[Enteral nutrition at home: National register for the year 2000].

GOAL: Once again, the NADYA-SENPE Working Group analysed the registered data of Home Enteral Nutrition (HEN) in our country, during the year 2000. MATERIAL AND METHODS: The data were collected through a closed questionnaire included on our web site (www.nadya-senpe.com). Apart from epidemiological information, the form includes the indication to prescribe this treatment, the specific nutritional treatment used and its duration, access path, complications and readmission rate in hospital, follow-up of the treatment, patient's quality of life and progress. All data were processed and analysed by the co-ordinating team. RESULTS: Twenty two hospital participated and 2,986 patients, aged 65.1 +/- 19.7 years, were enrolled. Of these patients, 41.2% were diagnosed with neurological diseases and 33.3% with cancer. The mean time on HEN was 6.3 +/- 4.4 months. Oral nutrition was the preferential route (50.8%), followed by nasoenteral tube (30.5%), and in 17.4% ostomy tubes were placed. Polymeric was the formula composition mainly used (83.2%). Patients were followed (70.1%) by the hospital reference Nutritional Support Unit. The complications related to nutrition included the gastrointestinal (0.25 complications/patient), the mechanical one (0.19 complications/patient), and the metabolic (0.007 complications/patient). Feeding tube need to be replaced 0.3 times/patient/year. The readmission rate, for nutritional problems, was observed in 0.03 patients. At the end of the year, 54.9% of the patients were in the HEN program, and in 30.3% HEN was finish due to different reasons. In 21.9% of the patients no, o light, discapacity degree was found. CONCLUSIONS: Related to previous years, there is an increment in the number of enrolled patients. Neurological diseases and cancer were the more frequent diagnoses in HEN patients. Oral access was the higher feeding route due, probably, to the high prevalence of cancer patients. In spite of the elevated prevalence of neurological diseases, a few number of patients were feed with ostomy tube. Finally, due to the few readmission rate and complications, HEN is a safe treatment in our country.

[Lipomatous hypertrophy of the interauricular septum: diagnosis with transesophageal echocardiography and computerized tomography].

Transesophageal echocardiographic and computed tomographic imaging in a patient with lipomatous hypertrophy of the interatrial septum and advanced left bundle branch block are presented. Transesophageal echocardiography demonstrated a thickening of the atrial septum sparing the fossa ovalis region. Computed tomographic scan at the level of the interatrial septum showed a mass of fat density, and the HIS bundle electrogram showed an advanced infra-his block after intravenous ajmaline. We stress the utility of transesophageal echocardiography in this disorder, and the possible relationship between lipomatous hypertrophy of the interatrial septum and conduction disturbances.

[Pulmonary thromboendarterectomy as a treatment of chronic pulmonary arterial hypertension secondary to unresolved pulmonary thromboembolism. A preliminary report].

This is a preliminary report of our initial experience with thromboendarterectomy in the treatment of four patients with chronic major-vessel thromboembolic pulmonary hypertension. Before surgery, these patients all had severe pulmonary hypertension and cor pulmonale as well as significant abnormalities in lung function (mechanics and gas exchange). They were in class III-IV (NYHA). One of our patients died immediately after the procedure as a result of major bleeding. The remaining three surviving patients have shown a remarkable improvement in both, pulmonary hemodynamics and lung function which has been translated in an also significant improvement in their clinical condition. Although there is still a significant risk involved with this procedure, the thromboendarterectomy represents a reasonable and useful therapeutic approach for selected patients with this deadly form of chronic pulmonary hypertension in which other forms of treatment have proved to be unsuccessful.

[Ferritin in the cerebrospinal fluid as an early indicator of neuromeningeal involvement in patients with malignant lymphoma].

In order to evaluate whether cerebrospinal fluid (CSF) ferritin could be useful to determine early infiltration of central nervous system (CNS) in patients with malignant lymphoma, the ferritin concentration was measured in 30 previously untreated patients with malignant lymphoma without evidence of neurologic infiltration. Six patients showed elevation of CNS ferritin 2 to 6 months after clinical and cytologic diagnosis of CNS involvement was confirmed. Twenty-four patients with normal CSF ferritin did not show involvement of CNS 6 to 23 months after the study was done. Measurement of CNS ferritin appears to be important in the early detection of CNS involvement of malignant lymphoma and should be included in the clinical evaluation to detect patients at high-risk to develop this complication and prophylaxis could be done to avoid it.

Nutritional evaluation and dietetic care in cancer patients treated with radiotherapy: prospective study.

Ninety-three cancer patients about to undergo radiotherapy were studied. These patients were affected by cancer of the head and neck, breast cancer, and abdominopelvic cancer. They were randomly distributed into control and experimental groups. The patients from the control group were allowed to eat ad libitum, whereas those from the experimental group followed a strict nutritional protocol that covered their needs while satisfying their particular tastes and habits. Data were collected the day before radiotherapy commenced and the day treatment ended. Anthropometric, analytical, and clinical parameters were determined. The results showed the benefit of dietetic treatment in experimental patients with head and neck or abdominopelvic cancer.

Peritoneal nutrition in dogs after intestinal resection: comparative study.

A total of 18 adult dogs were kept on peritoneal nutrition (PeN) for 30 days via peritoneal catheter. Eleven dogs were put on peritoneal nutrition immediately, receiving a caloric rate of 45 kcal/kg per day, half of which was supplied by glucose and the other half by 20% intralipid, along with 0.5 g/kg per day of protein, as well as ions, trace elements, and vitamins. Two-thirds of the glucose was administered separately. Seven other animals underwent an 80% resection of the small intestine and were kept on special oral nutrition for 6 weeks. After this time, peritoneal nutrition was applied with the same caloric rate as in the previous group but with a protein dose of 1 g/kg per day and administration of the nutrients every 6 hours. We found two cases of peritonitis in the group without intestinal resection, whereas no infectious outbreaks occurred in the group subjected to intestinal resection. This group showed an exacerbated nutritional state, with a consequently greater loss of weight and albumin and a negative nitrogen balance. The peritoneal membrane showed signs of cellular hyperplasia and hypertrophy following peritoneal nutrition. These phenomena disappeared 1 month after PeN was terminated.

Apicoaortic conduit implanted without cardiopulmonary bypass: a useful contingency technique.

Apicoaortic conduit implantation is easier and safer with cardiopulmonary bypass. We present an analysis of two cases in which the operation was successfully concluded without the bypass. The first patient had a simultaneous correction of aortic coarctation and tunnelliform subaortic stenosis 8 years ago and is asymptomatic at present. The second patient suffered from hyperlipoproteinemia, calcification of the ascending aorta and femoral arteries and severe aortic stenosis. One month after surgery be suffered cardiac arrhythmia and died. Although this experience is limited, we believe that information about this alternative method may be useful in some cases.

[Coronary dissection following coronary radiography].

We present 4 cases of dissection of the coronary arteries during coronary radiography, which required emergency surgery. The right coronary was dissected in 2 cases and the left coronary in the remaining cases. Coronary artery bypass was carried out using saphenous vein. Although one patient suffered myocardial infarction, all patients had successful recovery and evolution.

Post-coronariography dissection of the right coronary artery.

The case of a patient undergoing an emergency aortocoronary bypass to the right coronary artery after its acute dissection at coronariography is presented. Surgery was carried out two hours after the incident. After successful recovery and postoperative study, it was concluded that the main technical problem posed by this operation which was whether or not to ligate proximally the right coronary was satisfactorily solved by the initial decision not to ligate it.

[Effect of pregnancy and the puerperium on the development of extrinsic allergic alveolitis].

Extrinsic Allergic Alveolitis (EAA) is a disease characterized by an exaggerated immune response to inhaled organic dusts that can result in alveolar, and occasionally bronchiolar, damage. This damage can disappear completely with the elimination of the antigen and/or treatment with anti-inflammatory agents. Nonetheless, an important group of these patients do not improve; the inflammatory reaction persists. With time, this results in the deposit of abnormal connective, tissue with respect to its quantity and quality, in the pulmonary interstitium destroying the normal parenchymal architecture and making gas exchange impossible. The most common cause of this disease is inhalation of pigeon antigen with a 5:1 predominance in women. Pregnancy has been considered as a case of allotransplantation that produces in the mother a relative hyporeactive condition. In modifying the immune response to the fetus, it is also modified to other foreign antigens. In this work, we studied 80 cases of EAA, 67 females and 13 males. Of the 67 women, 11 (17.1%) presented with the disease after delivery. Symptoms began from 5 days to 7 months after delivery. Eight of the 11 women had been in contact with the antigen before the beginning of the pregnancy without showing symptoms until after delivery. In 4 cases, there had been contact with the antigen in other pregnancies, without symptoms. EAA is a disease involving the immune system in which changes can be seen in its development during pregnancy, as is also seen in other immune diseases, such as Lupus Erythematosus, Rheumatoid Arthritis and other connective tissue diseases. During pregnancy, the mother becomes tolerant. There is a decrease in non-self recognition.(ABSTRACT TRUNCATED AT 250 WORDS)
