Morphological characteristics of human endometrial epithelial cells cultured on rat-tail collagen matrix.

Epithelial cells were isolated from normal human endometrium and cultured on reconstituted matrices of rat-tail collagen gels. Cells attached within 24 h after plating. Initially, epithelial cells were not structurally polarized, had projections from both apical and basal domains and were generally flat to cuboidal in shape. When the gels were made free floating 36-48 h after initiation of the cultures, the epithelial cells became columnar in shape as the gels underwent contraction. Although a maintained growth profile was observed during the 10 days of culture, there was a linear decrease in gel matrix diameter along with increasing loss of transparency. Electron microscopy revealed the presence of apical microvilli, junctional complexes, lipid droplets and endoplasmic reticulum in cultured epithelial cells. The basolateral dilatations and lateral membrane plications were seen in these cells by 6-8 days in culture. With gel contraction, rearrangement of matrix material was observed. Occasionally there were basal lamina-like structures adjacent to the flattened basal surface and the formation of gland-like structures within the matrix. The three-dimensional primary culture of human epithelial cells on collagenous biomatrix appears to be a potential experimental tool for the study of cell-cell and cell-matrix interactions, and of the study of the effects of endocrine and paracrine factors on these cells in vitro.

Breast lumps: a study of 10 years.

A retrospective study of lumps in the breast sent for histopathological examination in Calcutta National Medical College for a period of last 10 years was done. It was found that 2062 cases (6.82%) of histopathological examination of breast lump were studied out of a total 30208 specimens during that period. There were 1528 cases from benign breast lesions and 504 cases from the malignant breast lesions. The most common lesion found histologically was fibro-adenoma in 950 cases. Others included 150 cases of inflammatory breast diseases of which 36 cases were diagnosed as tuberculosis, 386 cases as fibrocystic disease and 42 cases as other benign neoplastic diseases. Out of 504 malignant cases 456 had carcinoma and 48 had sarcoma. Only 30 cases of male breast lesions were sent of which only a single case of infiltrating duct carcinoma was found, 28 cases had gynaecomastia and one case showed the evidence of fibrocystic disease.

Experiences with fine needle aspiration cytology as a diagnostic procedure for breast lesions in a Calcutta Hospital.

Nine hundred and forty cases of breast lesions were subjected to fine needle aspiration. Nine hundred twenty-four of them were females and sixteen were males. Smears from the 60 female cases were rejected as unsatisfactory. Of the remaining 864 female cases (aged 15 to 72 years) 704 (81.4%) were benign. Histopathological correlations were done in 448 cases with diagnostic accuracy of 97.3%. One hundred and sixty (18.5%) out of 864 females were malignant and cytohistological correlation was done in 142 cases with 98.6% corroboration. Ten of the 16 male cases were histologically examined with full corroboration. Overall accuracy considering both males and females was 98.6%. Though there was underdiagnosis involving 12 cases of infiltrating ductal carcinoma when the lesions were small and deeply situated there was no overdiagnosis in this study.

Luteal phase ovarian oestrogen is not essential for implantation and maintenance of pregnancy from surrogate embryo transfer in the rhesus monkey.

The aim of this study was to investigate whether luteal phase ovarian oestrogen is required for blastocyst implantation and pregnancy maintenance in the rhesus monkey. Preimplantation embryos were retrieved from naturally ovulated, mated embryo donor monkeys. In group I, developmentally normal, age- and stage-matched embryos were transferred to recipient monkeys showing naturally synchronized ovulatory cycles. Immediately prior to embryo transfer, recipients were subjected to bilateral ovariectomy, and following transfer they were treated with i.m. injections of either progesterone (group Ia, n = 4), or oestradiol + progesterone (group Ib, n = 2). Recipient monkeys of group Ic (n = 4) were subjected to sham ovariectomy and vehicle injection. In group Ia, progesterone supplementation alone led to three pregnancies and live births. In group Ib, there was one live birth. In the control group Ic, four transfers resulted in two live births and one abortion on cycle day 58. Analysis of serum progesterone and oestradiol profiles showed that oestradiol had declined to undetectable levels within 3-5 days after ovariectomy in group Ia recipients, and the area under the curve of serum oestrogen concentrations during the peri-implantation period (days 10-20 after ovulation) were less (P < 0.001) in group Ia compared with group Ic. There were no changes in the area under the curve among serum progesterone concentrations in all the subgroups. In group II, long-term ovariectomized embryo recipients (n = 4) were primed with oestradiol till cycle day 11 of simulated transfer cycle, and received progesterone treatment from cycle day 10 till the end of the experiment.(ABSTRACT TRUNCATED AT 250 WORDS)

Incidence of malignancy in Bankura (a retrospective study).

A retrospective study on pattern of malignant lesions in a rural district of West Bengal from 1981-1990 was conducted in the department of pathology, BS Medical College, Bankura, West Bengal. A total number of 17,130 cases were received out of which 1976 were malignant lesions (11.53%). The peak incidence of cancer in both sexes was found between the 4th, 5th and 6th decades, ie, 31-60 years of age. Sex-wise distribution showed more incidence in female (1246-62.17%) than in male (730-37.83%). Cancer of the cervix (35.62%) showed the highest incidence in this series. Cancer of the larynx (11.94%) was the commonest cancer in males. Malignancy of the lymph node was observed in 11.43% cases. Malignancy of the lungs and vagina (< 0.5%) showed very low incidence.

Luteal phase contraception with mifepristone (RU 486) in the rhesus monkey.

Mifepristone (RU 486), an antiprogesterone, is a promising luteal phase contraceptive agent for human use. However, at present its use is limited by the practical constraint of determining the day of ovulation for an LH + 2 day administration of the drug as indicated from experimental and clinical studies. The aim of the present study was to identify the effective period of luteal phase (luteal phase window) when a single administration of mifepristone would induce antinidatory activity without disturbing menstrual cyclicity and ovulatory pattern in the rhesus monkey. RU 486 (2 mg/kg body weight in benzyl benzoate/olive oil, 1:3) was given to mated monkeys (n = 9) on cycle day 16 in the first treatment cycle (treatment group T1, n = 9), and in the following cycle on cycle day 20 (treatment group T2, n = 8). A single s.c. injection of this antiprogestin during early to midluteal phase (days 1-10 after ovulation, as determined from retrospective analysis of serum concentrations of estrogen and progesterone) provided a one hundred per cent protection against pregnancy, with no apparent side effects. There were no changes in cycle lengths (F = 3.5; P < 0.3), day of ovulation (F = 1.8; P < 0.7) and duration of menses (F = 3.5; P < 0.3) compared with the pre-treatment and post-treatment cycles. Pooled analyses of serum concentrations of estrogen and progesterone during luteal phases of T1 and T2 cycles also showed no variations with those in pre- and post-treatment cycles.(ABSTRACT TRUNCATED AT 250 WORDS)

Morphological characteristics of preimplantation stage endometrium in the rhesus monkey.

The morphological characteristics of endometrium on day 6 after ovulation of conception (group 1) and non-fecund, menstrual (group 2) cycles have been studied in the rhesus monkey (n = 30). A conception cycle was distinguished by the presence of a developmentally normal, age-stage-synchronized embryo. Thus, 78% of the mated cycles (n = 18) yielding synchronous embryos (12 zona-encased and two zona-free blastocysts) were used for this study. On day 6 after ovulation, no significant changes in the serum concentrations of oestrogen and progesterone were seen in conception cycles (n = 14) compared with the non-mated, normal ovulatory cycles (n = 12). Morphometric analyses revealed that on day 6 of gestation (n = 8), endometrium differed from the corresponding non-mated luteal phase (n = 7) with significant increases in epithelial mitosis (P < 0.01), height of glandular epithelium (P < 0.05), volume ratio of gland cell to gland (P < 0.03), degree of pseudostratification (P < 0.02), and higher frequency of supranuclear, adluminal accumulation of vacuoles in gland cells (P < 0.05). The degree of stromal oedema was higher (P < 0.02) in fecund cycles but there was no change in venular diameter. In a separate set of experiments, estimates of tissue vascular response revealed a higher (P < 0.02) endometrial extravascular albumin space on the same day of gestation; there were no differences, however, in endometrial blood volume, or in the number of von Willebrand antigen-positive capillaries and small vessels between the two groups (group 1, n = 6; group 2, n = 5).(ABSTRACT TRUNCATED AT 250 WORDS)

Anti-nidatory effect of a single, early post-ovulatory administration of mifepristone (RU 486) in the rhesus monkey.

The hypothesis that post-coital administration of mifepristone (RU 486) as a single dose in the early luteal phase can be an effective anti-nidatory strategy was tested using the rhesus monkey as the experimental model. Incidence of pregnancy, vaginal bleeding patterns, profiles of menstrual cyclicity and of serum levels of progesterone and oestrogen were examined following administration of RU 486 as a single dose of 10 mg/kg and 2 mg/kg body weight on the second day after ovulation. In control monkeys (group 1; n = 5) receiving the vehicle alone (benzyl benzoate:olive oil, 1:4, v/v) there was a 60% pregnancy rate. Following s.c. administration of RU 486 at both doses, no pregnancy was recorded in a total of 33 treatment cycles in 12 monkeys. Five monkeys received RU 486 at 10 mg/kg s.c. (group 2) in three consecutive cycles. All animals had complete inhibition of implantation; in addition, the treatment cycle length was prolonged (P < 0.001) due to an extension of the luteal phase. The subsequent follicular phase was unaffected. Mild, premature vaginal bleeding during the luteal phase was recorded in five treatment cycles, 3-5 days after drug application. Though the serum profiles of progesterone and oestrogen in these monkeys showed marked individual variations, there was a characteristic progesterone rebound about 18-20 days after drug administration. Monkeys in group 3 were given RU 486 at 2 mg/kg, s.c. either for three consecutive cycles (group 3a; n = 4) or for two consecutive cycles (group 3b; n = 3). Contranidatory activity of early luteal phase administration of mifepristone in the rhesus monkey.

The contranidatory action of mifepristone (RU 486) given as a single application at different dosages to mated rhesus monkeys (Macaca mulatta) on second day after ovulation has been examined in the present study. In group 1, monkeys (n = 3) received only vehicle (benzyl benzoate: olive oil, 1:4, v/v) and were treated as controls. In group 2 monkeys (n = 4), RU 486 was given by gavage at 10 mg/kg in group 4 (n = 5). The patterns of cycles and profiles of serum estrogen and progesterone were monitored for assessing the occurrence of implantation and pregnancy. At a single dose of 10 mg/kg, RU 486 was found to be ineffective in preventing nidation, resulting pregnancy in three females out of four treated monkeys. Similarly, an s.c. administration of 1 mg/kg could provide pregnancy protection in two of the four treated monkeys. In these monkeys, however, the menstrual cycle characteristics were not affected as compared to pretreatment cycles. Interestingly, the administration of 2 mg/kg, s.c., RU 486 could provide a hundred percent pregnancy protection in mated monkeys, and there was no significant changes in the pattern of menstrual cycle characteristics. It appears that an early post-ovulatory administration of RU 486 may be successfully used in human as an effective once-a-month, early luteal phase contranidatory agent.

Malignant lymphoma--a 15-year study report.

Analysis of incidence of different types of malignancies during 15 years showed occurrence of malignant lymphoma in 192 cases (4.1%). There were 82 cases (42.7%) of non-Hodgkin's lymphoma and 110 cases (57.3%) of Hodgkin's disease. Lymphocytic lymphoma, the major type observed among the non-Hodgkin's lymphoma with 58 cases (30.21%), showed a preponderance of well differentiated type (40 cases). Other cases in this group comprised mixed cellularity (2.08%), histiocytic (3.64%) and unclassified one (6.77%). Analysis of the Hodgkin's disease cases showed majority in it belonged to the mixed cellularity type (44.5%) followed by the lymphocytic predominant type (27.3%), lymphocytic depletion type (18.2%) and nodular sclerosing type (10%). The incidence of 14 cases of extranodal lymphomas was 7.3% among the lymphomas with maximum occurrence in the gastrointestinal tract (42.8%).

Sandwich immunoassay of small molecules. I. Investigation with testosterone as model hapten.

A new strategy has been developed for enzyme immunoassay of small molecules. The method is based on the formation and estimation of the complex Ab.H-H.Ab* where Ab and Ab* are immobilised and labeled antibodies respectively and H-H is a synthetic bis-analogue of the hapten H. The amount of the complex formed is inversely related to the concentration of free hapten present. The estimation of the sandwich complex increases the sensitivity and the specificity of the assay compared to conventional assays. Using the N,N'-diamide of testosterone-3-(O-carboxymethyl)oxime with ethylenediamine and a polyclonal rabbit anti-testosterone antibody (for both labeling and immobilisation) a sensitivity of 100 fg/well (3.5 x 10(-16) mol) was obtained for testosterone. The cross-reactivity of 5 alpha-dihydrotestosterone was 16% compared to 27% obtained by conventional competitive assays.

Sandwich immunoassay of small molecules. II. Effects of heterology and development of a highly sensitive and specific enzyme immunoassay for testosterone.

The effect of dimer heterology in the sandwich immunoassay of testosterone was studied using symmetrical and asymmetrical dimers prepared from testosterone-3-(O-carboxymethyl)oxime and 4-(carboxymethyl-mercapto)testosterone. The effect of antibody heterology was studied using antibodies against 3-carboxymethyl oxime and 17-hemisuccinate derivatives of the steroid and an asymmetrical dimer prepared from the same two derivatives. Using antibody against the 3-carboxymethyl oxime and the dimer of 4-(carboxymethylmercapto)testosterone, the sensitivity of direct enzyme immunoassay of testosterone in serum by this method was 0.5 pg/well and the cross-reactivity of 5 alpha-dihydrotestosterone was 5%.

Effect of RU 486 on the endometrial response to deciduogenic stimulus in ovariectomized rhesus monkeys treated with oestrogen and progesterone.

Using the artificial plaque--decidual cell model in rhesus monkeys, the potential role of progesterone in the process of of epithelial and stromal cell responses was investigated by time-adjusted application of an antiprogestin, RU 486. Epithelial plaque formation is an immediate response of the endometrium to either trauma or invading trophoblast cells. To study this process, RU 486 (2.5 mg/kg body weight) or vehicle (ethanol/saline, 7:3, v/v, i.m.) was administered to the first group of monkeys immediately following traumatization (days 16 and 17 of treatment cycle). Histometric analysis revealed that treatment with RU 486 led to significant inhibition (P less than 0.001) in the recruitment of epithelial cells into plaque acini and consequent reduction (P less than 0.001) in the spread of the plaque reaction compared with control monkeys. In the second group of monkeys, experimental treatment was delayed until plaque formation had occurred (days 21 and 22 of cycle). RU 486 induced increased degeneration (P less than 0.01) in plaque cells. Thus the transformation and the maintenance of the epithelial plaque response appears to be progesterone-dependent. Glandular epithelium showed only marginal changes (P less than 0.05) in maximum cell height, amount of pseudostratification and secretory activity of glands as a consequence of RU 486 treatment; however, the antiprogestin induced a higher incidence of glandular apoptosis (P less than 0.01 for both groups). It has been suggested that the higher degree of apoptosis in the glandular epithelium could be a consequence of the progesterone receptor blocking action of RU 486. No distinctive change in endothelial cell ultrastructure was evident following RU 486 treatment; however, venular diameter was significantly increased (P less than 0.01, group I; P less than 0.001, group II) along with an apparent reduction in the extent of oedema. There was increased extravasation (P less than 0.01) and leukocytic infiltration (P less than 0.001, group I; P less than 0.05, group II) following RU 486 treatment. RU 486 induced vascular responses and increased diapedesis could presumably have resulted from its progesterone blocking action in vascular cells. RU 486 accelerated the incidence (P less than 0.01) of stromal decidualization (group I), as well as quantitatively accentuating (P less than 0.001) the decidual cell reaction (group II). It is possible that RU 486 may inhibit specific functions of decidual cells, despite morphologically consistent decidual transformation.

Patterns of ovulation, conception and pre-implantation embryo development during the breeding season in rhesus monkeys kept under semi-natural conditions.

The occurrence of ovulation and conception in the female rhesus monkey (Macaca mulatta) under natural conditions is seasonal and maximum during the months of October to March. Such seasonality in reproduction suggests dependence on environmental factors like photoperiod, temperature and rainfall. There occur significant fluctuations in these environmental factors also during the breeding season itself. Whether there occur any discernible changes in the occurrence of ovulation and conception, and in the incidence of abnormality in pre-implantation stage embryo development in the rhesus monkey, associated with changes in environmental factors within the breeding season has not been known and was investigated in the present study using a total of 304 menstrual cycles over a period of five years. No preferential sidedness for any ovary to ovulate more was evident, the frequency ratio of ovulations in left ovary to right ovary being 1.02. Despite the absence of any pattern in the side selection of ovulations in successive cycles, the chance of ovulations in alternate ovaries for successive cycles appeared more likely (67% cases). An average of 21% of menstrual cycles were found to be non-ovulatory with significantly higher menstrual cycle lengths (p less than 0.03) compared with that of ovulatory cycles. An additional 28% of the mated, conception cycles yielded degenerated or desynchronized pre-implantation stage embryos. Furthermore, the number of non-ovulatory cycles (p less than 0.05) and the incidence of abnormality in pre-implantation stage embryos (p less than 0.02) were higher during the months of transition in (October) and out (March) of the breeding season.(ABSTRACT TRUNCATED AT 250 WORDS)

Endometrial response to deciduogenic stimulus in ovariectomized rhesus monkeys treated with oestrogen and progesterone: an ultrastructural study.

The present work continues our aim of establishing an experimental model to study the decidual cell reaction to an artificial deciduogenic stimulus in the long-term ovariectomized rhesus monkey treated with oestrogen followed by progesterone. The fine structural details of decidual, granular and plaque cells, which constituted the endometrial cellular response to the deciduogenic stimulation in the present study, revealed striking similarities with those reportedly present in an endometrial response to blastocyst implantation in the rhesus monkey. Plaque epithelia showed a significant degree of hypertrophy, hyperplasia and differentiation followed by a steady degeneration by day 32 (equivalent to day 16 after trauma) of treatment. The plaque cells were shown to contain numerous regular-shaped mitochondria, polyribosomes and large amounts of rough endoplasmic reticulum (RER) in their cytoplasm and were characteristically arranged in clusters or acini formation surrounded by discrete basal laminae. As early as day 28 of treatment, the initiation of stromal decidual cell transformation was noted and, by day 48, a sizeable pool of decidual cells was found. The decidual cells had rounded nuclei and elaborate arrangements of interconnected cisternae of RER which were often moderately dilated and filled with amorphous, electron-dense material. Granular cells were characterized by eccentrically located nuclei and numerous membrane-bound, electron-dense granules in their cytoplasm and were found in increasing numbers in the stroma around decidual cells, blood vessels and glandular epithelia.(ABSTRACT TRUNCATED AT 250 WORDS)

Estradiol and progesterone levels in the endometrium and the reproductive tract luminal washing during the pre-implantation stages of gestation in the rhesus monkey.

To obtain an understanding of the sex steroid hormonal milieu during early gestation, the concentrations of estradiol-17 beta and progesterone were measured in endometria and reproductive tract luminal fluids collected during the post-ovulatory period (days 2 to 6) of conception, and of non-mated menstrual cycles of the rhesus monkey (Macaca mulatta). Estradiol content was found to be higher (P less than 0.05) in endometria obtained from conception cycles; day-wise analysis revealed higher (P less than 0.05) level of tissue estradiol concentration on day 6 of post-ovulation as compared with the normal menstrual cycle. Endometrial progesterone content remained, however, unaltered in both groups of monkeys and thus relatively lower (P less than 0.05) endometrial progesterone to estradiol ratios were seen on days 4, 5 and 6 of gestation compared with normal menstrual cycle. There were no significant changes in the profiles and concentrations of estradiol and progesterone in luminal washings. The typical sex steroid hormonal milieu observed in the endometrium during the peri-implantation stage of gestation in conception cycle may be related to the endometrial differentiation towards implantation in the rhesus monkey.

Endometrial responses to a deciduogenic stimulus in ovariectomized rhesus monkeys treated with oestrogen and progesterone.

To establish an artificial model for the study of decidual cell reaction in the rhesus monkey, long-term ovariectomized animals were treated with oestrogen followed by progesterone and an artificial deciduogenic stimulus was applied on day 16 of the treatment cycle. Histological examinations of the endometrial samples on different days following stimulation showed prominent epithelial plaque reaction by day 20. Marked subepithelial oedema surrounding plaque cells was also noted. On day 32, degenerating plaque cells with leucocytic infiltration were found. Beginning on day 24 of hormone treatment, stromal fibroblasts adjacent to plaque and glands showed swelling with rounding-up and enlargement of nuclei; these stromal cells appeared more decidual-like with moderate amounts of hypertrophy and few were binucleate by day 28. Endometrial granular cells became more conspicuous and appeared to increase in numbers adjacent to glands and blood vessels. Endometrial glands showed no notable changes, and were mostly tortuous with columnar epithelium and apocrine-type secretions. Around days 40 and 48 of the treatment cycle, the luminal margins of the glands became bosselated, and significant amounts of secretion were present in the glandular lumen. Measurement of immunoreactive concentrations of oestradiol-17 beta and progesterone in endometria from traumatized, hormone-treated monkeys revealed significant (P less than 0.01) decreases in levels of oestradiol with time while the level of progesterone remained unchanged (P greater than 0.02).(ABSTRACT TRUNCATED AT 250 WORDS)
