Pathogenetic role of adipose tissue lipase deficit for development of hypertriglyceridaemia.

In 87 patients (74 males, 13 females; age 43 +/- 9.2 years) covering a wide range of triglyceride (TG) concentrations (8.8 to 80.7 mmol/l) the turnover of total serum TG has been measured using the technique of labelling endogenously synthesized TG with 3H-glycerol. In parallel, lipoprotein lipase activity in adipose tissue and the adipose-tissue lipase activity in post-heparin plasma have been assessed following an oral 50 g glucose load. Despite the well-recognized function of this enzyme for intravascular lipolysis no direct relationship between lipase activities and fractional catabolic rates (FCR), reflecting TG removal processes, was found. On the other hand, relative enzyme activities (per TG moles) are positively correlated with the FCR. However, a detailed analysis of this relationship revealed that only in HTG patients whose FCR are below 0.210 h-1 a rate-limiting influence of a relative lipase deficit can be shown. In these patients, a statistically significant elevation of concentrations of free fatty acids is supposed to contribute to the impairment of the TG removal.

Clinical study on the effect of biotin on skin conditions in dogs.

In a collaborative study with small-animal veterinary surgeons, dogs with fur and skin conditions were treated with biotin (approximately 5 mg biotin/10 kg body weight/day) for 3 to 5 weeks. In total 119 cases could be treated which were reported to show symptoms such as dull coat, brittle hair, loss of hair, scaly skin, pruritus or dermatitis. Cases requiring other treatments with e.g. glucocorticoids, were excluded from the study. In 60% of the cases all symptoms were reported to be cured after the biotin treatment and in a further 31% an improvement was noted; in only 9% no effect was recorded. All breeds responded but to a variable extent: e.g. in Poodles the response was lower (no response in 6 out of 11 cases) than in Alsatians where all improved and 14 out of 29 were completely cured. The results confirm the favourable effect of biotin for treatment of fur and skin conditions in dogs.

[Intensive basic therapy decreases the need for drug treatment and improves glucose control: an important result of the diabetes intervention study].

In a period of five years in the diabetes intervention study (DIS), 1,139 diabetics aged 30-55 years, who in a 6-week screening phase had been classified as dietetically manageable, were treated in a controlled randomized study with intensified non-medicamentous basic therapy and--in a double blind trial--with 1,6 g clofibric acid. The essential components of the basic therapy were a fat-modified diet (so-called prudent diet) and recommendations for endurance training. Thus in comparison to the control group we succeeded in performing a ca. 40% decrease of the application of oral antidiabetic drugs. The fasting blood values were, nevertheless, with 155 mg% significantly lower than in the control group (169 mg%). Clofibric acid had no influence neither on the necessity to use insulin or oral antidiabetic drugs nor to the control of glycaemia.

[The value of hypertension and metabolic factors on the development of coronary heart disease in type II diabetic patients].

In 1,126 newly manifested type II diabetics of the diabetes intervention study (628 male, 498 female) between 30 and 55 years of age by a univariate and multivariate analysis the influence of atherogenic risk factors on the development of the coronary heart disease was tested. The diagnosis coronary heart disease is based on the ECG at rest (Minnesota-code), in 796 test persons an exercise electrocardiogram was made. In a manifestation of diabetes males with coronary heart disease showed higher mean values for systolic and diastolic blood pressure, body mass index and serum triglycerides than the control group free of coronary heart disease. In females systolic and diastolic blood pressure, uric acid level and age were significantly increased. By means of multidimensional analysis of variance as to the major findings in the ECG at rest (Q/QS type) in males the diastolic blood pressure, in females the systolic blood pressure and the serum uric acid proved to be significant. Minor findings (above all disturbances of repolarisation) were in males significantly associated with the triglycerides and the diastolic blood pressure, in the females with the systolic blood pressure and the age. Test persons with a stage of coronary heart disease which is to be recognized only in the exercise ECG in the behaviour of the mean value of the atherogenic risk factors did not differ from the control group free of findings with the exception of the increased serum triglycerides in the females.

[Liver morphology and clinical aspects of a case of cholesterol ester storage disease].

Liver specimen morphology and clinical course of a case of cholesterol ester storage disease are presented. In a 16-year-old boy on the first biopsy a massive storage of neutral fats was found light microscopically and of cholesterol ester micropolariscopically. Advanced portal field fibrosis and severe reactive hepatitis indicate the danger of progression up to metaplasia. Typical membrane-surrounded lipid inclusions were found electron microscopically in hepatic cells, stellate cells and also in bile duct epithelium.

[Metabolism kinetic characterization of hypertriglyceridemias in drug therapy variations].

In 26 male patients with primary hypertriglyceridemia (HTG) the radioglycerol labelling technique was used to assess triglyceride production rates and fractional catabolic rates pertaining to the rapidly turning-over compartment of endogenously produced triglycerides (TG). On the basis of these data the patients were allotted to 3 groups: group I with predominating TG overproduction, group II with predominating disturbances of TG removal from the bloodstream, and group III with a combination of both dysfunctions. All patients were examined prior to and after 4 weeks of drug intake at the metabolic ward. Each group got a medical treatment aiming at removing the pathogenetic cause of the HTG. Biguanides used in group I decreased TG production rates without affecting fractional catabolic rates. In group II an anabolic steroid (chlormethyltestosterone) proved to be able to improve TG removal, but this improvement was accompanied by a significant lowering of HDL cholesterol and an increase in LDL cholesterol. In contrast, clofibric acid influenced both overproduction and removal impairment in group III. Our results give an insight into mechanisms of action of certain lipid-lowering drugs providing a basis for future improvements in the elimination of the risk factor HTG.

Incorporation of highly purified melittin into phosphatidylcholine bilayer vesicles.

Melittin free of phospholipase A2 was prepared. In the absence of salt this highly pure protein starts to aggregate in solution at a protein concentration of Cp greater than 10(-3) M. In high salt solution (2 M) aggregation starts at Cp greater than 10(-6) M. This was determined from the blue shift of the intrinsic fluorescence of the protein. Reinvestigation of the quenching behaviour clearly shows that self-aggregation cannot be deduced from quenching experiments using nitrate or 2,2,6,6-tetramethylpiperidine-1-oxyl as quencher. The incorporation of melittin into phosphatidylcholine bilayer vesicles was studied by fluorescence quenching and by energy-transfer experiments using 2- and 6-anthroyloxypalmitic acid as acceptor and peptide tryptophan as donor. Incorporation of melittin into small unilamellar vesicles was found to be reduced below the lipid phase transition temperature, Tt, whereas it incorporates and distributes more randomly above Tt. Cooling the temperature below Tt after incubation at T greater than Tt leads to a deeper incorporation of the peptide into the lipid bilayer due to electrostatic interaction between the lipid phosphate groups and the positively charged amino acids. This stabilizing effect is lost above Tt and melittin is extruded to the polar phase. Quenching experiments support this finding. EPR measurements clearly demonstrate that even in the presence of high amounts of melittin up to 10 mol% with respect to the lipid broadening of the phase transition curves was only observed with fatty acid spin labels, where the doxyl group is localized near the bilayer surface. The order degree of the inner part of the bilayer remains almost unchanged even in the presence of high melittin content.

[The microecological effect of human milk].

With mother's milk, in the colon a buffer system round pH 5 is dominating which consists of short chain organic acids and the corresponding anions. Thus, the activity of the microbial metabolism is retarded. The degradation of lactose remains maintained down to the faeces. With cow's milk, a neutral buffer system on the basis of phosphate, bicarbonate, and protein degradation products causes a rapid lactose degradation already in the upper colon. Putrefactive metabolites thereby released and absorbed burden the infant's still immature detoxifying capacity.

[Ballast matter from a microecological viewpoint].

In a traditional sense, ballast matter means food constituents not attacked by digestion enzymes of macro-organism. Thus, they get to the hind-gut chemically unchanged. There they can be metabolised by microbes. Intestinal bacteria also utilize substances that are usually digestible and have escaped digestion within the small intestine, for various reasons. Such substances reveal the same effects as undigestible "classic" ballast matter. Therefore, from microecological-physiological aspects it is suggested to expand the term ballast matter by so-called "optional" or "potential" ballast matter (in the small intestine usually digestible but incompletely degraded nutriments) in addition to "obligatory" ballast matter (nutriments not digestible by indigene enzymes). In experiments with rats, microecological effects caused by the "optimal" ballast matter lactose are evidenced.

[Importance of stable gastrointestinal microflora in growing animals].

A zysbiosis can be caused by food constituents. Food portions with 40% rye for chickens release a destabilisation of the gut flora. This can be prevented by effective stabilizers. Early weaned piglets harbour high numbers of E. coli in their ileum and jejunum content. These become decreased by about three tens powers by affective gut flora stabilizers. Simultaneously the body mass rises considerably without an increase of the food consumption with reference to the living mass. Partially, the ergotropic effect of the gut flora stabilizers is attributed to the smaller expense for the immune defence. Therefore, a new hypothesis for the growth stimulating effect of ergotropics for early weaned piglets is created.

[Intestinal beta-galactosidase in gnotobiotic animals following monoassociation with various E. coli strains].

Germ-free rats were monoassociated by E. coli germs not utilizing (L-) or utilizing lactose (L+) on endo-medium. There was no influence of germ status on the beta-galactosidase activity in the mucosa of the small and large intestine. With standard food, beta-galactosidase activity were to be measured in the chymus of all intestinal segments and in the faeces of germ-free as well as of monoassociated rats. In the chymus of caecum and colon and in the faeces of E. coli(L+)-animals, only, the short-time (12-16 days) application of lactose containing food resulted in an increase of the enzyme activity. Compared with the E. coli(L-)-animals, the lactose content in the chymus of all intestinal segments of this test group was decreased.

Predictive value of the index of desirable body weight for total body fat mass as measured by dilution of tritiated water--problems and limitations.

The index of desirable body weight is currently used for classification of patients' weights. We examined how it is related to the degree of adiposity, and whether it can serve in the prediction of body fat to the same degree as Quetelet's index. Sixty-five women and 142 men comprising a wide range of weight indices were studied. Body fat (F) was measured by the tritium dilution technique with a precision of +/- 1.26 kg. Regressions of F/W0 on the index of desirable weight W/W0 gave a correlation coefficient of 0.969 for women and 0.939 for men. Regressions of F/H2 on Quetelet's index W/H2 had correlation coefficients of the same order, i.e. 0.971 for women and 0.936 for men. Hence both indices are equally powerful for calculation of body fat within the study population. Further progress was achieved by multiple regression of F on W and W0 which yielded correlation coefficients of 0.976 for women and 0.953 for men. The residual standard deviation reflecting the mean difference between calculated and estimated fat was 3.4 kg for women and 3.8 kg for men. Despite this self-consistency, prediction of body fat for persons outside the study population is only possible within wide limits. These are given by about twice the residual standard deviation. Therefore, prediction of body fat from weight indices can only be used for detection of larger abnormalities in body composition.

Extrahepatic microsomal metabolism of N-nitrosodi-n-butylamine in rats.

The omega- and omega-1-hydroxylation of N-nitrosodi-n-butylamine (NDBA) has been studied in microsomes from rat liver, lung, intestine and kidney. Both reactions followed at least two enzyme kinetics with low (2-10 microM) and high (1 mM) Km values. Whereas omega-1-hydroxylation was the predominant pathway in liver, omega-hydroxylation was more important in extrahepatic tissues. First-pass metabolism of NDBA in lungs and intestinal mucosa may be of importance in the development of urinary bladder tumours in rats.
