Functional roles of today's public health nutritionist.

This study found that functional roles of public health nutritionists in eight southeastern states differed significantly on the basis of education, dietetic registration status, years of experience, and type and level of service. Subjects (n = 992) were mailed a 95-item questionnaire with three parts: demographic, professional practice, and grant-writing experience. Results based on a 59% response rate indicated that most of the 14 functional roles studied were practiced in accordance with guidelines presented in Personnel in Public Health Nutrition for the 1980's. Only two roles, counselor and educator, were performed somewhat inconsistently. Personnel with bachelor's degrees did have more counseling responsibilities than those with more advanced degrees. However, nonregistered dietitians also had more counseling responsibilities than did registered dietitians. Direct care providers had fewer educator responsibilities than did personnel with administration/management job responsibilities. All personnel reported research responsibilities that increased with advanced education, training, years of experience, and type and level of service. We found that beyond-entry-level personnel had more administrative and policy-setting responsibilities, which is consistent with The American Dietetic Association's Role Delineation for Registered Dietitians and Entry-Level Dietetic Technicians (1990). We conclude that, with the exceptions of the counselor and educator roles, public health nutrition practice is consistent with personnel guidelines.

A 51Cr release cytotoxicity assay for use with human intrahepatic biliary epithelial cells.

Human intrahepatic biliary epithelial cells are important immune targets in a variety of hepatobiliary diseases particularly primary biliary cirrhosis and primary sclerosing cholangitis. The ability to isolate and maintain these cells in short term primary tissue culture has permitted us to develop an in vitro cytotoxicity assay for the study of these cells as potential targets to a variety of toxic stimuli. We have therefore established a chromium-51 (51Cr) release cytotoxicity assay for use with primary cultures of human intrahepatic biliary epithelial cells. The method is simple, reproducible and is more sensitive than dye exclusion, light microscopy and release of lactate dehydrogenase, aspartate aminotransferase and alanine aminotransferase.

Transforming growth factor-beta induces human T lymphocyte migration in vitro.

In addition to its activities as a growth factor, recent studies suggest an immunoregulatory role for transforming growth factor-beta (TGF-beta). In this context we have demonstrated that TGF-beta is a potent chemotactic factor in vitro for human T lymphocytes at a concentration of 40 fM and for monocytes at a concentration of 0.4 fM but that it has no chemotactic activity for neutrophils. Furthermore, using an assay of lymphocyte subset chemotaxis we have been able to show that TGF-beta can induce migration of both CD4+ and CD8+ T lymphocytes in vitro. This study provides further evidence that TGF-beta acts as a cytokine, being able to attract T lymphocytes and monocytes to sites of inflammation. Its role in the pathogenesis of inflammatory reactions is likely to be complex.

New monoclonal antibodies reacting with bile ducts: further insights into the pathogenesis of bile ductular proliferation in biliary diseases.

We have produced a range of monoclonal antibodies which stain human intrahepatic bile ducts of different sizes. Amongst 26 monoclonal antibodies produced, five clones reacted specifically with bile ducts of different sizes, of which three have been maintained in culture and their viability following freezing and thawing confirmed. Staining patterns varied between normal adult liver tissue, normal fetal liver tissue and a variety of hepatobiliary diseases. The antibodies provide further evidence of the immunological heterogeneity of the human intrahepatic biliary tree and support the hypothesis that proliferating bile ductules are derived from periseptal hepatocytes. The preparation of the antibodies, their staining reactions in normal adult, normal fetal and a variety of liver diseases are described.

The cost of somatization.

Fifty-two consecutive patients referred to a psychiatrist with somatic symptoms of underlying psychiatric disorder were studied. The costs of investigations performed in the general hospital prior to referral to the psychiatrist were assessed. The median cost was pounds 286 but the range was pounds 25-2300. Those costs were determined by the physician's original assessment of the likelihood of organic disease and were independent of the view expressed in the general practitioner's referral letter. This preliminary study indicates the need to understand more fully the determinants of early or late referral of somatization patients to a psychiatrist. The determinants include the diagnostic difficulties of the presenting symptom, patient factors including resistance to adopting a psychological view of the symptoms, and physician factors determining the number of investigations performed to exclude organic disease.

Bone mineral content of British and rural Gambian women aged 18-80+ years.

The bone mineral content (BMC) of the mid-shaft radius was measured in 446 rural Gambian and 349 British women aged 18-80+ years using single-photon absorptiometry. The two groups of women differed substantially in habitual calcium intake, calcium requirements for reproduction, physical activity and incidence of minimal-trauma fractures. The relationships of BMC with bone width (BW) and with body weight and height were explored using stepwise multiple regression analysis. The pattern of BMC with age was similar in The Gambia and Britain; BMC increased slightly to a peak at the end of the fourth decade followed by a decrease of about 30% between 40 and 70+ years. Gambian women had significantly lower BMC than British women of the same age (-5.6%, P less than 0.001) but after adjusting for weight, height and BW, the BMC of Gambian women was shown to be slightly higher than that of British women (+2.1%, P less than 0.05).

Identification of CpG islands in a physical map encompassing the Friedreich's ataxia locus.

The Friedreich's ataxia locus has been previously assigned to chromosome 9q 13-21.1 by the demonstration of tight linkage to two anonymous DNA markers. MCT112 (Z greater than 80, theta = 0) and DR47 (Z greater than 50, theta = 0). The absence of recombination between these three loci has prevented the resolution of gene/probe order in this region, impeding strategies for gene isolation. We report physical mapping over a 4-Mb genomic interval, linking the markers MCT112 and DR47 on a common 460-kb NotI fragment and identifying 11 CpG islands in the 1.7-Mb interval most likely to contain the Friedreich's ataxia locus. Four of these islands were detected only by analysis of three YAC clones spanning a 700-kb interval including the MCT112/DR47 cluster. Without clear evidence of the precise location of the disease locus from recombination events, each of these regions must be considered as specifying a potential "candidate" sequence for the mutated gene.

Intrathecal morphine 0.2 mg versus epidural bupivacaine 0.125% or their combination: effects on parturients.

To compare the efficacy and side effects of 0.2 mg intrathecal (IT) morphine with 0.125% epidural bupivacaine, 62 women in labor were studied. They were randomly divided into three groups: group 1 (n = 20) received IT morphine; group 2 (n = 22) received epidural bupivacaine; and group 3 (n = 20) received a combination of both using a combined spinal-epidural (CSE) technique. According to a visual analogue scale for assessing analgesia, neither IT 0.2 mg morphine nor 10 ml 0.125% epidural bupivacaine was effective in producing adequate pain relief in labor, whereas the combination produced excellent analgesia. The use of IT morphine significantly reduced the dosage requirement of epidural bupivacaine. The incidence of nausea, vomiting, and pruritus was significantly higher when IT morphine had been administered, whereas that of urinary retention did not differ. No serious respiratory depression occurred in any of the patients. When the course of labor was studied, the prior use of IT morphine significantly prolonged the duration of the first stage of labor and the total duration of labor. We conclude that the administration of 0.2 mg IT morphine in combination with epidural administration of 0.125% bupivacaine provides better analgesia than the administration of either drug alone.

Increased expression of intercellular adhesion molecule 1 on bile ducts in primary biliary cirrhosis and primary sclerosing cholangitis.

It has been suggested that immunological mechanisms involving lymphocyte-mediated damage are important in the characteristic bile-duct damage that occurs in primary biliary cirrhosis and primary sclerosing cholangitis. Because adhesion is necessary for the interaction of lymphocytes with their target structures, we have studied the expression of intercellular adhesion molecule 1, a ligand for the leukocyte adhesion receptor lymphocyte function-associated antigen 1 in the liver of patients with primary biliary cirrhosis and primary sclerosing cholangitis. Strong expression of intercellular adhesion molecule 1 was seen on interlobular bile ducts and proliferating bile ductules in both conditions. In primary biliary cirrhosis, medium-sized ducts, which are spared by the disease, were negative. Minimal bile-duct staining was seen in conditions in which bile-duct damage is not a major feature, such as nonbiliary cirrhosis and acute liver diseases. In patients with cirrhosis from any cause, strong expression of intercellular adhesion molecule 1 was detected on the periseptal hepatocytes adjacent to new connective tissue. The intensity of immunohistochemical staining was recorded using a semiquantitative visual scoring system that was subsequently validated quantitatively by confocal laser scanning microscopy. The expression/induction of intercellular adhesion molecule 1 on bile ducts may be important in the pathogenesis of bile-duct damage in primary biliary cirrhosis and primary sclerosing cholangitis and is further evidence to support an immune pathogenesis in these two conditions. Furthermore, the induction of intercellular adhesion molecule 1 on hepatocytes may be an important factor in the liver-cell damage and fibrosis that occur during the development of cirrhosis.

Antithrombin Rouen-IV 24 Arg----Cys. The amino-terminal contribution to heparin binding.

A variant antithrombin with reduced heparin affinity was shown by mass spectrometry sequencing and DNA amplification to have a substitution of a cysteine for an arginine at residue 24. The position of Arg-24 can be fixed within a 12 A radius from the bridge at Cys-21. This is compatible with findings in the homologous protease nexin-1 which indicate an extension of the binding site of heparin from the D-helix to under the adjacent amino-terminal pole.

Menstruation phobia treated by cognitive correction: a case report.

The management of a case of cessation of menstruation in which differing beliefs and expectations between the patient and psychiatrist led to a diagnosis of phobia of menstruation is described. The treatment was purely cognitive. The problems were acknowledged and clarified. A reorientation was achieved that increased self-efficacy and made the return of menstruation acceptable.
