[Prognostic factors for progression of superficial bladder cancer].

To clarify prognostic factors for progression of superficial transitional cell carcinoma of the bladder (Ta, T1 and G1, G2), 159 patients, treated by transurethral resection from 1975 to 1988, were analysed concerning clinical findings, laboratory data, endoscopic findings and histopathological findings of the tumor. Histopathological findings included ABH blood group isoantigen (ABH) and Thomsen-Friedenreich antigen (T-ag) as well as other result. Twenty two cases in the series showed progression; 10 with up-grading and up-staging, 12 with either one of them. Advanced age, positive urinary cytology, multiple or broad base tumor, G2 or T1 tumor, negative ABH and abnormal T-ag were closely associated with progression. By multivariate analysis of these factors, ABH, stage of the tumor, T-ag and form were shown to be important prognostic factors in this order. ABH and T-ag were not correlated with other clinicopathological factors in predicting tumor progression. Therefore we concluded that ABH and T-ag were much important for prediction concerning potential for progression of the superficial bladder cancer.

[Incidence of renal cell carcinoma in Chiba Prefecture, Japan; ten years experience].

Many reports about the increase of renal cell carcinoma patients have been published in Japan recently, however, the real fluctuations in the total number of patients in relation to the change of population have not been reported yet. Most of the patients with renal cell carcinoma in the last 10 years were examined in Chiba prefecture, which has a population of about five million and 25 active urological offices. Histologically confirmed cases were investigated by sending questionnaire letters. The items were as follows; sex, age, address, occupation, family history, past history, symptoms, examination methods that first detected the tumor, operation date, tumor diameter and clinical stage. Twenty two offices returned answers and 560 cases who lived in Chiba were found to have renal cell carcinoma from 1980 to 1989. Yearly incidence rates per 100,000 persons demonstrated a significant increase from 0.32 to 2.07. Small, asymptomatic and low stage cancers have been increasing rapidly, however, the rate of metastatic disease has not shown any decrease. The main cause of rapid increase seems to be attributed to progress in diagnostic methods and increase of early detection, but the possibility of an increase in some carcinogenic factors can not be ruled out.

[Production of polyclonal antibody against human androgen receptor and immunohistochemical study of human androgen receptor in prostatic tissues].

We produced polyclonal antibody against human androgen receptor (hAR) by means of immunizing a rabbit with hAR fusion protein that was expressed in E. coli. In Western blot analysis, this antibody, NH27, recognized two protein bands at the site of 110 kDa and 107 kDa in androgen-independent human prostatic cancer cells (PC-3), transfected with full-length hAR expression plasmid DNA and at the site of 114 kDa and 108 kDa in androgen-dependent human prostatic cancer cells (LNCaP). In immunohistochemical examination with NH27, the nuclei of epithelial and stromal cells in human benign prostatic hyperplasia were mainly stained as did with AN1-15, commercially available hAR monoclonal antibody. Titer of NH27, however, was about five times more high than that of AN1-15. In prostatic cancer cells the nuclei were stained with NH27 as did with AN1-15. Intensity of staining was various between the nuclei of cancer cells. The polyclonal antibody, NH27, produced in the present study is useful in investigating the characterization of AR in androgen-dependent and -independent prostatic cancers.

[A case of pre-Cushing's syndrome].

A 66-year-old female was admitted to Chiba University Hospital for the evaluation of a left adrenal mass which was incidentally discovered by computerized tomography. The patient had no clinical signs of Cushing's syndrome. Although the plasma ACTH level was suppressed, serum cortisol and urinary 17-OHCS levels were normal. Serum cortisol was not suppressed by dexamethasone and loss of diurnal rhythm of cortisol was observed. Uptake of 131I-aldosterone in the left adrenal gland was noted, but no accumulation was observed in the right one. Left adrenalectomy was performed. The tumor resected was 20 x 22 x 26 mm in size. Pathological diagnosis was adreno-cortical adenoma. Whether slight abnormality of adrenocortical function without clinical symptoms observed in the present case would develop into a clinically typical Cushing's syndrome remains to be solved.

[Metabolic analysis of the diseased human corneal endothelium].

Redox states of the corneal endothelium in 42 recipient corneas obtained at the penetrating keratoplasty were measured non-invasively using ocular redox fluorometry. Autofluorescence from reduced pyridine nucleotides (PN) and oxidized flavoproteins (Fp) were measured, and the PN/Fp ratio was used as an indicator of the redox state. Endothelial damage was graded as normal, mildly damaged, moderately damaged, and severely damaged, based on the histopathological findings. Mildly damaged endothelium showed a significantly higher PN/Fp ratio than the that in normal endothelium whereas the ratio was significantly lower in the severely damaged endothelium. These changes in the redox state may represent compensation and decompensation processes of the endothelial metabolism. Ocular redox fluorometry was shown to be useful for the evaluation of the metabolic state in the human corneal endothelium.

[Vesicoenteric fistula complicating Crohn's disease, responded to medical treatment: a case report].

A 20-year-old man was admitted on February 2, 1990 with the chief complaint of pollakisuria, miction pain and macrohematuria. Urinalysis revealed numerous leukocytes and Enterobacter was identified by urine culture. Cystoscopy revealed an edematous region. Contrast film of small intestines demonstrated vesicoileal fistula. Judging from these and the radiographic findings, we made the diagnosis of vesicoileal fistula with Crohn's disease. After medical therapy between February 22 and May 25, urinalysis became normal and we assumed that the fistula had closed. Now, it is about a year since the therapy began, the patient remains well, without urinary and enteric symptoms. Cystoscopy and urinalysis did not reveal recurrence of the fistula. Despite previous reports that vesicoenteric fistulas complicating Crohn's disease require surgical treatment, this case responded to medication, and required no surgical treatment. This case is a reminder of the importance of the principle of therapy.

[Noninvasive metabolic analysis of the diabetic cornea and lens: in vivo measurement].

In vivo measurement of metabolic changes in diabetic cornea and lens were performed using redox fluorometry in nonobese diabetic (NOD) mice. Autofluorescence from reduced pyridine nucleotides (PN) and oxidized flavoproteins (Fp) were measured, and the PN/Fp ratio was used as a tissue metabolism indicator. The PN/Fp ratios were significantly higher in the diabetic corneal endothelium. Morphometric analysis of the corneal endothelium using specular microscopy revealed no significant differences between the two groups. These results indicate that redox fluorometry is able to detect early metabolic changes in the corneal endothelium and lens epithelium, which are induced by diabetes mellitus. Activation of the polyol pathway may be responsible for the change. Corneal epithelia may be less susceptible to diabetic changes than the corneal endothelium and lens epithelium.

Inverted papilloma of the renal pelvis associated with renal cell carcinoma: a case report.

Twenty-one cases of inverted papilloma of the renal pelvis have been described in the literature. A 71-year-old man was admitted to our hospital to examine a right renal mass. We diagnosed a right renal tumor on the basis of the findings from excretory urogram (IVP), computerized tomography (CT) and magnetic resonance imaging (MRI). Surgical material revealed an inverted papilloma in the renal pelvis. We report on the first case of an invested papilloma of the renal pelvis associated with renal cell carcinoma.

Endoscopic re-establishment of membranous urethral disruption.

A total of 17 patients with traumatic membranous urethral disruption underwent urethral reconstruction via a core-through technique. Followup was 1 to 8 years (mean 3.7 years) postoperatively, and included 6 weeks with an indwelling catheter, periodic dilation for 6 months and occasional sounding. Within 1 year postoperatively, 6 patients required additional scar incision, including 3 who underwent scar resection. At 1 to 8 years postoperatively 6 patients had complications: 3 had stricture requiring periodic dilation (including 2 who underwent scar incision), while 2 had mild stress incontinence and 1 had nocturnal enuresis. Traumatic impotence was noted in 7 patients but the operation was not the cause in any. This method of endoscopic management was found to be an acceptable alternative to urethroplasty in cases of membranous urethral disruption.

Corneal epithelium in diabetic patients.

The corneal epithelia of phakic control (n = 13), aphakic control (n = 10), diabetic phakic (n = 10), and diabetic aphakic patients (n = 13) were surveyed under a wide-field specular microscope with a special contact lens. In control patients, the corneal epithelium showed relatively hexagonal cells, which we divided into three groups: bright cells, medium-bright cells, and dark cells. These epithelial cells displayed a smooth-surface configuration. In diabetic phakic patients, the epithelium seemed almost normal, with slightly more irregularity. In diabetic aphakic patients, the epithelium appeared larger. The cells were irregular and easy to distinguish from normal cells. Morphometric analysis showed that the mean cell areas of phakic control, aphakic control, diabetic phakic, and diabetic aphakic patients were 648 +/- 152, 643 +/- 125, 658 +/- 146, and 821 +/- 203 microns 2, respectively. The increase in the average cell area in diabetic aphakic patients was statistically significant. Although no apparent changes of the epithelium could be detected in the diabetic corneal epithelium under biomicroscopy, abnormalities that could be evaluated by specular microscopic images and morphometric analysis were observed.

Appearance of malignant phenotype with partial loss of hormone dependency in androgen-dependent Shionogi Carcinoma 115 transfected with hst-1 gene.

Shionogi Carcinoma 115 (SC 115) and Chiba Subline 2 (CS 2) are an androgen-dependent mouse tumor and an androgen-independent subline derived from SC 115, respectively. Since new expression of the transforming oncogene hst-1 might be related with autonomous progression in CS 2, the present study was designed to examine the influence of expression of hst-1 gene on SC 115. The transfectants of SC 115 with hst-1 still retained androgen sensitivity of growth, although the cells could grow without androgen. The transfectants, however, developed fibroblast-like appearance in the absence of androgen, in contrast to SC 115, which showed epithelial-like appearance after deprivation of androgen. The transfectants acquired an ability to form colonies in soft agar in the absence of androgen. SC 115 could not form tumors in intact mice, but the transfectants formed tumors at a high rate in male mice but not in female ones. From these results, it was concluded that expression of hst-1 was able to alter the phenotype of the androgen-dependent tumor with partial loss of androgen-dependency, and these changes might be advantageous for malignant progression.

Change of external urethral sphincter function in prostatic patients.

External urethral function was urodynamically examined in 13 patients with benign prostatic hypertrophy (BPH) associated with chronic urinary retention and in 5 volunteers. Prevoiding drop in external urethral sphincter pressure was noted in all the volunteers, whereas it was not found in 6 of the 13 cases of BPH. Bladder neck opening pressure was higher in these 6 cases (p less than 0.05). After administration of phentolamine, prevoiding drop was noted in 5 of these 6 cases, and bladder neck opening pressure decreased so much that there was no significant intergroup difference. The above results mean that the increase in alpha-adrenergic receptors makes the prostate, which has been already hypertrophied, less elastic, inhibiting external urinary sphincter function.

Serum immunosuppressive acidic protein as a tumor marker for renal cell carcinoma.

Since there are no reliable tumor markers in renal cell carcinoma, the present study was undertaken to evaluate immunosuppressive acidic protein (IAP) in patients with this tumor. Serum IAP levels were measured in 143 consecutive patients before and/or after nephrectomy by turbidimetric immunoassay. IAP levels had increased according to tumor diameter. Positivity rates of IAP were noticed as 45%, 75%, and 100% for patients with stage I/II, stage III, and stage IV diseases, respectively. Three-year survival rates also correlated with IAP: 96%, 81%, and 44% in preoperative levels below 500, of 501-1,000, and of more than 1,001 micrograms/ml, respectively. Serum IAP levels decreased within 3 months after the operation and increased with recurrence. These results suggest that serum IAP may serve as a tumor marker in patients with renal cell carcinoma.

Effects of estrogen on growth of androgen-responsive rat prostatic tumor (R 3327).

It has been known that estrogen has synergistic effects with androgen on growth of normal male accessory sex organs of rats. The present study was therefore undertaken to examine the effects of estrogen on androgen-responsive rat Dunning R 3327 prostatic tumor. The weight of male accessory sex organs was suppressed by estrogen on growth of treatment, but synergistic effects of estrogen and androgen on these organs were seen following combined treatment with androgen and estrogen. In contrast to the effects of estrogen on accessory sex organs, estrogen influenced a R 3327 tumor only in the negative direction regardless of whether androgen was injected simultaneously or not. When the dihydrotestosterone injection was reduced from 500 to 100 micrograms/rat/day after the tumor appeared as subcutaneous nodules, the weight of the accessory sex organs was similar to that of the control animals. However, this amount of dihydrotestosterone increased tumor growth equally when compared to those treated with a pharmacological dose of dihydrotestosterone. Therefore, the response of R 3327 tumor to androgen was different from that of the accessory sex organs.

Aspiration and tetracycline sclerotherapy of hydrocele.

Seventeen patients with hydrocele of the testis were treated by aspiration of fluid and substitution with tetracycline supplemented with 1% lidocaine. In 15 patients (88%) symptoms disappeared by sclerotherapy. After sclerotherapy scrotal ultrasonography was useful to diagnose either relapse or thickened tunica vaginalis. No side effect was noticed.

[Sexual disturbance caused by endocrine dysfunction].

Nineteen cases of sexual disturbance caused by endocrine dysfunction were surveyed for the clinical features. In hypogonadotropic hypogonadism ejaculatory failure was observed in 12 out of 13 cases. On the contrary, 2 of three hypergonadotropic patients preserved their ejaculation. The serum testosterone levels of hypogonadotropic and hypergonadotropic hypogonadism were less than 200 ng/dl. When the testosterone level decreased to less than 100 ng/dl, nine patients out of 13 complained loss of libido and 12 patients complained loss of ejaculation, but six patients preserved erectile function. Hyperprolactinemic patients showed loss of libido in 4, erectile failure in 4 and loss of ejaculation in 2. When good response to hCG test was present or the testicular volume was more than 4 ml, patients with hypogonadotropic hypogonadism showed good response to hCG therapy.

[Urinary cytology in patients with urolithiasis].

Urinary cytological examination was performed on 1032 patients of urolithiasis at the Department of Urology, Chiba University Hospital between 1980 and 1990. Seven hundred twenty-four were male and 308 were female, and the mean age was 44 years. The results of cytological examination of I-II and IV-V were classified as negative and positive, respectively. Eleven patients (1.1%) were positive, 2 of whom were found to be with renal pelvic tumor. False-positive findings were noticed in 9 cases (0.9%), and the abnormal cytologic changes in these cases disappeared after the calculi were removed. In negative cytological cases, 2 cases of renal pelvic tumor were found, one at nephrectomy and the other at percutaneous nephrolithotripsy. These cases were with staghorn calculi with hydronephrosis. The significance of cytological examination in management of calculous diseases were discussed.

[Urodynamic studies in patients with intervertebral prolapse].

Thirty-one patients with intervertebral disc prolapse (IDP) underwent urodynamic study before surgical operation. The lesion was located between C4 and T10 in 14, T11 and L2 in 2 and L2 and S1 in 15 patients. Preoperative urinary symptoms were obstructive in 11 (36%), irritative in 5 (16%) and both obstructive and irritative in 5 (16%) patients, and the remaining 19 patients (32%) had no symptoms. Preoperative urodynamic abnormalities were noted in 23 cases (74%): abnormal urinary uroflow in 6 of 14 cases (43%), residual urine of more than 30 ml in 9 of 31 cases (29%), abnormal cystometrogram and/or external sphincter EMG in 16 of 31 cases (52%). These urodynamic abnormalities were closely correlated with perineal hyposensitivity. Voiding function was re-examined in 22 patients after operation: improved in 14 (64%), unchanged in 1 (4%), worsened in 2 (9%) and normal in 5 (23%) before and after operation. Therefore the above results could indicate that the presence of perineal hyposensitivity should be suspected to have voiding dysfunction in patients with IDP. Orthopedic surgery may improve voiding dysfunction in the majority of patients.
