Acceleration of the onset of liver regeneration by carnitine in partially hepatectomized rats.

Immediately and 6 h after removal of 70% of the liver tissue, rats were treated with L-carnitine (Carnitene, Sigma-Tau, Italy) and received an injection of 100, 200 or 1,000 mg/kg b.w. into their femoral vein. The control rats were given the same volume of saline solution. The rats were sacrificed 18, 21, 24 or 30 h after the operation. The development of liver regeneration was evaluated from the incorporation of 14C-thymidine into DNA and from the hepatocyte mitiotic activity. In rats given carnitine in a dose of 100 or 200 mg/kg b.w. significantly higher DNA specific activity values were found 18 and 21 h after partial hepatectomoy and higher hepatocyte mitotic activity values after 30 h. In rats given carnitine in a dose of 1,000 mg/kg b.w., DNA specific activity values 21 h after partial hepatectomy were lower than in the control group. We conclude that L-carnitine, in a dose of 100 or 200 mg/kg b.w. has an enhancing effect on the onset of liver regeneration after 70% hepatectomy.

[The significance of mediastinoscopy in the diagnosis and treatment of diseases of the lungs and mediastinum].

A total of 299 patients is presented in them the mediastinoscopy has been performed from 1970 up to 1987. The indications and examination technique are also reported. Its significance is evaluated in recognizing the individual groups of diseases of lung and mediastinum. Comparing this method with other techniques, its advantages are referred to as for its precision a good tolerance by patients. The relatively small rate of complications is stated which represented 1.33 per cent in total proposed.

[Mediastinal inflammation as a complication of heart surgery under extracorporeal circulation].

The authors describe mediastinitis in two patients operated on account of ischaemic heart disease. In both patients they used the closed approach which involved suture of the sternum and surgical wound, perfect drainage and the administration of antibiotics (general and local). They describe the applied method of fixation of the sternum. In both patients the sternum and surgical wound healed.

[Malignant tumors of the heart from the viewpoint of the surgeon].

The authors present three cases of malignant affections of the heart. In two instances a primary haemangiosarcoma was involved which originated in the right atrium, in the third patient a secondary of a left ovarian carcinoma was involved. They draw attention to the similar symptomatology of the disease and the possibility of echocardiographic visualization of intracardiac formations. The possibilities of radical removal of malignant tumours of the heart are very limited and call for the use of extracorporeal circulation. The incidence of this disease is very rare.

Effect of the infusion of glucose, itralipid and nutramin on the initiation of rat liver regeneration after partial hepatectomy.

Rats were subjected to 67% hepatectomy and immediately after the operation were given a 4-hour infusion containing 5 ml saline solution, 28% glucose, 10% Intralipid, 8% glucose or 8% Nutramin. The rats were killed 18, 21, 24 and 30 h after partial hepatectomy. The effect of the tested solutions on the rate of liver regeneration was evaluated from changes in DNA specific activity and the mitotic activity of the hepatocytes. The infusion of 28% and 8% glucose markedly inhibited the onset of regeneration after partial hepatectomy. Nutramin likewise had an inhibitory effect, but not so pronounced as that of glucose. Conversely, the infusion of a lipid emulsion (Intralipid) and saline stimulated the initiation of liver regeneration compared with glucose and/or Nutramin. The possible mechanisms of the effect of infusion of the individual tested solutions on the onset of liver regeneration are discussed.

Different effects of glucose and intralipid on the onset of liver regeneration in the early period after partial hepatectomy in the rat.

One day fasted rats were partially hepatectomized (PH) and immediately after operation infused for 4 h, receiving each 5 ml of tested solutions as follows: a) saline solution, b) 10% Intralipid (I)--Vitrum AB, Sweden, with the energy content of 4.6 MJ/1,000 ml, c) 28% glucose (G), with the energy content of 4.6 MJ/1,000 ml. All rats were sacrificed 18, 21, 24 or 30 h postoperatively. The results obtained showed that glucose infusion in an early period after PH slowed down the onset of liver regeneration in contrast to Intralipid which exerted stimulative action on the rate of liver regeneration. This was concluded from the following findings: 1. A significantly higher liver DNA specific activity 18 and 21 h after PH in the groups given I compared with those given G. 2. A significantly higher mitotic hepatocyte activity 24 and 30 h after PH in the groups given I as compared with those given G.
