[Fungemia by Candida non albicans in patients with HIV infection].

OBJECTIVE: To analyse candidemia by Candida non albicans in adult HIV-infected patients from 1989 to August 1998. PATIENTS AND METHODS: We retrospectively reviewed all HIV patients who presented at least one positive peripheral blood culture for Candida spp. Clinical, epidemiologic, immunologic and evolutive characteristics, and risk factors for candidemia were analysed. RESULTS: During this period of time, 2,025 HIV-infected patients were diagnosed. Five of them developed candidemia by C. albicans and 6 by C. non albicans. These last ones were all men with mean age of 32.1 years old, and intravenous drug use was the risk factor for HIV in 5 of them; aids had previously been diagnosed in 4 patients and 3 had receipt imidazoles. None was neutropenic. Three cases were nosocomially acquired. Median CD4 cell count was 108.6 x 10(6)/l (3-366). Candida krusei was isolated in two intravenous drug users, Candida parapsilosis in two other patients and Candida glabrata and Candida tropicalis in one of the other two cases each. We treated 2 patients with amphotericin B and 2 with fluconazole. One patient died. CONCLUSIONS: Candidemia is not frequent in HIV-infected patients. Candidemia by C. non albicans in HIV-infected patients presents similar characteristics as in other immunodepressed patients and it is fundamentally associated with previous use of imidazoles, nosocomial acquisition and late-stage aids. It is noteworthy that C. krusei was isolated in two intravenous drug users.

[Comparative study of propofol with thiopental and etomidate in anesthetic induction].

Anesthetic characteristics were studied in three homogeneous groups of twenty patients ASA I who underwent intravenous anesthetic induction with propofol 2 mg/kg; thiopental 5 mg/kg; or etomidate 0.3 mg/kg. The unconsciousness time was similar in the three groups, whereas awaking time and time of response and orientation were longer after thiopental and etomidate than after propofol. Intravenous injection of the three anesthetic agents was followed by a decrease in systolic and diastolic arterial pressure. Heart rate increased after thiopental and etomidate and had only slight fluctuations after propofol. After tracheal intubation there was a significant increase in systolic and diastolic arterial pressure and heart rate in thiopental and etomidate group. These changes were minimal after propofol. The highest number of complications occurred after etomidate.

Nutritional evaluation and dietetic care in cancer patients treated with radiotherapy: prospective study.

Ninety-three cancer patients about to undergo radiotherapy were studied. These patients were affected by cancer of the head and neck, breast cancer, and abdominopelvic cancer. They were randomly distributed into control and experimental groups. The patients from the control group were allowed to eat ad libitum, whereas those from the experimental group followed a strict nutritional protocol that covered their needs while satisfying their particular tastes and habits. Data were collected the day before radiotherapy commenced and the day treatment ended. Anthropometric, analytical, and clinical parameters were determined. The results showed the benefit of dietetic treatment in experimental patients with head and neck or abdominopelvic cancer.

[Hypoglossal nerve paralysis after intubation and direct laryngoscopy].

We described an extremely uncommon case of hypoglossal nerve paralysis occurred after a direct laryngoscopy under general anesthesia with McIntosh's laryngoscope intubation. We reviewed the literature on similar cases and studied the etiology and pathogenesis of these paralyses.

[Bacillary angiomatosis: report of 2 cases].

BACKGROUND: Epithelioid bacillary angiomatosis (EBA) was studied as an infectious disease associated to immunosuppressive states, establishing the bases for performing differential diagnosis with other pathologic processes. METHODS: Two new cases of EBA in patients with human immunodeficiency virus infection (HIV) are presented. Diagnosis was performed by anatomopathologic study of the cutaneous lesions which had undergone biopsy. RESULTS: In one of the cases bacillary structures were observed under electron microscopy. This patient also presented Kaposi sarcoma (KS) with histologic study being therefore necessary to perform differential diagnosis between these pathologic processes. Both patients presented good response to treatment with erythromycin. CONCLUSIONS: 1) EBA is an infectious disease of good prognosis with antibiotic treatment which fundamentally affects severely immunosuppressed patients with human immunodeficiency virus infection. 2) Biopsy is the only differential diagnostic method for this disease with other processes with similar clinical appearance and different prognosis as in Kaposi sarcoma which may even coexist in these patients.

AIDS-Related Opportunistic Infections↗

The association between cancers of the small and large bowel.

Malignant tumors of the small bowel are rare and little is known about their etiology, although adenocarcinomas share certain epidemiological features with colorectal cancer. This study investigated what cancers, if any, occurred as second neoplasms following adenocarcinomas, malignant carcinoid tumors, lymphomas, and sarcomas of the small bowel. For all 2581 cases of small bowel malignancy registered in one of the Surveillance, Epidemiology, and End-Results program areas, 1973-1988, the relative risk of a second malignancy was determined. The risk of colorectal cancer was increased following adenocarcinoma of the small bowel, and the risk of adenocarcinoma of the small bowel was increased following colorectal cancer in both males and females. This study also found an association between small bowel sarcomas and malignant melanoma in males, consistent with earlier studies, and an association between prostate cancer and malignant carcinoid tumors of the small bowel, a new observation. We conclude that adenocarcinomas of the small bowel may share risk factors with colorectal cancer.
