Effects of age, BMI, smoking and contraception on levels of Cu, Se and Zn in the blood of the population in the Czech Republic.

Variations in the levels of Cu, Se and Zn in blood were analyzed in relation to gender, age, BMI, smoking and hormonal contraception. The blood samples were collected from 3,207 blood donors (2,362 men, 845 women) during years 1996-2003. Blood concentrations of these elements were in the same range as those found for populations of other European countries. Significant differences exist between men and women, in blood concentration, for Cu (840 microg Cu.l(-1) vs. 970 microg Cu.l(-1), p<0.01) and Zn (6,780 microg Zn.l(-1) vs. 6,235 microg Zn.l(-1), p<0.01) only. The level of Cu in relation to age is increasing in men, but decreasing in women. The level of Se in relation to age increases regardless of sex. Concentrations of Zn rise with age in women group, only. Positive correlation was found between BMI and level of Cu only for group of men. For Se levels and BMI differences exist only in men namely between groups <20 and >35 (82 microg Se.l(-1) vs. 92 microg Se.l(-1)). Slight positive correlations exist between Zn concentration and BMI in women only. The significant differences were found between smokers and non-smokers for Cu in men (850 microg Cu.l(-1) vs. 830 microg Cu.l(-1)) and Se (81 microg Se.l(-1) vs. 84 microg Se.l(-1)). The smoking has not significant influence on concentration of Zn in the blood. The hormonal contraception significantly increases the concentration of Cu in blood (920 microg Cu.l(-1) vs. 1,270 microg Cu.l(-1), p<0.01). The levels of Se and Zn in blood are not influenced using hormonal contraception.

Mutagenicity of airborne particulate matter PM10.

Mutagenic activity of extractable organic matter (EOM), from airborne particles collected over winters in four towns of Czech Republic, was investigated by the means of Salmonella typhimurium indicator strains TA98 and YG1041 using the Ames plate incorporation assay. Mutagenicity of all tested samples showed significant dose-related increase in number of revertants per mg of EOM. The direct mutagenic potency detected with TA98 increased further in the presence of external metabolic activation. The mutagenic potency detected with YG1041 was about two orders of magnitude higher than that detected with TA98. The mutagenicity results correlated with the concentrations of the polycyclic aromatic hydrocarbons (PAHs) determined by GC/MS. Local differences in mutagenicity, expressed as numbers of revertants per m3 of air, were observed with the highest values in Prague air samples. For routine ambient air mutagenicity monitoring, the use of YG1041 and the plate incorporation test are recommendable.

Determination of normal concentration levels of Cd, Pb, Hg, Cu, Zn and Se in urine of the population in the Czech Republic.

Knowledge of normal levels of concentrations of trace elements (Cd, Pb, Hg, Cu, Zn, and Se) in the population serves, among others, in design of regulations concerning health protection, determination of exposition limits and prevention of diseases caused by deficiency of trace elements. Concentrations of the named elements in urine of the Czech population were determined by means of atomic absorption spectrometry. The urine has been collected during 1996-2000 from 1192 individuals (816 males and 376 females, average age 34.6 years) and 2008 children (1052 boys and 956 girls, average age 9.9 years). Mineralization in a microwave digestion system was used in sample preparation. The accuracy of results was checked by means of the Control Materials Seronorm 403,125 and BioRad 69,041. Values of concentrations of the trace elements in urine found for adult (medians) were 0.36 microgram Cd.g-1 creatinine, 10.6 micrograms Cu.g-1 creatinine, 0.68 microgram Hg.g-1 creatinine, 3.3 micrograms Pb.g-1 creatinine, 6.2 micrograms Se.g-1 creatinine, and 397 micrograms Zn.g-1 creatinine, respectively. Statistically significant differences between men and women have been found in the concentrations of Cu and Hg. In the juvenile population following concentrations have been found: 0.29 microgram Cd.g-1 creatinine, 16.1 micrograms Cu.g-1 creatinine, 0.32 microgram Hg.g-1 creatinine, 4.8 micrograms Pb.g-1 creatinine, 10.2 micrograms Se.g-1 creatinine and 460 micrograms Zn.g-1 creatinine. Statistically significant differences between boys and girls were found only in Cu and Hg concentrations. Concentrations of the studied elements correspond to the published values concerning population not exposed professionally.

Copper saturation pathways of the urban population in the Czech Republic.

The estimation of the copper saturation pathways of the Czech urban population is presented. The data on copper concentrations proceed from the System of monitoring the environmental impact on population health in the Czech Republic in the period 1994-1998. The copper concentrations were monitored in foodstuffs from the commercial network, in drinking water at the outlets of the waterworks and in the public water main networks, and in the ambient air. The copper levels were measured also in biological material to obtain the data on the copper saturation of the population under monitoring. The copper intake from foodstuffs and drinking water did not vary significantly either in the particular years of monitoring or individual cities. The same concerns also the copper levels in biological material. The total copper intake for an average adult was estimated to be 20.2 micrograms/kg b.w./d., i.e. 1300 micrograms/d. Over 99% of the total intake was that from the diet. The exposure from the intake of drinking water as well as from ambient air was low. The total daily intake lies in the interval 1000-2000 micrograms/d which is usually found in the similar studies of the copper intake. It represents only 40% of the daily dietary copper intake recommended by the JECFA FAO/WHO Commission, 1982. The copper levels in biological material did not differ from the reference values, and did not indicate any evincible hyposaturation of the population with copper.

Reference values for lead and cadmium in blood of Czech population.

This study aimed at determining blood concentrations of lead (B-Pb) and cadmium (B-Cd) in a group of 1215 healthy blood donors (895 males, 320 females) and 758 children (397 boys, 361 girls) aged 10 years, sampled in 1996-1998 in four districts of the Czech Republic. The analysis was performed using atomic absorption spectrometry. The median B-Pb values were lower in children (34 mg/L) than in adults. In the group of adults, the level was significantly higher in men (46 mg/L) than in women (29 mg/L). In none of the groups the 90th percentile exceeded the value of 100 mg/L. The B-Cd values in adults depended on smoking habit (median non-smokers: 0.5 mg/L, male smokers--1.2 mg/L, female smokers--1.0 mg/L). In children, more than 50% of values were under the limit of detection. The obtained values were used to propose reference values for the Czech population.

Retrospective study of concentration levels of Pb, Cd, Cu and Se in serum of the Czech population in time period 1970-1999.

The retrospective study evolution of trends in concentrations of Cd, Pb, Cu and Se in the population of the Czech Republic over 30 years period could be useful in design of regulations concerning health protection, prevention of diseases caused by deficiency of trace elements and have considerable economic importance. Concentrations of the named elements in the serum were determined by means of atomic absorption spectrometry (AAS) after mineralisation in a microwave digestion system. The 1433 samples of serum (730 males and 703 females, average age 48.7 and 49.1 years respectively) from a time period 1970-1995 were obtained from the Serum bank of National Institute of Public Health and volunteers (1999). Serum samples from the years 1970, 1975, 1980, 1985, 1990, 1995 and 1999 were examined. The accuracy of the results was checked by means of the Control material Seronorm Whole Blood 404,107 and Seronorm Serum 704,121, Nycomed. The concentration of Cd had a decreasing trend for the years 1975-1999 (0.82 microgram Cd.l-1 vs 0.47 microgram Cd.l-1). The levels of Pb are falling in the interval 1970-1980 (15.6 micrograms Pb.l-1 vs. 6.6 micrograms Pb.l-1). In year 1985 concentration of Pb increased (10.7 micrograms Pb.l-1) but subsequently decreased again (1995--6.4 micrograms Pb.l-1). In year 1999, it was 8.9 micrograms.l-1. The concentration of Cu decreased in period 1970-1990 (1525 micrograms Cu.l-1 vs. 990 micrograms Cu.l-1). From 1990 to 1999 the levels of Cu had an increasing tendency (1999--1060 micrograms Cu.l-1). Levels of Se in the interval 1970-1985 were surprisingly equal (58.1 micrograms Se.l(-1)-54.4 micrograms Se.l-1). However in the interval 1986-1990 the level of Se sharp declined (38.3 micrograms Se.l-1). Since 1990 levels of Se in serum increased again, up to 67.1 micrograms Se.l-1 in 1999. Statistically significant differences between men and women were found only in Cu and Pb concentrations. Concentrations of under study elements corresponded to the published values concerning unexposed population.

Determination of thirty-two elements in human autopsy tissue.

Inductively coupled plasma-mass spectrometry (ICP-MS) was used to determine the concentrations of 32 elements in the human liver and kidney and 20 elements in the bone, obtained from 70 autopsied dead individuals (54 males, 16 females) between 18 and 76 yr of age from the North Bohemia territory of the Czech Republic. Geometric means, median, minimal-maximal range, as well as distribution and correlation analysis were calculated. Some significant differences among tissue concentrations of trace elements of the women and men were found. In the liver, medians of the concentrations of some elements were higher for men than that for women (Al: 770 vs 610 microg/kg; As: 42 vs 27 microg/kg; Cd: 1800 vs 1390 microg/kg; Rb: 3955 vs 3210 microg/kg; V: 160 vs 105 microg/kg). On the contrary, the content of other elements for men was lower (Bi: 0.8 vs 3.2 microg/kg; Cr: 57 vs 72 microg/kg; Hg: 228 vs 325 microg/kg; Zn: 57.1 vs 68.5 mg/kg). In the kidney of men, there were higher contents of Al (360 vs 245 microg/kg) and Hg (135 vs 75 microg/kg) and lower contents of Zn (47.7 vs 59.7 mg/kg) and I (135 vs 220 microg/kg) than those of women. In the case of bone, the concentrations of Cu and Rb were higher for men (1410 microg Cu/kg and 405 microg Rb/kg, respectively) than for women (655 microg Cu/kg and 285 microg Rb/kg, respectively). On the contrary, the content of Mn was considerably lower for men (110 microg Mn/kg) than for women (215 microg Mn/kg).

The accuracy of results was checked by means of the Control material Seronorm Whole Blood 404107 and Seronorm Serum 704121, Nycomed. Values of concentrations of the trace elements in blood found for adult (medians) were 0.7 microgram Cd.l-1, 800 micrograms Cu.l-1, 0.78 microgram Hg.l-1, 41 micrograms Pb.l-1, 76 micrograms Se.l-1, and 5,800 micrograms Zn.l-1, respectively. Statistically significant differences between men and women have been found in the concentrations of Cu, Hg, Pb, and Zn. In the juvenile population following medians of concentrations have been found: 0.15 microgram Cd.l-1, 1,047 micrograms Cu.l-1, 0.46 microgram Hg.l-1, 34 micrograms Pb.l-1, 69 micrograms Se.l-1, and 8,180 micrograms Zn.l-1. Statistically significant differences between boys and girls were found only in Pb and Zn concentrations. Concentrations of the studied elements correspond to the published values concerning population not exposed professionally.

Mutagenicity monitoring of airborne particulate matter (PM10) in the Czech Republic.

The mutagenic activities associated with inhalable airborne particulate matter (PM10) collected over a year in four towns (Czech Republic) have been determined. The dichloromethane extracts were tested for mutagenicity using the Ames plate incorporation test and the Kado microsuspension test both with Salmonella typhimurium TA98 and its derivative YG1041 tester strains in the presence and absence of S9 mixture. The aim of this study was to assess the suitability of both bacterial mutagenicity tests and to choose the appropriate indicator strain for monitoring purposes. To elucidate the correlation between mutagenicity and polycyclic aromatic hydrocarbons (PAHs), the concentration of PAHs in the air samples were determined by GC/MS. In general, the significant mutagenicity was obtained in organic extracts of all samples, but differences according to the method and tester strain used were observed. In both mutagenicity tests, the extractable organic mass (EOM) exhibited higher mutagenicity in the YG1041 strain (up to 97 rev/microg in the plate incorporation and 568 rev/microg in the microsuspension tests) than those in TA98 (up to 2.2 rev/microg in the plate incorporation and 14.5 rev/microg in the microsuspension tests). In the plate incorporation test, the direct mutagenic activity in YG1041 was on average 60-fold higher and in microsuspension assay 45-fold higher with respect to strain TA98. In the presence of S9 mix, the mutagenic potency in YG1041 declined (P<0.001) in summer, but increased in TA98 (P<0.05) in samples collected during the winter season. The microsuspension assay provided higher mutagenic responses in both tester strains, but in both strains a significant decrease of mutagenic potency was observed in the presence of S9 mix (P<0.001 for YG1041, P<0.05 for TA98 in winter). The mutagenic potencies detected with both indicator strains correlated well (r=0.54 to 0.87) within each mutagenicity test used but not (for TA98) or moderately (r=0.44 to 0. 66 for YG1041) between both of the tests. The mutagenic activity (in rev/m(3)) likewise the concentration of benzo[a]pyrene and sum of carcinogenic PAHs showed seasonal variation with distinctly higher values during winter season. A correlation between the PAH concentrations and the mutagenicity results for the plate incorporation, but not for the microsuspension tests was found. In samples from higher industrial areas, the higher mutagenicity values were obtained in plate incorporation test with TA98 and in both tests with YG1041 in summer season (P<0.05). According to our results, plate incorporation test seems to be more informative than microsuspension assay. For routine ambient air mutagenicity monitoring, the use of YG1041 tester strain without metabolic activation and the plate incorporation test are to be recommended.

The use of YG bacterial tester strains for the monitoring of drinking water mutagenicity.

The organic extract from the 50 drinking water specimens taken in four cities were tested for mutagenicity in the Ames test (plate incorporation assay) using the parent TA98 and TA100 strains, and derived YG1041 and YG1042 strains. Four dose levels of extractable organic matter (EOM) with duplicate plate per dose were used. Slopes (revertants/mg EOM) were calculated by the Bernstein linear regression rejection model using GeneTox Manager software. The mutagenicity observed in the conventional strains TA98 and TA100 did not reach the significant increase in all tested samples with the higher mutagenic response found in TA100-S9. With the YG1041 and YG1042 tester strains, the results obtained demonstrated the clear-cut direct dose-related mutagenicity response in all tested drinking water extracts. Compared with TA98 and TA100 strains, the numbers of YG induced revertants were approximately 20 times higher. The high sensitivity of the YG tester strains could facilitate the mutagenicity monitoring in drinking water extracts, and help reduce the volume of sample required. However, to identify the chemical contaminants in drinking water responsible for the mutagenicity further studies are required.

Population-based biomonitoring in the Czech Republic--the system and selected results.

In the framework of the system of monitoring the environmental impact on population health, the concentration of lead, cadmium and selenium in blood and cadmium in urine was measured in adults (n = 670), children (n = 599) and umbilical blood (n = 549) using atomic absorption spectrophotometry. Furthermore, cytogenetic analysis of peripheral lymphocytes in all population groups under study was investigated. The median blood Pb level for the overall group of adults (47.8 micrograms/l, i.e. 0.23 mumol/1) was significantly higher in men (51.5 micrograms/l, i.e. 0.25 mumol/l). Smoking significantly influenced the blood Pb level in women. The 90th percentile in no group exceeded the value of 100 micrograms/l (0.48 mumol/l). The median blood Cd level in adults (0.9 microgram/l, i.e. 0.008 mumol/l) depends on smoking habit (1.25 micrograms/l, i.e. 0.01 mumol/l). The median urine Cd level was 0.585 microgram/g creatinine (0.59 mumol/mole creatinine) in adults and 0.37 microgram/g creatinine (0.37 mumol/mole creatinine) in children. The median blood Se level (53.5 micrograms/l, i.e. 0.68 mumol/l) was found to be higher in the group of non-smokers (57.5 micrograms/l, i.e 0.73 mumol/l). Lead and selenium level were significantly lower in the umbilical blood. Cytogenetic analysis results showed age-dependent average percentages of aberrant cells: 1.1% in umbilical blood, 1.27% in children and 1.71 in adults in line with the reference values for the Czech population.

Genotoxicity of industrial effluents, river waters, and their fractions using the Ames test and in vitro cytogenetic assay.

Total and fractionated organic extracts from industrial effluents and Labe river water were tested for mutagenicity using the Ames test with TA98 strain and its YG derivatives (YG1021, YG1024 and YG1041) and cytogenetic analysis with human peripheral lymphocytes in vitro. The bacterial mutagenicity results showed a dose-dependent increase in numbers of TA98 revertants (10(5)) with effluent extracts and lower, but still significant, increase (10(3)) with river water extracts 6 km downstream. The further increase of YG revertants indicates the possible presence of nitroarenes and aromatic amines in the tested samples. In fractionated samples the significant mutagenicity was detected in two-polar acidic and polar basic-fractions, but the numbers of revertants in all effluent fractions were about one order of magnitude lower compared with total extracts results. The cytogenetic effect in human peripheral lymphocytes in vitro was not significant. The increase in chromosomal aberrations was not clearly dose-dependent and may reflect more the combination of toxic and clastogenic effects.

[Personal experience with the care of children and adolescents at risk for ischemic heart disease].

Children and adolescents with hereditary disposition for ischemic heart disease form a substantial part of the clients of preventive cardiological surgeries for children. The authors present their results and experience they assembled by monitoring and supervising these subjects for a five-year period. During the initial examination they found in these subjects, as compared with controls, a more frequent prevalence of habits dangerous with regard to the genesis and development of ischaemic heart disease and deviations in the lipid metabolism with an atherogenic character. Arterial hypertension and protracted insulin secretion were also more frequent. The collaboration with their parents as regards adherence to a regime and therapeutic provisions was very satisfactory. After five years' supervision in children and adolescents with hereditary disposition for ischaemic heart disease the prevalence of obesity, arterial hypertension and abnormalities of blood lipid levels declined significantly. The above measures had the least effect on the impaired insulin secretion.

[The effect of isradipine (Lomir, Sandoz) on coronary insufficiency during the esophageal stimulation stress test].

The new calcium antagonist, the dihydropyridine derivative isradipine (Lomir, Sandoz) is used above all in the treatment of hypertension. During oesophageal stimulation the authors proved its very favourable effect also in patients with coronary insufficiency during exertion. A single oral dose of 7.5 mg of isradipine led in all 14 examined patients to increased tolerance of exertion; in 36% of the patients after administration of the drug coronary insufficiency was no longer found. In the remaining patients the incidence of coronary insufficiency was shifted to higher frequencies. The authors recorded a drop of systolic and diastolic pressure during exertion, during rest as well as during recovery. No undesirable effects nor hypertension were present. The drug is a useful preparation in particular in hypertonic patients with angina on exertion.

Electrophysiological effects of oral isradipine (Lomir, Sandoz).

Electrophysiological effects of oral isradipine were studied. There are controversial references about the presence of its selective negative chronotropic influence from in vitro and in vivo studies using intravenous route. The authors administered 7.5 mg and 2.5 mg of isradipine orally in two groups of patients. Electrophysiological parameters were measured by use of the transoesophageal atrial pacing prior to the administration and 120 minutes after it. Significant fall in blood pressure was observed in both groups. Changes of atrioventricular nodal parameters such as stimulus-R interval. Wenckenbach point and probable effective refractory period of atrioventricular node showed positive dromotropic effect on artioventricular conduction, but the negative chronotropic action was not proved. On the contrary, the mean sinus cycle length shortened significantly in both groups and the maximal sinus node recovery time decreased after isradipine. The extent of changes of both sinus and atrioventricular nodal parameters is similar and is not consistent with selective effect of isradipine on sinus node.

[Termination of atrial flutter using esophageal stimulation].

The risk of possible complications in atrial flutter leads to attempts to use all available therapeutic possibilities to eliminate this disorder of the cardiac rhythm. By oesophageal stimulation the sinus rhythm was restored in 40% and a change to atrial fibrillation was achieved in 50% of the patients. The advantages of this method include above all speed and the minimal risk of complications; the disadvantage of the method is the unpleasant sensation when the electrode is inserted and the painful perception of the stimuli. Oesophageal stimulation can be recommended as the method of choice in treatment of atrial flutter.

[The effect of VUFB diltiazem tablets on the sick sinus and on electrophysiologic parameters of the heart atrium].

Using invasive electrophysiological methods, the authors evaluated the preparation Diltiazem VUFB tablets on parametres of sinus automaticity in the "sick sinus syndrome" and on properties of the atria. They confirmed significant inhibition of automaticity which is not signalized by a corresponding decrease of the nodal frequency, and may remain concealed and lead unexpectedly to disorders of the cardiac rhythm. This is not a specific property of the preparation, a similar action on the sick sinus is exerted by all calcium antagonists with clinically confirmed effects on the conduction system. Diltiazem is already produced in this country and sold on the market with the name of Discordin SPOFA tablets.
