A suction device for evacuation of gallbladder stones.

Interventional radiologists are being asked more frequently to evacuate stones from the gallbladder. In order to facilitate such evacuation in patients with a large number of small gallbladder stones, a readily available medical suction machine was adapted for this purpose, and we describe here the adaptation and its successful use in six patients.

Obstructive jaundice and cholangitis due to choledocholithiasis: treatment by extracorporeal shock-wave lithotripsy.

Endoscopic shock-wave lithotripsy, although now the standard treatment of urolithiasis, has only recently been applied to cholelithiasis. The authors describe the case of an 88-year-old man, a high-risk patient with choledocholithiasis, in whom endoscopic stone extraction after sphincterotomy failed. Extracorporeal shock-wave lithotripsy was used for noninvasive stone fragmentation and the fragments were passed without complication.

Intraductal papillary carcinoma of the pancreas.

We report a case of intraductal papillary adenocarcinoma of the distal pancreatic duct. Although a rare subgroup of exocrine pancreatic tumors, their diagnosis is an important one in view of their favorable prognosis with pancreatectomy. Because of the established behavior of villous tumors of the colon, to which they are similar histologically, these tumors should be resected, even if biopsy shows benign disease. Patients should be followed radiologically for recurrence or the development of new adenoma following resection.

MR imaging artifacts produced by dental materials.

This report evaluates a variety of dental materials with respect to MR imaging artifacts and their mechanisms of production. A 0.15-T superconducting system and an MR phantom were used in assessing imaging artifacts. Several in-vivo studies were also carried out. The dental materials were analyzed for composition and magnetic permeability and the results were compared with the artifacts produced by each material. It was found that the higher the magnetic permeability the greater the artifact produced. Magnetic permeability of a material is generally not predictable on the basis of its composition, however.
