Relation between HLA-B27 and clinical features in patients with acute anterior uveitis.

Twenty-six patients with acute anterior uveitis associated with HLA-B27 antigen were compared with 31 patients with acute anterior uveitis HLA-B27 negative. No statistical difference was found in the clinical picture of both groups except for the occurrence of unilateral or bilateral involvement. Almost all patients with bilateral eye involvement were HLA-B27 negative and had no evidence of systemic disease, while unilateral involvement occurred in both groups of HLA-B27 positive and negative patients.

Lysozyme concentration in tears of patients with sicca syndrome.

Fifty-three patients with sicca syndrome and 34 healthy controls had their tear lysozyme concentration examined. Lysozyme level was significantly decreased (P less than .0005) in all patients with low Schirmer test values as compared with healthy controls. Even in rheumatoid arthritis patients with normal Schirmer test results a low concentration of tear lysozyme was found. Tear lysozyme can be used to detect subclinical involvement of lacrimal glands in collagen diseases.

Sialochemistry for diagnosis of Sjögren's syndrome in xerostomic patients.

The flow rate and composition of whole unstimulated saliva were measured in fifteen healthy controls and in forty-eight xerostomic patients, fourteen suffering from xerostomia per se, twenty-two from xerostomia with keratoconjunctivitis sicca (KCS), and twelve from xerostomia, KCS, and rheumatoid arthritis. A significant lower salivary flow rate was found in all the xerostomic patients. Sodium, potassium, and IgA concentrations were significantly elevated in the KCS and in the RA + KCS group in comparison with the patients who had xerostomia per se and with the healthy controls. Sialochemistry is thus recommended for the diagnosis of Sjögren's syndrome in xerostomic patients.

[Experiences with the uripret-system in pediatric practice (author's transl)].

The present study reports on the application of the Uripret-System for identification and susceptibility testing of microbial agents causing urinary tract infections carried out in a pediatric outpatient clinic. The findings were compared with those determined with the conventional methods in an institute of medical microbiology. For the detected mono- and mixed-cultures, in 85% of the cases a concurrent result of both methods was found. Only in one case a different microbial agent genus was determined with Uripret from that found in the institute. The quota of concurrence of the antibiograms for the mono-cultures was highest at 100% for gentamicin and lowest at 85.07% for tetracycline. With the mixed-cultures the concurrence ranged between 81.25% for tetracycline and 96.88% for sulphamethoxazole-trimethoprim. With proteus in mono- and mixed-culture the antibiograms showed concurrence to a lesser degree compared to the other microbial agent geni. The application of the Uripret-System in pediatric practice can be recommended on the basis of the present results.

Anterior uveitis in ankylosing spondylitis: a histocompatibility study.

The occurrence of acute anterior uveitis in ankylosing spondylitis was compared in 53 HLA-B 27 positive and 12 HLA-B 27 negative patients with this disease. Uveitis was found in HLA-B 27 positive patients only. These results suggest that uveitis and ankylosing spondylitis are independent diseases occurring on their own and strongly associated with HLA-B 27.

Sialochemistry of KCS patients.

Sialochemistry was used to detect salivary gland involvement in KCS patients. Significantly elevated IgA was found in 52% of the patients, potassium in 48% and sodium in 32%. No correlation between the grade of Schirmer's test and the extent of changes in salivary composition was observed. The results indicate that salivary IgA, K and Na concentrations can be used for detection of salivary gland involvement of KCS patients.

Spontaneous extrusion of intraocular foreign body after 11 years.

A case of spontaneous extrusion of intraocular foreign body after 11 years is presented. Despite being magnetic and containing mostly iron the foreign body could not be removed in repeated magnet trials. It was retained in the ciliary body region for a very long time without causing siderosis. The foreign body was spontaneously extruded without any signs of intraocular inflammation.

Histocompatibility antigens (HLA) and open angle glaucoma.

In 70 patients with primary open angle glaucoma and 70 unrelated controls of identical ethnic origin, the prevalence of different HLA antigens were examined. There were statistically no significant differences in the incidence of the antigens in both groups. There was no association between specific HLA antigen and primary open angle glaucoma.

Fundus lesions in rheumatoid arthritis.

Cotton wool exudates were found in a patient during a flare-up of seropositive rheumatoid disease with polyarthritis and pericarditis. After therapy with cortisone a concomitant improvement of the cardiac arthritic and fundal lesions was observed. It is suggested that the fundal lesions in this patient were due to the rheumatoid disease.
