[Carcinoma in situ of the breast: 10 cases].

The authors have reviewed 10 cases of carcinoma in situ of the breast, which they were able to isolate from 180 invasive cancers of the breast that were operated on during the same period of time. (These rare lesions occasionally occur at the same time as an invasive cancer; then they increase the risk of local recurrences). When they occur by themselves in situ cancers of the breast tend to be duct cancers or lobular cancers. Duct lesions are more common. They do, in a significant proportion of cases, become invasive. The authors point out the value of more mammographies being taken to diagnose these duct carcinomas in the breast in order to work out the correct treatment. They think it is important to delay making a decision as to the surgical procedure to be adopted until the report on the histology of the specimen taken for biopsy has been received. For a long time total mastectomy was the only treatment: the results were too good... (some patients were overtreated). The present tendency is to limit the operation (to segmental excision) in cases where the lesion is limited. Mastectomy is reserved for cases that are spread out. Carrying out irradiation after widespread segmental excision increases the risks of local recurrences. Clearing the whole area has only to be considered when there are widespread forms of the condition. Lobular cancers in situ are marker lesions and have to be followed up very carefully indeed. The authors illustrate their article with some anatomopathological specimens and mammographies that they have come across.

[Management of a pregnant woman with Streptococcus group B].

The relative rarity (1 to 5 cases for 1,000 births) of neonatal infections secondary to B Streptococcus, the epidemiological characteristics of this germ, especially the unstable vaginal carriage, make it difficult to select a therapeutic approach. Systematic screening of B Streptococcus and the treatment of all carriers or only of high-risk patients, present several practical problems, are complex to implement but the cost/benefit ratio seems however acceptable. Prophylactic intrapartum antibiotic treatment of known carriers of B Streptococcus does not seem debatable any longer, at least the treatment of those presenting other risk factors: premature delivery, premature rupture of the membranes, fever occurring during delivery. Today, the best prophylaxis of neonatal infections seems to be the intrapartum antibiotic treatment (ampicillin) resulting in a spectacular decrease of the frequency of neonatal contamination.

[Mechanism of duplication formation relating to a case of 10q22 to q25 duplication].

A duplication 10q22q25 was studied in the fetus of a mother carrier of a t(14q21q). On this occasion, duplications reported in the literature are reviewed. Chromosomal rearrangements involving two breaks always result in tandem duplications, while three break- rearrangements result in tandem or mirror adjacent duplications, or in non adjacent direct or inverted duplications, or in direct or inverted autointerstitial duplications.

Tubal sterilization by a combined laparoscopic and external technique: preliminary report on 75 cases.

A preliminary report is given on a new laparoscopic sterilization procedure. By a double-puncture method, a loop of tube is brought out of the abdomen through the suprapubic incision. The loop is then resected by a modified Pomeroy method. Between Octorber 1976 and June 1978, 75 sterilizations were performed without any major problem. Sterilization failed in one case when one tube could not be seen. All recoveries were uneventful. Safety, effectiveness, simplicity, and potential reversibility seem to be the qualities of this method.

[Indication and technic of tubal sterilization by vaginal route. Posterior transverse colpotomy].

40 tubal sterilisations have been carried out in 5 years. The indications most often are for grand parity. The anatomical criteria for effective sterilisation by the vaginal route should be sought by gynaecological examination under general anaesthetic. The date for the operation, the technique and its difficulties have been described. The operation lasted on an average 30 minutes. The average stay in hospital was 8.2 days. Only one local complication using this techinque was noted. On one occasion the authors had to abandon the vaginal route to reach the tubes. This simple procedure without any aesthetic inconvenience seems to them to be the ideal route for tubal sterilisation.

[Evaluation of a series of 137 carcinomas of the endometrium of stage I TNM/UICC].

PURPOSE: To assess retrospectively the long-term results of the combination of surgery and radiotherapy in carcinoma classified cT1. PATIENTS AND METHODS: From 1974 to 1993, 137 women suffering from endometrial carcinoma cT1Nx-0 M0 were entered into the study. The median age was 62 years (range: 39-85 years) and the median follow up was 67 months (range: 0-224 months). RESULTS: Surgery was performal in 132 women (96.35%). For cT1, the 5-year overall and specific survivals were 81.1% and 84.5%, respectively. The 10-year overall and specific survivals were 68.8% and 82.2%, respectively. Concerning cT1pT1, the 5-year overall and specific survivals, were 83.9% and 87.4%. The 10-year overall and specific survivals were 71.1% and 85%, respectively. Histological grade, pelvic lymph node involvement and myometrial infiltration influence significantly the overall and specific survivals of cT1pT1 tumors. According to multivariate analysis, pelvic lymph node involvement was a powerful prognostic factor for both the overall and specific survivals. If we rule out pelvic lymph node involvement, WHO histological grade was a significant prognostic factor. CONCLUSION: Combination of surgery and radiotherapy is still a common procedure for cT1 tumors. When surgery is done before radiotherapy, tailored irradiation may further take place, according to WHO histological grade and pelvic lymph node status.

[Biomarkers and uterine cervical cancer. Are they useful for prognosis of cancer and triage of precancerous lesions?].

Biomarkers are "keys" of oncogenesis. Many applications are soon possible as gene therapy and vaccination. Authors have reviewed different aspects of classical prognosis factors and examined new possibilities of prognosis and selection of precancerous lesions as ASCUS by biomarkers. Ag SCCTA4, HPV typing, microangiogenesis, are the main biomarkers in cervical cancers.
