Accuracy of blood volume estimations in critically ill children using 125I-labelled albumin and 51Cr-labelled red cells.

Blood volume was estimated using 51chromium labelled red cells and 125iodinated human serum albumin in 5 children with sepsis, in 6 burned children and 7 children with acute lymphoblastic leukaemia. Studies of the equilibration pattern demonstrated that the mixing time of labelled red cells was prolonged to 40 minutes or more in 5 children, indicating the existence of slowly circulating red cells. Mixing of labelled albumin was complete within 10 minutes in 15 patients and within 20 minutes in all the children studied. In a burned patient with severe sepsis, exchange transfusion improved the clinical state and normalized the equilibration pattern of labelled red cells. The mean body/venous haematocrit ratio was 0.893+/-0.018 (SD) in the children with sepsis, 0.859+/-0.052 in the burned patients, and 0.916+/-0.078 in the children with acute lymphoblastic leukaemia, increasing with spleen size in the latter group.

[Long-term therapy using horse anti-dog lymphocyte globulin without sensitization against horse protein].

Eight mongrel dogs received a standard daily i.v. infusion of 20 mg/kg b.w. deaggregated horse-anti-dog-lymphocyte-globulin (ALG) and additional prednisolone (1 mg/kg b.w. daily i.v.) over a maximum period of 82 days following pretreatment with deaggregated normal horse IgG. No sensitization against horse protein was observed during therapy of afterwards as proved by lack of humoral antibodies against horse antigens, maintained lymphopenia, good compatibility, longterm prolongation of xenogeneic skin graft survival (85.6+/-20.6 days, n=8' untreated controls 12.5+/-1.3 days, n=4) and longterm suppression of cytotoxic antibodies against donor lymphocytes. The level of preformed agglutinating antibodies against horse erythrocytes was significantly reduced, while preformed antibodies against other species remained normal. The immune response to a challenge injection of anti-lymphocyte-serum (ALS) 6-11 weeks after termination of treatment was significantly lower in the ALG treated animals as compared to the control group. These results suggest the involvement of a specific mechanism of unresponsiveness against ALG other than immunosuppression only. It is concluded, that by the described method sensitization against ALG can be prevented during longterm treatment.

High incidence of horse serum protein allergy in various autoimmune disorders.

In 186 persons (68 patients suffering from different so-called autoimmune diseases, 30 kidney recipients, 38 control patients from a surgical ward, and 50 healthy volunteers) the immune response to horse IgG was examined. The lowest rate of sensitization was found in kidney transplant recipients (3%) and the highest in autoimmune patients (33%). After excluding 39 patient who had received horse serum treatment prior to the examination, it was found that without previous injection of horse serum, 27% of the patients with autoimmune disease were sensitized to horse IgG. Compared to the other groups (kidney transplant recipients, 4%: surgical controls, 0%; healthy volunteers, 3%), this difference was statistically significant (p is less than 0.01).

Decreased utilization of [2-14C]orotic acid for the synthesis of cytidine nucleotides in rat liver after administration of alpha-hexachlorocyclohexane.

Administration of alpha-1,2,3,4,5,6-hexachlorocyclohexane (alpha-HCH) to rats decreased the utilization of [2-14C]orotic acid for the synthesis of liver cytidine nucleotides. The specific radioactivities of uridine components of the acid-soluble pool and rRNA increased during the first hours of treatment with the drug. Later on the specific radioactivities of uridine nucleotides remained unchanged, while those of cytidine components decreased gradually. Administration of hydrocortisone increased the incorporation of labelled orotic acid into rRNA cytidylic acid.

Influence of salts on the microbial activity in arid and simi-humid soils.

The influence of salt mixtures consisting of Ca (H2PO4)2, trace elements, CaSO4, CaCO3, Na2CO3, NaCl, and K2SO4 in different combinations on the nitrifying power, evolution of carbon dioxide, and the total number of bacteria was studied in arid soils (sandy and alluvial) and semi-humid ones (chernozem and rendzina). Salt mixtures comprising either monocalcium phosphate or sodium chloride showed highly inhibiting action on the studied microbial activities in sandy, alluvial, and chernozem soils, while monocalcium phosphate stimulated the heterotrophs of rendzina. Trace elements favoured the autotrophs of the caly soils. The other different salt mixtures acted according to the salt combination and nature of the microbial activity.

[Radioactive microsphere studies of stomach wall blood flow in dogs following truncular vagotomy].

Blood flow in the mucosa of fundus and antrum behave quite contrary after TV: Whereas mucosal flow of the fundus is reduced, mucosal flow of the antrum increases significantly. But 3 weeks after TV flow values of the fundus are in the range of the initial blood flow and are decreased significantly in the antrum. The early diminution of the blood flow in the mucosa of the fundus can be partly responsible for the favourable effect of TV, which is observed on erosive bleedings in this region.

Anaphylactoid reactions due to non-immune complex serum protein aggregates.

The infusion of aggregate-containing i.v. human gamma-globulin as well as human serum albumin can lead to severe anaphylactoid reactions with decrease in mean arterial pressure, increase in pulmonary artery pressure and decrease in cardiac output in unsensitized dogs, while the deaggregated solutions are well tolerated. During these anaphylactoid reactions, no significant changes in the serum complement activity of the dogs were observed. In clinical human serum albumin incompatibility, stimulation with albumin aggregates led to a high response in the lymphocyte culture, whereas deaggregated albumin had no stimulatory effect. By deaggregation of horse anti-human lymphocyte globulin prior to clinical administration, the compatibility of ALG therapy was improved.

[Scoring system for evaluation of severity and progress of chronic posttraumatic osteomyelitis (author's transl)].

From the anamnestic data of 50 patients suffering from chronic posttraumatic osteomyelitis a severity scoring system was developed, based on the duration of the disease, the time of hospitalization and the number of surgical interventions. The clinical course of 18 patients, undergoing an autovaccination treatment, was registered by another scoring system ("Munich-Murnau-Scale") regarding wound morphology, bone radiology and erythrocyte sedimentation rate.

Cumulation and elimination of horse-anti-dog lymphocyte and normal horse gammaglobulin in dogs.

Two groups of dogs received daily intravenous doses of 20 mg/kg 131-I-labelled horse-anti-dog lymphocyte globulin or normal horse gammaglobulin respectively over a period of 11 days. Horse-anti-dog lymphocyte globulin showed a significantly higher eleimination rate than normal horse gammaglobulin. In contrary to the continuous increase in serum radioactivity during normal horse gammaglobulin treatment, there was a plateau after the 5th day in the horse-anti-dog lymphocyte globulin group. The xenogeneic protein concentration, measured with the single radial immunodiffusion technique, at the end of treatment was 165 +/- 8 mg/1 in the horse-anti-dog lymphocyte globulin, compared to 498 +/- 15 mg/1 in the normal horse gammaglobulin group. After treatment horse-anti-dog-lymphocyte globulin treated animals showed a significantly higher increase in active hemagglutination titer against horse erythrocytes with an average of 2(-8).

[Chronic posttraumatic osteomyelitis. Attempt at oral autovaccine therapy].

18 patients suffering from chronic posttraumatic osteomyelitis were treated by oral autovaccination. 11 patients showed significant improvement of clinical condition. The therapeutic effect was evaluated by a scoring system considering the wound morphology, the bone radiology and the erythrocyte sedimentation rate. The most pronounced clinical improvements were observed in patients with established delayed type hypersensitivity against staphylococci.

[Chronic posttraumatic ostemyelitis. 3. Immunopathological aspects].

3. Immunological aspects 90 patients suffering from chronic posttraumatic osteomyelitis were examined immunologically. In 10-20% of the patients serum immunoglobulin levels were changed. Only 45% of the patients with staphylococcal infection had a positive anti-staphylolysin titer. In 10% of the patients so-called wound-specific antibodies were demonstrated; their possible autoimmune origin is discussed. A comparison of three groups of patients according to the immunological reactivity "hypoimmune", "humoral hyper-immune" and "cellular hyperimmune" showed that the worst clinical courses were observed in patinets with established delayed type hypersensitivity.

[Chronic posttraumatic osteomyelitis. 2. Circulation problems].

Blood volume and total body water was determined in 20 patients suffering from chronic posttraumatic osteomyelitis and compared to the values from 15 healthy volunteers. Only patients who were unable to leave the bed, showed a marked decrease in circulating blood volume of more than 10%. Total body water remained essentially unchanged.

[Chronic posttraumatic osteomyelitis. Clinical aspects and bacteriology].

50 patients suffering from chronic posttraumatic osteomyelitis were examined. In 66% an open fracture, caused either by traffic or industrial accidents was the primary reason for hospitalization. The most frequent pathogenic bacterium was staphylococcus aureus: 80% of the patients suffered from staphylococcal infections, mostly, however, in mixed culture with pseudomonas aeruginosa or clebsiella. Whereas the peripheral leukocyte count and the serum electrophoresis showed abnormal values only in a small percentage of the patients, the erythrocyte sedimentation rate proved to be a reliable index of the intensity of chronic infection.

[Humoral and cell-mediated immune reactions in chronic posttraumatic osteomyelitis (author's transl)].

The humoral and cell mediated immune response was investigated in 90 patients suffering from chronic posttraumatic osteomyelitis. In 10-20% of the patients the serum IgG and IgM was increased, in 5-10% the levels were decreased. The values were compared with those of a control group of 35 normal healthy volunteers. Of 55 patients with staphylococcal infection only 25 had a positive antistaphylolysin titer. Eleven out of 25 patients with staphylococcal osteomyelitis showed a positive in vitro lymphocyte transformation after stimulation with alpha-staphylolysin (mean transformation rate 5.4 equal to or less than 0.9). The phytohemagglutinin and the pokeweed-mitogen response were normal. A comparison of the immunological reactivity of the patients and the clinical severity of the disease showed that patients with established delayed-type hypersensitivity phenomena suffered a significantly worse clinical course of the disease.
