Is there a territorial differentiation in the prevalence of peptic ulcer among rural population in Poland?

The aim of this study was to determine the prevalence of peptic ulcer among rural population in various regions of Poland and to analyse the conditions influencing the prevalence of the disease. For organizational reasons, the division of the territory of Poland into eight regions was adopted for the study. The study covered a representative group of 6,512 rural inhabitants, comprising 3,107 males (47.7%) and 3405 women (52.3%), aged 20-64, selected by two-stage stratified sampling. At the first stage of the study all health centres (3,286) were classified into 150 groups and in each group two prevention-treatment regions were selected by means of stratified sampling. The second-stage samples were selected based on communes where the health centres classified for the study were located. People selected for the study were subject to examinations which covered: a specially designed questionnaire form, detailed physical examination, and the necessary specialist tests. The obtained results were recorded in a questionnaire form, which additionally contained questions concerning detailed demographic and social data, hazardous factors present in the working environment, as well as data pertaining to housing conditions, nutrition and habits. Among the rural population under study, peptic ulcer was found in 8.0% of males and 2.9% of females, gastric ulcer was observed in 1.2% of people under study, duodenal ulcer - in 3.2%, gastric and duodenal ulcer - in 0.2%, whereas patients who underwent surgical procedures due to peptic ulcer constituted 0.7% of respondents. Territorial differences were noted in the prevalence of peptic ulcer among Polish rural population. The highest peptic ulcer incidence rates were observed in Macroregion I (western Poland) - where the disease was diagnosed in 7.2% of people under study (Northern Region - 8.1%, Southern Region - 7.4%, and South-Western Region - 6.4%), while the lowest rates were noted in Macroregion II (central and eastern Poland), where peptic ulcer occurred among 4.7% of respondents (South-Eastern Region - 4.4%, North-Eastern Region - 4.5%, Middle-Eastern Region - 4.7%, Middle- Western Region - 4.8%, and Central Region - 5.1%). In regions where the highest incidence rates were noted, the greatest numbers of divorcees, widows and widowers were observed. An analysis by occupational groups showed that in these regions there were more unskilled and skilled workers, employees of services, and the largest number of people performed non-agricultural occupations. Cigarette smoking habit was also more prevalent in these regions.

[Relationships between hyperparathyroidism and Helicobacter pylori infection in long-term hemodialysis patients].

UNLABELLED: In the accessible literature we did not find data about the connection between Helicobacter pylori (H. pylori) infection and parathyroid hormone (PTH) abnormalities in patients on hemodialysis (HD pts). It is known that hyperparathyroidism is connected with stimulation of gastrin synthesis as well with increased acidity of gastric juice. We speculate that it should be connected with susceptibility to H. pylori infection in HD pts. The aim of our study was the assessment of relationships between PTH abnormalities and parameters of H. pylori infection expressed by concentration of IgG antibodies against H. pylori and histologically performed urease test. The study was conducted in 65 (37 M, 28 F) stable HD pts. They were dialyzed for 6 to 288 months (102.9 +/- 84.5). Simultaneously in 25 HD pts qualified for renal transplantation biopsy specimens taken during gastroscopy were examined by a histological test and urease test (CLO-test). According to the PTH concentration HD pts were divided into 3 groups with PTH < 100 pg/ml; with PTH 100-350 pg/ml and with > 350 pg/ml. Positive IgG H. pylori test > 24 U/ml was found in 60 (92%) HD pts. Mean IgG H. pylori concentration was similar in tree groups of HD pts. (82 vs 91 vs 88 U/ml) and did not differ significantly from control group. We found significant negative correlation between IgG H. pylori concentration and time on dialysis therapy (r = -0.50067, p = < 0.0001). Positive test in biopsy specimen was found in 14 HD pts (56%). PTH level in this group of HD pts not differ significantly from PTH level in pts with negative test (426 vs 398 pg/ml) and IgG H. pylori concentration was significantly higher in positive pts than in negative pts (104 vs 48 U/ml). CONCLUSION: We did not find significant relationship between PTH abnormalities and H. pylori infection in HD pts. Longer period of dialysis therapy is connected with decreased ability to produce antibodies againts H. pylori.

Peptic ulcer among Polish rural population and the nicotinic index.

The paper presents the results of observations concerning the effect of cigarette smoking on the prevalence of peptic ulcer among 6,512 rural inhabitants aged 20-64, selected by two-stage sampling. Of these, 2,506 (38.6%) were regular smokers. In order to determine precisely the negative effect of smoking on the human body the nicotinic index was used (N.I.), calculated by multiplying the number of cigarettes smoked daily by the period of smoking (years). The three-stage scale of the nicotinic index was applied: I degrees - N.I. < 100, II degrees - N.I. = 100-300, III degrees - N.I. > 300. The mean value of the nicotinic index calculated for the total number of smokers in the study was 290.3. A statistically significant higher N.I. was observed in patients with peptic ulcer - 432.5, compared to patients with other diseases - 337.2, and healthy individuals - 203.3. Among patients with peptic ulcer the highest percentage of people with N.I. > 300 was noted (59.0%), compared to patients with other diseases (42.9%) and those who were healthy (22. 6%). The differences observed between patients with peptic ulcer and those of the remaining groups were highly statistically significant (p<0.001). The percentage of people with the lowest value of the nicotinic index (N.I.<100) in individual groups was: in patients with peptic ulcer - 13.5% (the lowest), among patients with other diseases - 25.0%, in the group of healthy individuals - 38.5% (the highest). An increase was noted in the incidence of peptic ulcer with the value of the nicotinic index. Peptic ulcer occurred in 3.8% of patients with N.I. < 100, in 6.4% of those with N.I. = 100-300, and in 13.2% of patients with N.I. > 300. An increase in the percentage of patients with the nicotinic index was observed irrespective of the site of ulcer. It became most evident among patients who underwent surgical treatment due to peptic ulcer, where the highest value of the nicotinic index (N.I. > 300) was noted in 79.5%, in patients with gastric and duodenal ulcer - 66.7% and those with gastric ulcer - 59.6%. A positive correlation was observed between peptic ulcer incidence rates, complications of the disease and the value of the nicotinic index. The relationship between state of health and the value of the nicotinic index was confirmed. The results of the study showed that the nicotinic index was useful for determining the negative effect of cigarette smoking on the human body.

[System of health care of Lvov students in period between the two World Wars (in vies of "health care" reports)].

Lvov of the period between the two World Wars was an active academic centre concentrating approx. 20% of Polish students. In this area there was started a very interesting and innovatory initiative to create academic system of health care. In the paper there is presented the activity of the organization created that time under name "Health Care", the aim of which was providing the students of all universities in Lvov with comprehensive care. The system of its working improved between 1925 and 1932 became a basis to create similar undertakings in other academic centres. The initiator of these acts far the benefit of students was Professor Henryk Halban.

[A case of secondary amyloidosis in Hodgkin's disease].

A case of hepatic and intestinal secondary amyloidosis in 43 years old patient with Hodgkin's disease is presented. Diagnostic difficulties due to atypical localization of Hodgkin's disease were discussed. Therapeutic problems were caused by severe hepatic insufficiency.

[Peptic ulcer among workers in the engineering and chemical industries].

Incidence of peptic ulcer among workers employed in the engineering and chemical industries is presented in view of selected environmental factors. The study covered 157 workers with peptic ulcer who reported themselves to outpatient clinics at the Nitrogenous Fertilizers Plant in Pulawy and the Automobile Works in Lublin (5000 and 3252 employees, respectively). The study was conducted over a period of one year. It included: medical examinations, supplementary tests and a questionnaire survey. The questionnaire asked, among others, about working conditions, type of occupation performed and lifestyle. Peptic ulcer was diagnosed in 2.03% of workers engaged in the engineering industry and 1.82% of those working at the chemical plant. The incidence of peptic ulcer was higher among manual workers (2.18%) than in office workers (1.16%). The patients had been exposed to the following environmental factors: excessive noise--21%, vibration--17.2%, commuting--11.5%, extra work--14.5%, conflicts at work--13.5%, conflicts at home--15.3%, irregular meals--21.6%, alcohol intake--22.4%, cigarette smoking--62.4%. The study indicates that peptic ulcer is more often diagnosed in manual than in office workers and its incidence can be associated with working conditions and lifestyle. However, no significant variation in annual morbidity caused by peptic ulcer was observed among workers of the plants under study.

[Gastrointestinal parasitosis in patients treated in the Gastroenterology Clinic of the Medical Academy and Clinical Ward at the Institute of Agricultural Medicine in Lublin in the years 1981-1990].

Gastroenteric parasites were found in 118 patients, which made up 1.1% of the total number of patients. This number included 62 men and 50 women aged 17-74, 41 on average. The most frequent parasite was Giardia intestinalis. It was found in 82 patients, which constituted 0.78% of the total number of patients and 69.5% of patients infected by parasites. Trichuris trichiura was diagnosed in 16 patients, which made up 0.150% of the total number of patients and 13.4% of the cases of parasitoses. Ascariasis and oxyuriasis were observed in 8 and 7 patients, respectively. The most rarely found parasites were Taeniarhynchus saginatus (3 patients) and Strongyloides stercoralis (2). Parasitic diseases were most often concomitant with: cholecystitis (23 patients), and duodenal ulcer (15). The results of biochemical tests most frequently showed abnormal values of haemoglobin (31.3% of all parasitoses), elevated lipase values (28.8%), eosinophilia (22.2%). Hypoacidity was observed in 48.3% of cases and the positive bile culture results in 28.8%.

[Activity of enkephalinase (EC.3.4.24.11). Neutral endopeptidase (NEP)in human digestive tract epithelial cells].

The activity was studied of enkephalinase (endopeptidase 24.11, neutral endopeptidase)--a membrane enzyme of epithelial cells within human digestive tract (in the stomach, duodenum, small intestine, ascending, descending and sigmoid colon, and rectum). The enzyme activity was determined by column method using the labelled substrate (H-D-AlaLeu)--enkephalin and the selective enkephalin inhibitor--thiorfan in the presence of bestatin and captopril--inhibitors of aminopeptidases and angiotensin converting enzyme respectively. The highest enkephalinase activity was found in the duodenal epithelium (77.3 8.0 fmol of the substrate/min/mg of protein) and in the small intestine (23.4 0.9 fmol/min/mg of protein) with its gradual decrease when progressing down the gastrointestinal tract. This result confirms the hypothesis of enkephalinase participation in protein hydrolysis processes.
