Are chronic psychiatric patients at increased risk for developing breast cancer?

OBJECTIVE: The authors' goal was to assess the incidence of breast cancer among chronic psychiatric patients. METHOD: They reviewed mammograms and charts of 275 female patients over the age of 40 in a state psychiatric hospital and 928 women of comparable age at a general hospital radiology clinic. RESULTS: The incidence of breast cancer documented by pathology reports among the psychiatric patients was more than 3.5 times higher than that of patients at the general hospital and 9.5 times higher than the reported incidence in the general population. CONCLUSIONS: If confirmed, the suspected higher incidence of breast cancer among the psychiatric patients might be due to medications and further underscores the need for screening mammograms for breast cancer in these patients.

[The influence of infected mothers on their children's immune response to filariasis].

OBJECTIVES: To investigate the reason for broad clinical spectrum of filariasis among the people in the endemic area, and to know the effect of infected mothers on their children's immune response to filariasis. METHODS: We measured the specific IgG and IgM antibodies in the sera of women and their 1-10 year old children, and maternal and umbilical cord, including Mf cases and control subjects by using IEST. RESULTS: The positive rates of IgG in the sera of women with Mf and their children were 91.4% (53/58) and 65.5% (38/58), and those of the control group 28.6% (34/119) and 24.4% (29/119). The positive rates of IgM were 81.0% (47/58) and 20.7% (12/58), and those the control group 11.8% (14/119) and 10.9% (13/119), respectively. Also, the positive rates of IgG in the sera of women with Mf and their infants were 91.7% (22/24) and 75.0% (18/24), and those of the control group 20.2% (19/94) and 16.0% (15/94). The positive rates of IgM were 70.8% (17/24) and 8.3% (2/24), and those of the control group 10.6% (10/94) and 3.2% (3/94), respectively. CONCLUSIONS: Whether the mother infected with the disease or not, the acquired immunological tolerance and resistance to the disease in their children might not be affected.

Prevalence of human papillomaviruses (HPV) in benign and malignant tumors of the upper respiratory tract.

The role for human papillomavirus (HPV) in head and neck tumors is not established. To evaluate the possible role of HPV in head and neck neoplasms, 22 cases of laryngeal squamous papilloma (LP), 32 cases of laryngeal squamous cell carcinoma (LSCC), 40 cases of nasal inverted papilloma (NIP), 14 cases of nasal squamous cell carcinoma (NSCC), and 40 cases of nasopharyngeal carcinoma (NPC) were evaluated for the presence of HPV DNA using the polymerase chain reaction (PCR) with two sets of primers in separate reactions: HPV-L1 consensus primers, HPV16 and HPV18 E7 primers for HPV nucleic acid detection. Restriction fragment length polymorphism of L1 PCR product was used for typing of HPV. Overall, HPV DNA was detected in 18 of 22 cases (81.8%) of LP, 3 of 32 cases (9.4%) of LSCC, 17 of 40 cases (42.5%) of NIP, 3 of 14 cases of NSCC (21.4%), and none of 40 cases of NPC. HPV6 was found more frequently in LP and HPV11 in NIP (P < 0.001). In the three HPV positive LSCCs, two cases had previous LP and were HPV6 and HPV11 positive, as were the prior papillomas. One other case was HPV18 positive. Only HPV16 was found in the NSCC patients. There was no significant difference in the index of HPV positivity between the NSCC cases associated with (16.7%) and without NIP (25.6%). Our results suggest that HPV plays a role in the development of both LP and NIP, and that similar viral types (i.e., HPV6 and HPV11) may exhibit relative differences in their tissue specificity. HPV appears to be of limited importance as a co-factor in LSCC and NPC lesions, indicating differences in the pathogenesis between papillomas and carcinomas in the upper respiratory tract.

Study on the column process of adsorption decolorization for monosodium glutamate solution in fluidized beds.

The adsorption decolorization of monosodium glutamate solution employed in multiple column fluidized beds in a series is studied. The equilibrium data, the mass transfer kinetics, and fluid flow parameters are determined. A mathematics model taking into account the effects of particle size classification, particle size distribution, external and internal diffusions, and axial mixing in the liquid and solid phases is proposed, and the simulated results are in good agreement with the experimental data.

[Selective training of the vastus medialis muscle using electrical stimulator for chondromalacia patella].

Chondromalacia patella is closely related with subluxation and tilt of patella, as well as with muscular atrophy of quadriceps, especially in vastus medialis muscle. 364 cases of chondromalacia patella were treated with selective training of the vastus medialis muscle using electrical stimulator in our hospital. 211 cases were followed up after treatment from 6 months to 3 years. Among them excellent and good results were seen in 130 cases (62%), fair results were seen in 69 cases (33%) and no change was seen in 12 cases (5%). Significant reduction of CA (P < 0.01) and LPA (P < 0.001) were observed in all these patients in comparison with their primary angle. We believe that the selective training of the vastus medialis muscle using electrical stimulator is one of the effective methods for the treatment of chondromalacia patella.

Surgical treatment of spinal tumor (analysis of 32 cases).

With the advance of spinal surgery in the last decade, surgical treatment of spinal tumors has been no longer limited to simple laminectomy. The principles of surgical treatment of spinal tumors include: (1) anterior approach for the anterior lesion and posterior approach for the posterior lesion; (2) combined anterior and posterior approach for extensive lesions and (3) internal fixation for spinal stability. 32 cases of spinal tumors were treated on the basis of the above guiding principles and 84% (27/32) showed excellent or good results.

[Translabyrinthine--transtentorial approach for removal of large acoustic neuromas].

In 30 consecutive patients with large acoustic neuromas, which were more than 4.0 cm in diameter, tumor excision was performed by a one-stage combined translabyrinthine-transtentorial approach. The tumors were totally removed in 29 patients (96.7%), without death. The facial nerve was preserved anatomically in 16 patients (53.3%), and functionally in 11 (36.7%). Main advantages of this approach include: (1) direct approach with a more extensive exposure; (2) identification of both the origin of the facial nerve at the brain stem or in the internal auditory canal: (3) minimal postoperative reaction; (4) combination with other approaches. A brief discussion is been made on operative technique, prevention of postoperative complications and facial nerve preservation.

[The study of the morphology of ultramicrostructure in molecular level of nucleus herniation of lumbar intervertebral disc].

In order to investigate the morphology of ultramicrostructure in molecular level of nucleus herniations, the authors studied the ultramicrostructure of three cases of nucleus herniations of lumbar intervertebral disc which were proved by operations and one case of normal nucleus pulposus of fresh corpse by using of atomic force microscope. The results showed that the arrangement of collagen bundle, collagen fiber and procollagen fiber were different between the two conditions, and the morphology of molecular particles of proteoglycan in nucleus herniations was different from that in the normal condition. In the condition of nucleus herniations, the collagen fiber formed procollagen fiber reticular porous adhering to the molecules of protein and proteoglycan, and the DNA adhering to the monochain of RNA in fiberal cell nuclei, in the form of bichain and multichain three dimensions helix structures. The banded structure (64-78 mm) of the procollagen fiber, and the collagenation and calcification of the degenerated nuclens pulposuses, the quantity of which was significant greater than normal (P < 0.05) indicated that the chronic injury of lumbar intervertebral nucleus pulposuses gave rise to the injury and break of the collagen fiber which resulted in twist (tangle) phenomenon so that nucleus pulposuses lost their normal resistance strength.

[Failure and complication following surgical treatment of scoliosis: analysis of 101 cases].

Complications occur frequently after surgical treatment of scoliosis. In order to prevent from them effectively, 101 cases with failure and postoperative complications were analysed. They included rod fracture in 22 cases (15 Harrington rods, 4 Zielke rods, and 3 Luque rods); recurrence of curve severity in 12 cases; broken or loossened luque wires in 15 cases; loss of thoracic kyphosis (flat back) in 6 cases; progressive kyphosis with or without paraplegia following incorrect posterior decompression in 5 cases; and increased unbalance of shoulders after instrumentation in 2 cases due to neglect of the tilting of the first thoracic vertebra. Infection occurred in 8 cases (incision infection 7 cases; deep wound infection in 1 case); and pneumothorax in 1 cases. They were induced by biomechanical factors in 23 cases (22.77%), incorrect selections of indications in 29 cases (28.71%), oprational mistakes in 37 cases (36.63%), internal fixation factors in 15 cases (14.85%). The authors hold that there are quite a lot of factors leading to occurrence of complications and the effective way for prevention from them is to understand the factors and main technical points related to internal fixation.

[Dynamic changes in soluble interleukin-2 receptor levels after rat corneal allograft].

PURPOSE: To evaluate the diagnostic utility of serum IL-2R level test in corneal allograft rejection. METHODS: Serum soluble IL-2R was detected by radioimmunoassay in rat corneal allograft model. RESULTS: Levels of soluble IL-2R were markedly elevated prior to the clinical diagnosis of rejection. CONCLUSIONS: Marked elevations of soluble IL-2R are associated with rejection, measurement of soluble IL-2R may have a useful value in the early diagnosis of corneal allograft rejection.

Synergic effects of NO and oxygen free radicals in the injury of ischemia-reperfused myocardium--ESR studies on NO free radicals generated from ischemia-reperfused myocardium.

The ESR signal of NO bound to hemoglobin was detected during the ischemia-reperfusion of myocardium with low temperature ESR technique, and the synergic effects of NO and oxygen free radicals in the injury of the process were studied with this technique. Oxygen free radicals and NO bound to beta-subunit of hemoglobin (beta-NO complex) could be detected simultaneously in the ischemia-reperfused myocardium. Those signals could not be detected from the normal myocardium even in the presence of L-arginine. However, those signals could be detected and were dose-dependent with L-arginine in the ischemia-reperfused myocardiums and the signal could be suppressed with the inhibitor of NO synthetase, NG-nitro-L-arginine methylester (NAME). Measurement of the activities of lactate dehydrogenase (LDH) and creatine kinase (CK) in the coronary artery effluent of ischemia-reperfused heart showed that L-arginine at lower concentration (< 1 mmol/L) could protect the heart form the ischemia-reperfusion injury but at higher concentration aggravate the injury. Addition of NAME to the reperfusion solution could also protect the myocardium. Addition of xanthine (X)/xanthine oxidase (XO) or Fe2+/H2O2 to the reperfusion solution increased the production of NO and oxygen free radicals and the ischemia-reperfused injury simultaneously. Addition of superoxide dismutase (SOD) and catalase decreased the production of NO and oxygen free radicals and the ischemia-reperfusion injury.
