Marrow transplantation for juvenile osteopetrosis.

Two children with the juvenile form of osteopetrosis were treated with marrow transplants from their HLA identical siblings. Following transplantation each child exhibited extensive bone reabsorption with a marked augmentation of osteoclastic function attributable to donor osteoclasts, including remodeling of bone with expansion of intramedullary hematopoiesis and correction of associated abnormalities of thymic factor and natural killer cells. Osteopetrosis ultimately recurred in one patient in whom engraftment of donor hematopoietic elements was not achieved. Our studies indicate that marrow transplantation will correct osteopetrosis but that permanent reconstitution necessitates sustained engraftment of marrow precursors of cells with osteoclastic activity.

Unusual radiographic findings in ectopic ureterocele.

A case of an ectopic ureterocele with a cystic dysplastic upper pole and atretic proximal ureter was studied. A four-month follow-up showed marked decrease in the size of the dysplastic upper pole which is explained on an embryologic and pathophysiologic basis.

A case of neonatal intussusception.

A case of ileocotic intussusception in a 48-hr-old neonate due to saccular duplication of the terminal ileum is presented. The unique nature of intussusception due to a leading point is underscored and the case for early surgical intervention is emphasized. Barium enema examination is stressed as a prime diagnostic tool whenever intussusception is suspected.

Ultrasound diagnosis of ectopic ureterocele.

Three children with ectopic ureteroceles were examined with ultrasound, intravenous urography and cystography. In all cases the ultrasound studies outlined the ectopic ureterocele within the fluid-filled bladder and in one case added additional information regarding the non-functional portion of the duplication and its ureter.

Severe mixed metabolic acidosis secondary to rhabdomyolysis.

A patient with severe metabolic acidosis in association with rhabdomyolysis and nonoliguric acute renal failure is presented. Evaluation of his acidosis indicated a large excess of unmeasured anions as well as evidence of a renal acidifying defect. We conclude that the metabolic acidosis seen with myoglobinuric acute renal failure may represent a mixed acid-base disturbance.

Acquaintance networks among leukemia and lymphoma patients.

A case-control study of clustering through acquaintanceship among lymphoma and leukemia patients was conducted for the years 1967 through 1972 in Orleans County, New York. Twenty lymphoma and 17 leukemia cases met criteria for inclusion in the study. Data on acquaintanceship linkage were gathered from the source cases and controls, and from their acquaintances yielding a data base of 13,409 unique individuals linked by acquaintance. Three different analyses were carried out: a statistical analysis of linkage via intermediaries of case pairs in comparison with control pairs; a computer simulation of disease transmission from selected source cases to selected targets and controls based on the acquaintance data; and a secondary attack rate type analysis. The first two types of analysis yielded statistically significant case-control differences at the .05 level. The third method also yielded a positive result but was not subject to quantitative hypothesis testing. An additional conclusion is that these epidemiologic methods for disease with long induction periods merit further study.

Computed tomography in bone and soft tissue pathology of the extremities.

Forty patients suffering from various pathological conditions of the extremities were examined by computed tomography (CT) and the results were correlated with conventional imaging techniques: plain radiography, conventional tomograms, angiograms, and radionuclide scanning. The patients' ages ranged from 4 to 80 years. The study included cases of osteomyelitis, benign and malignant bone and soft tissue tumors, metabolic disorders, and a case of pulmonary osteoarthropathy. We were able to follow progression and/or regression of osteomyelitis by recording the changes in the attenuation coefficient of the medullary cavity and also were able to determine the extent of the malignant infiltration of the marrow cavity. Contrast enhancement was valuable in demonstrating the vascularity of soft tissue tumors and in determining the extension of primary bone lesions.

Complete and partial trisomy of different segments of chromosome 8: case reports and review.

This report describes some of the clinical, chromosomal and radiological findings in three unrelated patients with trisomy 8 mosaicism syndrome (T8ms), two first cousins with trisomy 8q and a patient with trisomy 8p. On the basis of the phenotypic and cytogenetic findings seen in our six patients and those noted in previous reports, we concur with Riccardi & Crandall (1978) that most physical malformations seen in T8ms are associated with trisomy for the long arm of chromosome 8.

Tubular stasis and ectasia due to intrarenal obstruction.

Segmental tubular stasis and ectasia may be early signs of intrarenal obstruction. Case reports of 3 patients are presented. In arriving at a diagnosis, the possibility of significant underlying pathology such as calculi and neoplasm should be considered.
