Sympathetically induced paradoxical increases of the cutaneous blood flow in chronically inflamed rats.

In adjuvant arthritic (AA) rats, an abnormal responsiveness of nociceptors (C-fibre polymodal receptors) to sympathetic activities, i.e., alpha 2-adrenoceptor mediated activation of C-fibre polymodal receptors (CPRs), has been observed. The present investigations were undertaken to determine if a similar plastic change would occur in the cutaneous vascular system in the rat chronic inflammation model. The vascular responses were measured by a laser-Doppler flowmeter in the hindpaw skin of the AA rats after electrical stimulation of lumbar sympathetic trunk (sympathetic stimulation). In control non-arthritic rats, the sympathetic stimulation caused decrease in blood flow of the skin (SkBF) in all animals tested (n = 7). On the other hand, the sympathetic stimulation in the AA rats caused both increase (n = 15) as well as decrease (n = 11) in SkBF. In contrast to the abnormal responsiveness of CPRs, the intra-arterial injection of noradrenaline caused the expected decrease in SkBF in all animals tested, and in no instances increases in SkBF were observed. To determine whether activation of nitric oxide (NO), which is known to be a potent endogenous vasodilatation substance, was involved in the vasodilating effect to sympathetic stimulation, an inhibitor of NO synthase, NG-monomethyl-L-arginine (L-NMMA), was applied systemically. L-NMMA significantly increased baseline blood pressure in the control and the AA rats, but it did not significantly alter the SkBF in the control or the AA rats after the sympathetic stimulation, suggesting that NO is not a mediator in the vasoactive responses. The results of the current studies showed for the first time that electrical stimulation of the lumbar sympathetic trunk causes vasodilatation in the skin of the AA rats. This abnormal responsiveness of regional SkBF after sympathetic stimulation was not mediated by adrenergic or NO system.

Segmental distribution of afferent neurons innervating the canine testis.

To clarify the afferent innervation of the canine scrotal contents, retrograde labeling of neurons in the dorsal root ganglia (DRG) has been carried out using two methods: (1) horseradish peroxidase (HRP) injection into the surface of the testis and epididymis; and (2) exposure of the superior spermatic nerve to a fluorescent dye (Fast blue; FB). Injections of HRP resulted in labeling of DRG cells located predominantly from T10 to L4 (87%) and, to a lesser extent, at S1-S3 (13%). Transection of the vas deferens previous to testicular injections eliminated labeling in the S1-S3 DRG, but not at thoracolumbar levels. These findings indicated that primary afferent fibers of the testis and epididymis project mainly to the DRG at higher than L4 through the superior spermatic nerve, but an additional population of the fibers also projects the sacral level through the inferior spermatic nerve. Exposure of the superior spermatic nerve to FB resulted in a similar distribution of labeled cells as compared with testicular injections of HRP after vasectomy. Labeled cells (8.1%) were also observed in the contralateral T13-L3 DRG. In both FB and HRP groups, the major part of the labeled cells was located in L1 and L2. The sizes of HRP- and FB-labeled cells were smaller than those of unlabeled cells in the L1 and L2 DRG. The cumulative frequency distribution histogram for the diameter of HRP- and FB-labeled cells could be fitted by a normal distribution.

Abnormal modulation of cholinergic neurotransmission by opioid in hyperresponsive bronchus of rats.

1. The electrical field stimulation (EFS)-induced bronchoconstriction in vitro in rats challenged by DNP-Ascaris antigen was significantly greater than that in normal rats. 2. Morphine inhibited the EFS-induced bronchoconstriction in normal rats. Whereas the inhibition of EFS-induced bronchoconstriction by the opioid was little, if any, in the DNP-Ascaris-challenged rats. 3. These findings suggest that dysfunction of presynaptic inhibitory modulation through the opioid receptor may take place in the airways of DNP-Ascaris-challenged rats.

Effects of induced metabolic alkalosis on perception of dyspnea during flow-resistive loading.

Treatment of dyspnea in patients with advanced cancer is an important issue. The purpose of the present study was to assess the effect of induced-metabolic alkalosis produced by administration of sodium bicarbonate on dyspneic sensation. In seven healthy subjects, dyspnea was induced by having them breathe with a flow-resistive load (24 cm H2O/L/sec) for 6 min before and after administration of sodium bicarbonate (0.5 mmol/kg, per os+2 mmol/kg, IV). The intensity of dyspnea was rated using a visual analogue scale (VAS). The VAS scores and minute ventilation during loaded breathing after administration of sodium bicarbonate were significantly lower than those before administration of sodium bicarbonate. These results indicate that induced metabolic alkalosis may alleviate the intensity of dyspneic sensation by a reduction in ventilatory drive.

Effect of C-peptide administration on whole body glucose utilization in STZ-induced diabetic rats.

Recent studies suggest that C-peptide stimulates glucose transport in isolated skeletal muscle. In order to determine the effect of C-peptide on whole body glucose utilization, streptozotocin (60 mg kg-1) (STZ)-induced diabetic and normal rats were studied using the euglycaemic clamp procedure and continuous infusion of somatostatin (1.0 micrograms kg-1 min-1) in pentobarbital-anaesthetized rats. Plasma insulin levels during the 6.0- and 30.0-mU kg-1 min-1 insulin infusions rose to 70-90 microU mL-1 and 500-700 microU mL-1, respectively. Blood glucose concentrations were clamped at 7.5-7.9 mmol L-1 in the diabetic rats and at basal levels or 7.7 mmol L-1 in the non-diabetic (normal) rats. Biosynthetic human C-peptide (0.5 nmol kg-1 min-1) was infused in 12 diabetic and 11 normal rats, resulting in concentrations of 26-41 nmol L-1. The metabolic clearance rate of glucose (MCR) for the diabetic rats receiving C-peptide (12.0 +/- 1.0 mL kg-1 min-1) was significantly (P < 0.01) higher than that in the diabetic rats given saline (6.3 +/- 0.7 mL kg-1 min-1) or a randomly scrambled C-peptide (7.8 +/- 1.3 mL kg-1 min-1) at low-dose insulin infusion but not at the high-dose insulin infusion. In normal rats C-peptide did not significantly increase the MCR for glucose. These results thus demonstrate that C-peptide has the capacity to increase glucose utilization in STZ-induced diabetic rats.

ACTH independent Cushing's syndrome occurring in siblings.

Familial Cushing's syndrome due to ACTH independent bilateral macronodular adrenocortical hyperplasia occurring in siblings is reported. The proband was a 69-year-old woman who presented with a typical Cushingoid appearance. The serum cortisol level was elevated, with a loss of diurnal rhythm, and the plasma ACTH level was undetectable. Dynamic testing showed no suppression of urinary 17-OHCS by high dose dexamethasone and no stimulation by metyrapone. An abdominal CT scan showed bilateral adrenal enlargement. The patient died of a subarachnoid haemorrhage, and autopsy revealed a massively thickened adrenal cortex composed of nodules up to 3.5 cm in diameter. A pituitary adenoma was not found. We learned that the patient's elder brother was also diagnosed at 59 years of age with Cushing's syndrome due to bilateral macronodular adrenocortical hyperplasia. His plasma cortisol levels were not suppressed by high dose dexamethasone and the plasma ACTH level was undetectable. Screening of the available family members by administering 1 mg dexamethasone at midnight and performing abdominal CT scan revealed impaired suppressibility of serum cortisol associated with enlarged bilateral adrenal glands in a 64-year-old sister and a 54-year-old brother. The 64-year-old sister was considered as a possible 'affected' case in the early stages of development, because the basal level of ACTH was not suppressed and hyperplasia of the bilateral adrenal glands as revealed by CT scan was less evident.

Impaired endothelium-dependent relaxation in rabbit pulmonary artery after subarachnoid hemorrhage.

We investigated the changes in endothelial function of pulmonary as well as basilar artery after experimental subarachnoid hemorrhage (SAH) induced by cisternal blood injection in rabbits. The animals were killed on day 2 and day 7, and the mechanical responses were determined on transverse strips of the main pulmonary artery and rings harvested from the basilar artery. To examine the role of the vagal nerve, we cut the left vagal nerve immediately before injecting cisternal blood. Relaxation response of pulmonary and basilar arteries to acetylcholine (ACh) and A23187 were abolished after endothelial removal and reality inhibited by either NG-nitro-L-arginine (NOARG) or methylene blue (MB). Indomethacin failed to modify these relaxation responses. Relaxation response of the main pulmonary artery strips to ACh was significantly (p < 0.05 and p < 0.01) attenuated on day 2 in 6 of 12 rabbits. In the remaining 6 rabbits, relaxation was not affected. No change in relaxation responses to A23187 and sodium nitroprusside (SNP) was observed in any of the 12 cases. In the vagotomized rabbits no decreased response to ACh was observed. On day 7, relaxation response to ACh returned to normal. ACh also produced decreased relaxation in basilar artery rings on day 2 with no change in A23187- and SNP-induced relaxation. Contractile responses of pulmonary and basilar arteries to norepinephrine (NE), endothelin-1 (ET-1), 5-hydroxytryptamine (5-HT), and U46619 were not affected after SAH. These results suggest that less functional muscarinic receptors that produce/release less endothelium-derived relaxation factor [EDRF/nitric oxide (NO)] are involved in causing the reduced relaxation of pulmonary and basilar arteries after SAH and the vagal nerve may play a role in regulating the receptor-mediated, endothelium-dependent relaxation in the main pulmonary artery after experimental SAH.

Intimal hyperplasia in human uterine arteries accompanied by impaired synergism between prostaglandin I2 and nitric oxide.

1. The present experiments were designed to investigate the mechanisms causing intimal hyperplasia in connection with the impaired synergism between prostaglandin I2 (PGI2) and nitric oxide (NO) in human uterine arteries (UAs). 2. In order to assess the magnitude of intimal hyperplasia, the intima:media ratio (%) was estimated with the aid of an image analyser. Human UAs were classified into two groups, I and II on the basis of the ratio and the degree of elastin deposition of histologically normal specimens. The intima:media ratio in group II was determined to be 38.9 +/- 7.7% (n = 6), which was significantly (P < 0.01) higher than that in group I (16.5 +/- 1.5%, n = 7). Less deposition of elastin was found in group I than in group II. 3. The relaxation activities of iloprost (IP) as a stable analogue of PGI2 and sodium nitroprusside (SNP) as a NO donor were not different between the two groups. When the minimum concentrations (Cmin) of IP and SNP in producing relaxation were applied together to the UA strips, these compounds interacted synergistically in group I. The observed relaxation (48.7 +/- 8.8%, n = 7) in this group was significantly (P < 0.01) greater than the predicted value of 18.8 +/- 3.1% (n = 7) (the mathematical sum of the relaxations caused by IP and SNP alone). By contrast, these agents interacted in an additive manner in group II. The observed relaxation (20.8 +/- 9.5%, n = 6) was not significantly different from the predicted value (18.6 +/- 2.4%, n = 6) in this group. 4. During the relaxation produced by the addition of IP and SNP alone or in combination, the changes in cyclic nucleotides (cyclic AMP and cyclic GMP) contents (pmol mg-1 protein) were assayed. When IP and SNP at Cmin were applied together to the UA strips, these compounds interacted synergistically in increasing cyclic nucleotides in group I. The observed net increase in the content was determined to be 1.46 +/- 0.30 (P < 0.05 vs. the predicted value of 0.67 +/- 0.12) in this group (n = 7). By contrast, the observed net increase (0.40 +/- 0.07, n = 6) did not exceed the predicted value (0.65 +/- 0.07, n = 6) in group II. 5. These results suggest that the formation of intimal hyperplasia in group II may be closely related to the impaired synergism between PGI2 and NO in the human UAs.

Exercise training prevents maturation-induced decrease in insulin sensitivity.

We examined the effects of exercise training initiated before maturation or after maturation on insulin sensitivity and glucose transporter GLUT-4 content in membrane fractions of skeletal muscle. Female Wistar rats (4 wk of age) were divided into sedentary and exercise-trained groups. At 12 wk of age, a subset of the trained animals (Tr) was killed along with a subset of sedentary controls (Sed). One-half of the remaining sedentary animals remained sedentary (Sed-Sed) while the other half began exercise training (Sed-Tr). The remaining rats in the original trained group continued to train (Tr-Tr). Euglycemic clamp (insulin infusion rate at 6 mU.kg body wt-1. min-1) was performed at 4, 12, and 27 wk. After euglycemic clamp in all animals except the 4-wk-old, hindlimb (gastrocnemius and part of quadriceps) muscles were removed for preparation of membrane fractions. In sedentary rats, glucose infusion rate (GIR) during euglycemic clamp was decreased from 15.9 mg.kg-1.min-1 at 4 wk of age to 9.8 mg.kg-1.min-1 at 12 wk of age and 9.1 mg.kg-1.min-1 at 27 wk of age. In exercise-trained rats, the GIR was not significantly decreased by maturation (at 12 wk) and further aging (at 27 wk). Initiation of exercise after maturation restored the GIR at 27 wk of age to the same levels as these for the corresponding exercise-trained rats. GLUT-4 content in plasma and intracellular membrane fractions of hindlimb muscle obtained just after euglycemic clamp showed the same trend as the results of GIR. These results suggest that exercise training prevented the maturation-induced decrease in insulin sensitivity. Improvement of insulin sensitivity caused by exercise training was attributed, at least in part, to the increase in insulin-sensitive GLUT-4 on the plasma membrane in skeletal muscle.

Changes in urinary nitrate and nitrite during treatment of ulcerative colitis.

The urinary excretion rates of nitrate (NO3) and nitrite (NO2) were monitored in 14 patients with active ulcerative colitis during treatment using hydrocortisone and sulfasalazine. During the active phase of the disease, the NO3 excretion was significantly higher in the patients than in healthy controls (n = 6, p < 0.05), although it varied considerably among the patients. During the healing phase, the NO3 excretion decreased concurrently with improvement of symptoms and colorectal ulceration, but the NO2 excretion increased. During the inactive phase of the disease, the NO3 and NO2 excretions were significantly lower than during the active phase, and the NO2/NO3 ratio resembled that in the healthy controls. In contrast, a patient who failed to respond to treatment showed continuously high NO3 and NO2 excretion rates. These results indicate that urinary NO3 and NO2 excretions vary with the disease state in ulcerative colitis.

Effects of oxygen and transition metals on the advanced Maillard reaction of proteins with glucose.

The generation of fluorescence and 3-deoxyglucosone (3DG), browning, polymerization, and impairment of the amino acid residues of lysozyme incubated with glucose were investigated at 37 degrees C and 50 degrees C at pH 7.4 in a phosphate or TAPSO buffer under aerobic and non-aerobic conditions with or without DETAPAC as a chelating reagent. Browning, the generation of fluorescence, and polymerization were accelerated under the non-aerobic, compared to aerobic, conditions. Moreover, the formation of 3DG was also significantly increased under non-aerobic conditions. The incubation of both reaction systems resulted in noticeable losses of arginine and lysine residues. DETAPAC significantly inhibited the advanced Maillard reaction under both aerobic and non-aerobic conditions. However, DETAPAC had no effect on the impairment of lysine and arginine residues. The generation of fluorescence, browning and polymerization of lysozyme in the TAPSO buffer were markedly inhibited under both aerobic and non-aerobic conditions. These observations suggest that transition metals in the phosphate buffer may have accelerated the formation of Amadori compounds via Schiff's base. In addition, under non-aerobic conditions, the formation of advanced glycation end products from 3DG via Amadori compounds is presumed to be the major pathway, because the formation of N epsilon-(carboxymethyl)lysine, glyoxal, and glucosone was accelerated by an oxidative reaction catalyzed with transition metal ions. These presumptions are supported by the results from a lysozyme-3DG reaction system.

Experimental studies on bovine Hyena disease induced by administration of excessive vitamin AD3E premix, vitamin A, or vitamin D3.

The effects of the excessive administration of the vitamin AD3E(V-AD3E) premix, vitamin A (V-A) or vitamin D3 (V-D3) on experimental development of Hyena disease in the calves were examined. Hyena disease was recognized in 4 calves, both of the 2 calves administered a high dose of V-AD3E premix (V-A 3,000,000, V-D3 300,000, and V-E 1,200 IU/day, V-AD3E group), 1 of the 2 calves administered a half dose of the V-AD3E premix, and 1 of the 2 calves administered only V-A (V-A 3,000,000 IU/day, V-A group) when each vitamin was administered orally for 10 days from 1 week after birth. Both of 2 calves administered only V-D3 (V-D3 300,000 IU/day) did not developed. In the 4 calves with Hyena disease (Hyena calves), the plasma retinylpalmitate showed high values which was suggesting the hypervitaminosis A, and the epiphysial growth plate was narrow and destroyed structure of column. Compared with the Hyena calf in the V-A group, the Hyena calves in the V-AD3E group showed earlier appearance time of Hyena disease, lower growth rate and shorter lengths of fore and hind limb bones. In conclusion, the present findings suggested that excessive V-A administration to suckling calves might cause Hyena disease by V-A effects to the epiphysial growth plate, moreover such effects may be promoted by the V-D3.

Correlation analysis between bladder cancer and cigarette smoking in various countries.

Cigarette smoking is considered to be a risk factor of bladder cancer. We investigated the relationship between bladder cancer mortality rates and cigarette smoking prevalence in various countries, and examined whether only the difference of smoking prevalence could explain the difference of mortality rates of bladder cancer in each country or not. The age-adjusted mortality rates from bladder cancer were high in European countries and low in Asian countries. The sex ratios ranged between 2 to 6 in most of the countries. For 20 countries, the mortality rate from bladder cancer was not correlated with the cigarette smoking prevalence. However, when the analysis was restricted to 16 countries in Europe, North America and Oceania (Caucasians), the correlation coefficients between them were high, being 0.68 (p < 0.01) in males and 0.49 (0.05 < p < 0.1) in females. In both Japan and the United States, cigarette smoking prevalence declined while the incidence of bladder cancer increased in males, both being stable in females. Race might be considered to be a strong risk factor of bladder cancer, followed by the prevalence of cigarette smoking. It is necessary to follow the changes of incidence rates of bladder cancer in the future to evaluate the influence of cigarette smoking.

[The mass screening for prostate cancer and the age eligibility. Gunma Urological Oncology Study Group].

BACKGROUND: The age eligibility for the mass screening (MS) for prostate cancer was investigated. METHODS AND MATERIALS: This study was performed in subjects examined by MS from 1981 to 1994 in Gunma Prefecture, Japan. MS was performed by digital rectal examination (DRE) and prostatic acid phosphatase from 1981 to 1991, and by DRE, transrectal ultrasonography and prostate specific antigen from 1992 to 1994. RESULTS: It was demonstrated that the prostate cancer detection rate was affected by the ratio of subjects newly and previously examined, the age distribution in the group examined and the diagnostic modality. It was also demonstrated that the detection rate of early stage prostate cancer was also affected by the age distribution and the diagnostic modality. The effective detection rate was determined on the assumption that the purpose of MS is the detection of the subjects who have the indication for the radical prostatectomy, and the effective detection rate was calculated in the range from 50 to 69 years for age eligibility, and B and C for clinical stage. It was estimated that effective detection rate was 0.37 to 0.61%. They were higher than the detection rate of MS for other organ cancers. CONCLUSION: The eligibility is 50 to 69 years. If subjects in the range of 70 to 74 are included in this project, it is necessary to make an effort practice a subject-education about the prostate cancer natural history and the radical prostatectomy indication.

[Regional and chronological differences of urogenital cancer deaths in Japan].

PURPOSE: We investigated the changing patterns of urogenital cancer deaths during the past 22 years in Japan. METHODS: We analyzed patients that died from cancer of the prostate, bladder and kidney between 1973-1994. Age-adjusted death rates (adjusted to the world population), standardized mortality ratios (SMR) according to each prefectures and age-specific death rates for each types of cancer were calculated and changes in these patterns were analyzed. RESULTS: Age-adjusted death rates for cancer of the prostate increased from 2.29 in 1973 to 4.36 in 1994, a 1.9-fold increase. Death rates for cancer of the bladder were stable in males and declined in females. Death rates for cancer of the kidney (15 years or older) increased from 1.45 in 1973 to 2.72 in 1994, a 1.9-fold increase in males, and tended to increase in females as well. In the SMR analysis by prefecture, distribution in 1973-84 (former period) and that in 1985-94 (later period) were similar, and characteristic features were observed for each type of cancer. Age-specific death rates for cancer of the prostate and bladder rose by an index power of age, but that for cancer of the kidney reached a plateau or decreased after an index power increase to a certain age. The rate of increase in age-specific death rates (later period/former period) rose according to age, especially 70 years or older, in cancer of the prostate and kidney in both males and females. However, age-specific death rates in the later period from cancer of the bladder was higher only in patients 85 years or older, but was lower in other age groups. CONCLUSIONS: Death rates for cancer of the prostate and kidney (15 years or older) tended to increase, while that of the bladder remained stable or decreased. It is expected that detection and treatment of these disease can be improved by utilizing these epidemiologic information.

[Pharmacokinetics of 99mTc-MAG3 and 131I-OIH: comparative study based on 2 compartment model analysis].

We studied 50 patients with mild to moderate renal dysfunction to compare pharmacokinetics of 99mTc-MAG3 with that of 131I-OIH. After simultaneous bolus injection of both 99mTc-MAG3 and 131I-OIH, 8-point venous blood sampling was performed from 2 to 44 min post injection. Aliquoted plasma samples were counted for radioactivity along with the injected standard to obtain % injected dose/ml plasma for each tracer. Using obtained time-concentration data, classical 2 compartment model analysis was performed for both tracers to obtain various pharmacokinetic parameters, including distribution volumes (Vds), inter-compartmental rate constants, and plasma clearance. In these parameters, Vd of central compartment, Vd at steady state, central to peripheral inter-compartmental rate constant, and plasma clearance were significantly larger for 131I-OIH. In all parameters, significant correlation was found between 99mTc-MAG3 and 131I-OIH. The best correlation was seen in plasma clearance (r = 0.891, p < 0.0001). Plasma clearance ratio (99mTc-MAG3/131I-OIH), however, showed weak but significant negative correlation with serum creatinine, although this correlation was not likely to affect the overall correlation of clearance between 131I-OIH and 99mTc-MAG3. From these results, we confirmed that 99mTc-MAG3 clearance could be used as an alternative to 131I-OIH clearance, although pharmacokinetic behavior of 99mTc-MAG3 was not exactly the same as that of 131I-OIH.
