Energy substrate utilisation was evaluated over 21 days in two groups of malnourished children on total parenteral nutrition (TPN). Non-protein energy was infused as glucose (Group A; n = 7) or as a glucose/fat (1:1 v/v) mixture (Group B; n = 10). Results indicated that: 1) net glucose oxidation was related to glucose intake; 2) glucose storage was elevated in group A; 3) net fat synthesis occurred earlier in group B together with constant net fat oxidation which was inversely related to glucose intake (r = -0.89, p < 0.001); 4) lipogenesis from glucose occurred only when glucose intake exceeded 19.3g/kg/d; 5) energy expenditure increased by 36% (group A) and 18% (group B) during renutrition; 6) 73% and 82% of the energy administered in excess of energy required was stored in group A and B respectively. Hence, glucose/fat infusion appears to be more energy-efficient than glucose-alone in TPN of malnourished children.
The results of the echocardiographic evaluation of 72 patients with hypertrophic cardiomyopathy are presented. We have measured left ventricular wall thickness in 8 different segments and classified our patients in 6 types according to the hypertrophy extent. Moreover, we have evaluated by Doppler ultrasound the presence and severity of mitral regurgitation and the left ventricular inflow and outflow. The hypertrophy cardiomyopathy pattern was symmetric in 8 patients, apical in five, and asymmetric in 31. According to Maron classification, asymmetric cardiomyopathy was I type: 4 patients, II type: 16, III type: 11, and IV type: 0 patients. There were left ventricular outflow obstruction (greater than 25 mmHg) in 26 patients (36.1%). This obstruction was more frequent in II and III type hypertrophic cardiomyopathy and we found significative relationship between septal posterior segment hypertrophy and left ventricular outflow obstruction. Ventricular inflow showed bad distensibility pattern in 45 patients (62.5%). Mitral regurgitation was mild, moderate and severe in 25, 15, and 4 patients. Left ventricular outflow obstruction, bad distensibility pattern and mitral regurgitation were independent each other.
A random sample group of 300 children and adolescents (aged 0.5 to 16 years) from a Mediterranean town of Spain, were investigated as part of a health education program. Analysis of dietary iron intake and the biochemical parameters of iron status (red cell volume, erythrocyte protoporphyrin concentration, transferrin saturation percentage and serum ferritin) were measured. Based on dietary assessment, the percentage of children with iron intake levels below the Catalonia Recommended Daily Amounts varied from 24% to 77% and showed age and sex differences. The percentage of children with iron intakes below their own individual requirements varied between 7% and 41%. The age groups with the highest risk of iron deficiency were girls aged between 6 and 24 months, and boys aged between 13 and 16 years. However, based on the biochemically assessed iron status, the results revealed an overall prevalence of iron deficiency varying from 0 to 23.3%. The highest prevalence was encountered in boys and girls between 6 and 24 months (23.3%), compared with only 3.3% between 13 and 16 years. Despite conducting the investigation with the recommended controlled methodology, the results from dietary assessed risk of iron deficiency were significantly different from those assessed biochemically. Hence, caution is required when designing health programme strategies based exclusively on nutritional intake assessments.
Among the 1,993 patients undergoing open-heart operations, our incidence rate of mediastinitis was 0.6%. Six patients have been treated with the pedicled omentum. Two of the 6 patients had a valvular disease and a coronary bypass the others. All patients had a good result with this procedure. After this experience, we think it is the best technique for management of this major complication.

To know the prevalence and prognostic significance of finger clubbing in hypersensitivity pneumonitis induced by avian antigen, this physical sign was evaluated in 82 patients who were followed up from 1 to 5 years (mean, 2.6 years). According to clinical, roentgenographic, and functional criteria, the patients were classified in one of three stages at admission as well as at least 1 year later. Digital clubbing was retrospectively recorded as present or absent by physical examination. Our results showed that 44 patients (51%) included in this study presented clubbing at the time of diagnosis. Sixteen of these patients presented with worsening of their lung disease, whereas only 5 of the 38 patients without clubbing incurred a worsening of their condition. This finding suggests that digital clubbing is frequent in pigeon breeder's disease and may help to predict clinical deterioration.
The efficiency of an autotransfusion program was studied over two groups of patients undergoing an elective surgery. The A group (n = 41) was on dietotherapy program associated with a supplement of ferrous sulfate and the B group (n = 35) on dietary intervention only. Red cell volume (RCV), haemoglobin (Hb) and haematocrit (Ht) were determined previously to the autotransfusion, during the same and before the surgical intervention. Serum ferritin levels were measured in the B group patients previous to the autotransfusion and before the surgical intervention. No statistically significant differences were observed among the two groups with regard to the RVC variations, Hb or Ht during the autotransfusion process. Although 85% of the A group patients showed secondary digestive effects due to oral ferrous therapy, no patients from the B group showed any sign of these effects whatsoever. The results of the present study suggest that a diet rich in easily bioavailable iron could be an alternative therapy during the autotransfusion procedure.
The role of the host cell RNA polymerase II in African swine fever (ASF) virus growth has been examined using inhibitors of this enzyme. The adenosine analog 5,6-dichloro-1-beta-D-ribofuranosylbenzimidazole (DRB), an inhibitor of mRNA precursor synthesis in mammalian cells, strongly inhibits the production of infectious progeny virus in Vero cells, but does not significantly affect the synthesis of virus-specific macromolecules. On the other hand, virion assembly seems to proceed normally in the presence of DRB, as virus particles can be seen in electron micrographs with a morphology indistinguishable from that observed in the absence of the inhibitor. However, taking into account the inhibition of the infectivity caused by the drug, most of these particles must be defective. In contrast with this effect of DRB on ASF virus replication, the toxin alpha-amanitin does not inhibit the production of infectious ASF virus in Vero cells or porcine alveolar macrophages. This result indicates that the host RNA polymerase II does not transcribe viral genes and that active transcription of the cell genome is not needed for ASF virus replication.
Highly purified African swine fever virus contains a cyclic AMP-independent protein kinase which phosphorylates endogenous virus proteins with a specific activity of about 0.45 pmol/microgram of virus protein. The major substrates for the virion protein kinase in vitro were the structural proteins p10 and p9. Both proteins were phosphorylated preferentially at serine residues. A possible relationship between protein p10 phosphorylation and RNA synthesis in vitro by the virion-associated RNA polymerase is suggested by the finding that N-alpha-tosyl-L-lysyl-chloromethyl ketone inhibited both phosphorylation of p10 and transcription. Two phosphoproteins, with molecular masses of 35 and 17 kDa, were found in African swine fever virus purified from infected Vero cells labeled with [32P]phosphate. A phosphopolypeptide with a molecular mass of about 35 kDa was found in the cytoplasm of infected Vero cells.

A Vero cell line persistently infected with African swine fever virus was established by infecting the cells in the presence of 10 mM NH4Cl (Vero-P cell line). The virus derived from the Vero-P cultures infected Vero cells, and virus titers were comparable to those obtained in Vero cells acutely infected with African swine fever virus. The structural proteins of the virus from Vero-P cells were similar to those of the virus produced in lytic infections. Virus production was low when the Vero-P cells were growing logarithmically and increased considerably in confluent cultures when lysis appeared in a fraction of the cell population.
