of fibrinogen by thrombin and plasmin was determined and quantitated by specific radioimmunoassays. The samples were treated with cyanogen bromide (CNBr), and the total amount of fibrinogen and fibrin-derived protein was determined as NDSK, the NH2-terminal disulfide knot of fibrinogen. Thrombin-releasable fibrinopeptides A and B were used to quantitate fibrinogen and fibrin I. Previous plasmin cleavage of the B beta chain was inferred from the amount of B beta 1-42 and B beta 15-42 in undigested NDSK. The results obtained in both acute and organized thrombi indicate that approximately 60% of the total protein (as determined by amino acid analysis) was fibrinogen-derived and that 70% to 80% of the fibrinogen-derived material was fibrin II. These findings support the hypothesis that fibrin II as distinct from fibrin I is the predominant component in a thrombus. In samples from normal and atherosclerotic aortas, fibrinogen-derived protein comprised less than 10% of the total protein. Samples from grossly normal aortas contained only fibrinogen and fibrin I. Fibrinogen concentration decreased and fibrin II concentration increased with increasing severity of the lesions, suggesting that increased fibrin II formation is associated with progression of atheromas.

[Pseudocysts of the pancreas simulating a renal lesion. Apropos of 2 cases].

The authors report two cases of pancreatic pseudocysts simulating a left renal pathology. Although arteriography and sonography may visualize a cyst, the key to the diagnosis is the high amylase content of the fluid collected by translumbar puncture of the pseudocyst. Correct conservative treatment implies a precise preoperative diagnosis, as abdominal exploration is apt to be confusing and often leads to the performance of an unnecessary nephrectomy.

[Colo-anal intussusception of the adult. A new case (author's transl)].

Report on a case of intestinal obstruction due to a sigmoido-rectal and afterwards anal intussusception of a malignant tumor : review of the literature. Colo-anal intussusception remain very rare as, since Lataste in 1975 related six cases, this one is unique. On the contrary seven new cases of colo-rectal intussusception have been reported. Diagnosis can be difficult. The variability of the mass revealed by successive examination and the intussusception pictures sometime provided by the barium enema may give the correct direction. In this case, treatment consists in the perineal resection of the sigmoid intussusception and its causal tumor followed by an end to end primary anastomosis with an iliac colostomy. This treatment seems to be more suitable for old and deficient people where carcinologic problem is not of first importance.
