[Malignant neoplasms and kidney transplantation].

In 661 renal transplantations, 2 potentially migrated tumours (0.38%), 5 preexisting neoplasias (0.76%), and 31 "de novo" tumours were seen in 29 patients (4.4&). Although of very low incidence, the likelihood of tumour migration from elderly donors, given the circumstances surrounding removal, offers a high risk. None of the preexisting neoplasias relapsed following transplant. The highest prevalence was seen in skin (40%), lung (13%), kidney (13%) and bladder (6.6%) "de novo" tumours. Incidence of lymphoma was low. Dominant etiological factors of the recipient were older age, effective and tolerated immunosuppression, viral infections, environmental agents and antigenic stimulation of the graft. Skin lesions have responded well to local treatment, without need to discontinue immunosuppression, a measure that is mandatory in other malignant tumours. Also, the conclusions of a round table during the 25th National Meeting of Urotransplantation of the Spanish Association of Urology held in 1994 on "Oncology and Renal Transplantation" are presented.

[Acute aortic dissection in pregnancy. Report of a clinical case].

Aortic Dissection seldom affects young women. Nevertheless, the structural changes in the arterial wall, that occur during pregnancy, may predispose women to this complication, particularly in the third trimester and peripartum period. Aortic Dissection in gestation is associated specially with the Marfan's syndrome. The authors report a case of a pregnant woman in the 35th week of gestation, without Marfan's disease, admitted to Hospital on suspicious of type A aortic dissection.

[Acute myocardial infarct: the prognostic significance of complex coronary lesions].

OBJECTIVE: The aim of our study was to analyze the possible influence of coronary lesion morphology on the prognosis of patients (pts) with Acute Myocardial Infarction (AMI) evaluated by coronary angiography before hospital discharge. DESIGN: Retrospective study. SETTING: Study performed at the Cardiology Department of a Central Hospital. MATERIAL AND METHODS: 141 consecutive pts admitted because of AMI were evaluated, with an age average of 53.4 +/- 9.6 years, who had hospital discharge. All pts were submitted to angiography between the 2nd and 3rd week after AMI. The ventriculography was evaluated to determine the ventricular function score, as defined by the CASS protocol. The coronarography was performed to access the number of diseased vessels and to analyze their lesion morphology. Complexity was defined by the presence of one of the following characteristics: irregularity, shoulder, ulcers, filling defects, contrast retention and ecstasy. TWO GROUPS OF PTS WERE CONSIDERED: Group I--n = 64 pts, with complex lesions, and Group II--n = 69 pts with non complex lesions. Seven pts were excluded from the study because they had no significant coronary disease. Mean time of the follow up was 21.4 +/- 8.5 months and was similar in the two groups. The cardiac events considered were angina after AMI; reinfarction; heart failure; new hospital admission, percutaneous transluminal coronary angioplasty; coronary artery bypass grafting and death. RESULTS: In relation to both groups no statistically significant difference was found concerning sex, age, left ventricular function score and number of diseased vessels. In group I a higher incidence was found for cardiac events (p 0.006) and for the occurrence of angina after AMI (p < 0.02). In this group the number of pts with cardiac events was also higher (p < 0.01). No difference was found in relation to each of the morphologic characteristics and a high risk profile could not be found. CONCLUSIONS: Besides left ventricular function and the number of diseased vessels, the analysis of coronary lesion morphology, evaluated 2 to 3 weeks after AMI, could be useful in risk stratification after AMI.

Relationship of apical lymph node involvement to survival in resected colon carcinoma.

In a prospective study of 197 patients with resected colon carcinoma treated between 1974 and 1985, we explored the relationships between pathologic parameters, and the effect of the latter on survival, to identify the parameter whose systematic measurement would improve the predictive capacity of pathologic staging. Prognostic characteristics were studied by univariate analysis. The results showed significant relationships between the location and number of lymph nodes involved, blood vessel invasion, depth of tumor penetration, and metastases. The five-year survival rates were 45 percent and 17 percent (P < 0.001) for patients without and with apical lymph node involvement, respectively, and 44 percent and 6 percent (P < 0.05) for those with four or less nodes involved and more than four involved, respectively. Among the patients treated by incomplete resection, the respective survival rates of those resected for metastases and of those resected for apical lymph node involvement did not differ significantly. We conclude that the involvement of apical lymph nodes has a significant effect on prognosis and suggest systematic pathologic examination of these nodes to allow simpler and more reproducible selection of patients for treatment by incomplete resection who are at high risk of disease-related death.

Relationship between gastric acid secretion and the rate of recurrent ulcer after parietal cell vagotomy.

OBJECTIVE: This study assessed the effect of gastric secretion on the rate of recurrent ulcer after parietal cell vagotomy for duodenal ulcer. SUMMARY BACKGROUND DATA: Three hundred patients who underwent parietal cell vagotomy for duodenal ulcer between 1975 and 1986 were evaluated. The mean follow-up period for 280 patients was 5 years. METHODS: The gastric secretion tests concerned basal acid output (BAO) and peak acid output stimulated by pentagastrin or insulin. Tests were preoperative for 172 patients and postoperative for 118. RESULTS: At the end of that time, the overall incidence of symptomatic recurrent ulcer was 15%. Two criteria were shown to be important predictors of recurrent ulcer: preoperative BAO > 7 mmol/hr, for which the recurrence rate 5 years after vagotomy was 30% versus 11% for values below this threshold (p = 0.01), and postoperative BAO > 1.4 mmol/hr, for which the recurrence rate at 5 years was 72% versus 8% for lower values (p = 0.0001). All patients with recurrent ulcer had either a postoperative BAO > 7 mmol/hr and/or a postoperative reduction in BAO < 80%. CONCLUSION: Preoperative BAO > 7 mmol/hr and postoperative BAO > 1.4 mmol/hr were shown to be factors predictive of RU. All patients with RU presented either with preoperative BAO > 7 mmol/hr and/or a reduction in BAO < 80%. Consequently, in our opinion, these criteria could be used either to select patients for vagotomy or to assess the effectiveness of vagotomy of different types, especially those performed by celioscopy.

Mycoplasma pneumoniae infections in schoolchildren of a tropical community.

Information about Mycoplasma pneumoniae infections in tropical countries is scarce. This study reports the prevalence of anti-M. pneumoniae antibodies among schoolchildren from the tropical community of Palmares, Costa Rica. The serum samples were collected in July 1983 from 2021 children who represented 99% of the county's schoolchildren. Immunofluorescence (IFA) and in some cases ELISA serological methods were applied. IFA seropositivity was high (53%) and ranged from 22% to 79% depending on the district. Marked differences in the prevalence of antibodies in children attending schools of neighboring communities, suggest that the school may be epidemiologically important as a focus to spread mycoplasma infections among children. The presence of a low percentage of antibodies (IgG, IgM and IgA) against the 168 kd- M. pneumoniae antigen suggested that there were few cases of active or recent infections.

[Three years of acute bacterial meningitis in the pediatric service at the Temuco Regional Hospital].

The aim of this work was to study the clinical, epidemiological and laboratory features of 90 children, hospitalized between 1988 and 1991 with the diagnosis of acute bacterial meningitis in a region with a high proportion of aboriginal population. Twenty six percent of studied patients were of mapuche origin. The causative organism was identified in 82% of cases (H influenzae in 38% of cases, S pneumoniae in 29% and N meningitidis in 10%). H influenzae was resistant to ampicillin in 16% of cases and resistant to chloramphenicol in 4%. This agent was identified in 52% of patients of mapuche origin compared with 33% of non mapuche patients. Seventy three percent of children were less than 2 years old. Thirty seven percent of children had complications during hospitalization, 12 children died (13.3%) and 38% of children had neurological sequelae at the moment of discharge.

Effects of diltiazem on myocardial ischemia in patients with coronary artery disease.

OBJECTIVES: To evaluate the effect of diltiazem on the characteristics of ischemic episodes detected by Holter monitoring in a group of patients with proven coronary artery disease. SETTING: Department of Cardiology, Santa Marta Hospital, Lisbon. METHODS: Eleven selected out-patients, aged 48 to 79 years, with transient ST-segmental depression on Holter monitoring and proven coronary artery disease, were submitted to a double-blind crossover placebo controlled study, during hospitalization. The total ischemic burden of each patient and an analysis of ischemic episodes were evaluated before and during the drug/placebo phase trial. Three groups of ischemic episodes were considered: group I constituted by 66 episodes found on basal Holter recording; group II by 28 episodes detected during placebo trial and group III by 12 episodes detected during diltiazem trial. RESULTS: A reduction of the number of ischemic episodes was predominantly observed with the diltiazem administration. Statistically significant differences were observed between basal and placebo groups and especially between basal and diltiazem groups concerning the mean maximum ST-segment depression (2.17 vs 1.80; p = 0.030 and 2.17 vs 1.54; p = 0.0091). Significant differences were also obtained between the above mentioned groups concerning the heart rate variation from the onset of ST-segment depression to its maximum depression (13.5 vs 9.69; p = 0.023 and 13.5 vs 2.91; p = 0.01) and from two minutes before the onset of ST-segment depression to its maximum depression (21.2 vs 12.67; p = 0.012 and 21.2 vs 8.75 p = 0.016). CONCLUSIONS: Diltiazem seems to reduce the number of ischemic episodes in patients with coronary artery disease, during hospitalization. The study of its effects on the characteristics of ischemia requires further investigation with a greater number of patients. The limitations of the present study, described in the discussion, must be taken into account in future pharmacological investigations with Holter monitoring.

Clinical and electrophysiologic characteristics in patients with Wolff-Parkinson-White pattern.

OBJECTIVES: (1) To obtain a relation between clinical and electrophysiologic characteristics in patients with Wolff-Parkinson-White pattern. To evaluate the atrial vulnerability for inducing atrial fibrillation and its relation to the electrophysiologic protocol phase. (3) To study the effects of flecainide on the treatment of induced sustained supraventricular arrhythmias. SETTING: Department of Cardiology at Santa Marta Hospital. METHODS: An electrophysiologic testing was performed in twenty-three consecutive patients aged from 14 to 55 years, with Wolff-Parkinson-White (WPW) pattern on ECG. Two groups of patients were considered: group I with 12 asymptomatic or nearly asymptomatic patients; group II with 11 symptomatic patients. The parameters studied were the anterograde effective refractory period of the accessory pathway, the number of patients with different types of supraventricular tachycardias (SVT), the relation between the stimulation protocol phase (scanning/DDT/burst) and the occurrence of atrial fibrillation, and the effects of the flecainide on the treatment of induced sustained supraventricular arrhythmias. RESULTS: Statistically significant differences were obtained between group I and II concerning the number of patients in whom induced atrial fibrillation with conduction by the accessory pathway and RR < or = 250 msec was found (0 vs 6, p = 0.0045). No differences were obtained among the anterograde refractory period, the induction of atrial fibrillation independently of the type of atrioventricular conduction, and the induction of atrioventricular reentrant tachycardia and or atrial flutter. The induction of atrial fibrillation was attained during the Scanning/DDT protocol phase in 7 patients in whom this arrhythmia was found with conduction by the accessory pathway and in 2 without preexcitation. All the induced sustained supraventricular arrhythmias were converted by intravenous flecainide. CONCLUSIONS: (1) Only the symptomatic patients have a high risk profile which suggest that electrophysiologic testing do not need to be performed in those with no symptoms. (2) As atrial vulnerability is higher in patients with induced atrial fibrillation and conduction by the accessory pathway, a more intensive ambulatory control seems to be necessary. (3) The results of intravenous flecainide administration on the treatment of sustained induced supraventricular arrhythmias, advocate its use during the electrophysiologic procedures in patients with ventricular preexcitation and WPW pattern.

Experimental study of the efficacy of the endoluminal prosthesis in colonic anastomoses.

Colorectal surgery is still associated with a significant morbidity and mortality rate, mostly related to suture failure. We have carried out a randomized experimental study in dogs on colonic anastomoses exposed to a number of anastomotic risk situations. A total of 42 dogs was used. They were divided into three study groups (control, occlusion and diverticulitis), with and without an endoluminal tube. The aim was to assess the efficacy of the endoluminal prosthesis using clinical and radiological assessment of anastomotic healing. Pre- and post-anastomotic intraluminal pressures were also measured to determine whether these might be a factor in suture failure. There were significant differences in suture failure in animals in which the endoluminal tube was used. Mean duration of placement was 10.5 days. In the colon healing study, no significant differences were found between the groups in the pathological examination or in the assay of hydroxyproline content. The presence of the endoluminal tube may increase the maximum pressure applied on the colon wall. No significant differences were found in the intracolonic pressure differentials between the different groups or after the inclusion of the endoluminal prosthesis. The results obtained establish the efficacy of the endoluminal prosthesis in protecting the colonic anastomosis, and could be a valuable technique in colonic anastomoses with a high risk of suture dehiscence.

Coronariographic significance of heart rate variation related to ischemic episodes in patients undergoing peripheral vascular surgery.

OBJECTIVES: To evaluate the characteristics of myocardial ischemia during daily life and their coronariographic significance in a group of patients with proven coronary artery disease undergoing peripheral vascular surgery. SETTING: Department of Cardiology--Central Hospital--Lisbon. METHODS: In 14 patients undergoing peripheral vascular surgery and in whom coronariography revealed coronary significative lesions, Holter monitoring was performed during a 24-hour period. Two groups of ischemic episodes were considered: Group A constituted by 44 episodes detected in patients with left main or three vessel disease and group B by 12 episodes detected in patients with one ot two vessel disease. In each group the ischemic parameters were studied. RESULT: The incidence of myocardial ischemia was 64%. Statistically significant differences were observed between group A and B concerning the mean heart rate variation from two minutes before the onset of ST-segment depression to its onset (2.39 bpm vs 8.75 bpm; p < 0.05), from the onset of ST-segment depression to its maximal depression (4.43 bpm vs 16.67 bpm; p < 0.001) and from two minutes before St-segment depression to its maximal depression (6.82 bpm vs 25.4 bpm; p < 0.00001). No differences were found in duration and maximal ST-segment depression. CONCLUSION: Particular characteristics of heart rate variation related to the ischemic episodes seem to have a relation with the severity of coronary artery disease in patients undergoing peripheral vascular surgery.

Functional significance of premature ventricular complex morphology evaluated during treadmill exercise stress test in patients with coronary artery disease.

OBJECTIVES: To evaluate the relation between premature ventricular complexes morphology and left ventricular myocardial systolic function in patients with proved coronary artery disease. SETTING: Department of Cardiology at General Hospital. METHODS: From 112 patients (pts) with proved coronary artery disease, thirty-three pts with premature ventricular complexes (PVC) detected during treadmill exercise stress test were selected. Two groups of pts were considered: group I-20 pts with regular contour or with a narrow notching (< 40 ms; type I PVC) and group II-13 patients with wide notching (> 40 ms; type II PVC). In each group the following parameters were studied: ejection fraction, QRS duration, duration of exercise, number of metabolic units and the prevalence of ventricular aneurysm and mitral regurgitation. RESULTS: Statistical significant differences were found between group I and II concerning the mean ejection fraction (59.2% vs 48.6%; p < 0.05). The prevalence of ventricular aneurysm and mitral regurgitation was higher in patients with type II ventricular premature complexes (10% vs 30%). CONCLUSION: Particular characteristics of premature ventricular complexes detected during treadmill exercise stress test, in patients with proved coronary artery disease, suggest the presence of left ventricular myocardial systolic dysfunction.

[Shigellosis in children of the IX region of Chile: clinical and epidemiologic aspects and antibiotic sensitivity].

Ninety four children with diarrhea and a positive stool culture for Shigella, hospitalized at the Hospital Regional de Temuco, were studied. Forty six percent of patients were less than two years old. Forty two percent of microorganisms were resistant to Ampicillin, 45% to trimethoprim/sulfamethoxazole, 8% to tetracycline and none to chloramphenicol. Isolated species were Shigella flexneri 83% and Shigella sonnei 17%. Seventy nine percent of patients had fever, 60% dysentery and 21.3% seizures. Ninety two percent of symptomatic family contacts had a positive stool culture for Shigella. Due to the high incidence of resistance to ampicillin and trimethoprim/sulfamethoxazole, these antimicrobials are not recommended as the first choice treatment of Shigellosis in the Ninth region of Chile.

[Blood donation in the Balearic Islands. Analysis of supply and demand of blood from 1982 to 1987].

BACKGROUND: The supply and consumption of blood products in the autonomous community of the Balearic Islands were evaluated for the years 1982 to 1987 to assess the degree of hemotherapy coverage. METHODS: The registries of blood banks from 1982 to 1987, the clinical records of patients with congenital coagulation deficiencies and the registries of the hospital pharmacies and pharmaceutic distributing agencies were reviewed. RESULTS: The rates of use per product (units) and per one thousand population were as follows: red blood cells 22-25, platelets 0.8-3.2, transfusional plasma 0.5-2.8, and cryoprecipitates less than 0,2. The consumptions of albumin and gammaglobulins were 80 and 7.7 kg per one million population, corresponding to 3.200 and 1.540 I of plasma, respectively. The consumption of factor VIII was 1.3-0.9 international units (IU) per inhabitant, in highly of very highly purified form, corresponding to 18.500-12.857 I of plasma. CONCLUSIONS: The results show that a supply of 25-27 blood donations per one thousand population and per year and with a fractionning of 80%, yield 5.000 I of plasma per one million population. This fulfils the needs of all blood products, except those of highly or very highly purified factor VIII.
