Revitalizing a documentation system.

The nursing department of a 154-bed acute rehabilitation facility, cognizant of the changing trends in health care and responding to feedback from staff, developed and implemented a comprehensive documentation system. The previous system had been fragmented, inconsistent, and inefficient. The development of the new system focused on the complex needs of the rehabilitation client and the equally complex standards required by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the Commission on Accreditation of Rehabilitation Facilities (CARF), and insurance carriers. The final product, which was based on the nursing process and functional health patterns, encompassed the following areas from admission to discharge: providing feedback on clients' functional abilities and progress toward goals, satisfying requirements of the 1990 JCAHO standards, and, finally, using a flow sheet that saves nursing time and increases objectivity. This article describes the system from conceptualization to successful implementation.

University students' knowledge of STDs: labels, symptoms and transmission.

A sample of 630 Victorian university students, aged between 17 and 24, completed a questionnaire on knowledge of sexually transmitted diseases (STDs). Overall students demonstrated moderate levels of knowledge about the names of diseases which are transmitted by sexual contact, symptoms and medical facts and transmission modes of common STDs. However, they had more knowledge about the labels than they did about symptoms and transmission modes and were misinformed about certain clinical aspects of STDs. It was found that the different levels of knowledge varied with the type of STD. Knowledge about symptoms of transmission did not correlate with a disease's incidence or clinical aspects. Sexual experience and having an Australian born mother correlated with higher scores. This study suggests that young people do not have high levels of knowledge about common STDs they may encounter. Identification of a label is of limited personal value if there is no concurrent knowledge about disease transmission and prognosis.

Political implications of the named-nurse concept.

This paper considers the role of the named nurse as a linchpin in the commissioning/providing cycle. It argues that, whatever the impact on patient care, nurses should be wary of claims that the introduction of named nursing through government policy demonstrates esteem for British nursing.

Omeprazole: a comprehensive review.

Omeprazole is a member of a new class of substituted benzimidazoles. These agents inhibit the proton pump in the gastric parietal cell, blocking the final step in the gastric acid secretory pathway. Omeprazole has been investigated for the treatment of gastric ulcer, duodenal ulcer, gastroesophageal reflux, and various hypersecretory states. The prolonged inhibition of gastric acid secretion allows for once-daily dosing in patients with peptic ulcer disease and gastroesophageal reflux, and once- or twice-daily dosing in patients with Zollinger-Ellison syndrome. Compared with currently available therapies, omeprazole is well tolerated and demonstrates a more rapid ulcer healing rate. It is superior to conventional therapies in the treatment of Zollinger-Ellison syndrome. Side effects are infrequent when the drug is used for the short-term management of ulcers.
