[The evaluation of complications caused by insertion of Veres needle and trocars placement based on analysis of 4211 laparoscopic operations].

The retrospective analysis of 4211 laparoscopic operations was performed. The complications caused by insertion of Veres needle and trocars placement were found in 66 cases (1.6%). There were: retropneumoperitoneum--in 33 cases (8@1000), vascular injuries--22 cases (5@1000), injuries of the gastrointestinal organs--9 cases (2@1000). In 2 cases (0.4@1000) perforation of the wall of urinary bladder occurred. The accurate preparation of patients to laparoscopy, knowledge of management with complications, ability to laying sutures through all of abdominal layers, using of Foley's catheter for haemostasis enabled to cure both the main disease and complications.

[The analysis of caesarean sections in women residents of Bytom and in other women].

Authors analysed indications and number of caesarean sections performed in the I-st Department of Obstetrics and Gynaecology in Bytom in the years 1994-1998. In the analyse we considered place of living of pregnant women, dividing women on residents of Bytom and non-residents of Bytom. The results showed increasing number of caesarean sections. Percentage of caesarean sections in the group of non-residents of Bytom was twice higher as in the group of residents of Bytom. The most common indications were status after prior caesarean section or other uterus operation, fetal distress and fetus malposition.

[The assessment of expression of kininogen and bradykinin receptors genes in different vulvar pathologies by QPCR (TaqMan)].

OBJECTIVES: In different vulvar pathologies inflammatory process and pain are often observed. In these processes, kinins, released from kininogen, play an important role. Their effects are mediated by at least two types of bradykinin receptors--B1 and B2. B1 receptor appears in certain pathological states, B2 is widely distributed in normal tissues. The expression of genes coding kininogen, B1 and B2 receptors can be a very sensitive marker of tissue pathology. DESIGN: In the present study, the analysis of expression of genes coding kininogen, B1 and B2 was performed. The relation between the analysed genes expression and the pathology stage was analysed. MATERIALS AND METHODS: The specimens from condylomata accuminata, vulvar cancer and surgical margin were analysed. The number of DNA and mRNA copies of beta-actine, kininogen, B1 and B2 were examined basing on Q-PCR standard curves for beta-actine by use of Perkin Elmer-kit and the sequence detector ABI PRISM 7700-Taq Man application. RESULTS: In condylomata accuminata the high expression of mRNA of kininogen, B1 and B2 was found, while in vulvar cancer tissue, the expression of analysed genes was low. In the tissue from the tumour center, the lowest kinin genes expression was stated. CONCLUSIONS: The absence of kininogen and B2 mRNA expression characterised vulvar cancer tissue. The profile of expression of kininogen and its receptor genes can be a useful marker in the assessment of vulvar cancer surgical margin.

[The quantitative analysis of E6 HPV and P53 genes expression by QPCR (TaqMan) in the assessment of surgical treatment range in vulvar carcinoma induced by HTV infection].

OBJECTIVES: Human Papilloma Virus (HPV) infections especially 16 and 18 are risk factors for squamous cell vulvar cancer. E6 protein of HPV joins the tumor suppressor protein P53 and promotes its degradation. This is one of the possible mechanisms of viral oncogenes action. DESIGN: In this study the quantitative analysis of mRNA copies E6 HPV 18 and mRNA P53 expression in vulvar cancer tissue was performed. The expression of analysed genes was applied in the assessment of surgical treatment range. MATERIALS AND METHODS: The specimens from a 26 year old woman with vulvar squamous cell cancer stage II FIGO treated surgically modo Way were analysed. The number of DNA and mRNA copies E6 HPV and P53 were examined basing on Q-PCR standard curves for beta-actine by use of Perkin Elmer-kit and the sequence detector ABI PRISM 7700-Taq Man application. RESULTS: The overexpression of mRNA E6 HPV and P53 in analysed specimens was found. The highest number of mRNA copies in cancer tissue was ascertained. In lichen sclerosus and lymphonoduli tissue lower number of analysed copies was found. CONCLUSION: The quantitative analysis of E6 HPV and P53 genes expression can be useful in the assessment of surgical treatment range in vulvar cancer and can also be used as a prognostic marker.

[Perinatal outcome in multiple pregnancy after natural reproduction and ART techniques].

In this study we analysed occurrence of multifetal pregnancy in groups of patients after natural reproduction and after using ART between 1991-1998 in clinical material. We showed the most important complications during pregnancy, occurrence of TTTS, percentage of prematurity, percentage and indications to caesarean sections. We found that above-mentioned manifestations of perinatal care were comparable with reports of Polish and foreign authors.

[Analysis of indications for cesarean section in pregnancy complicated by diabetes mellitus].

The authors analysed a way of pregnancy ending and indications to cesarean section in group 256 diabetic pregnant women. We estimated that percentage for cesarean sections was 51.56%. We noticed downward tendency of elective indications to cesarean section (the most common was gestosis) and upward tendency of intrapartum indications (the most common was fetal distress syndrome). The percentage of cesarean sections in diabetic pregnant women group in our study is similar to results from the other centres.

[Lack of relationship between angiotensinogen gene m235t polymorphism and gene insertion/deletion (I/D-intron 16) and Pst I RFLP (P/M-intron 7) polymorphisms of the angiotensin I converting enzyme(ACE) gene and the development of H-gestosis. Preliminary results].

Genetic and familial factors may predispose to H-gestosis. The aim of our study was to answer the question if angiotensinogen gene m235t polymorphism, and ACE gene I/D and Pst I RFLP polymorphisms may be markers of genetic predisposition to the H-gestosis. 246 pregnant women (median age 26 years) were studied (the studied group consisted of 116 women with H-gestosis and the control group consisted of 130 healthy pregnant women). Genotyping was performed using polymerase chain reaction method. Statistical analysis was done by means of Statistica for Windows. Genotype distribution was analyzed using chi 2 test. P < 0.05 was considered as statistically significant. In our study we did not receive statistically significant differences in ACE and angiotensinogen genes genotype distributions and allele frequencies between the investigated groups. Based on results of the study we may suggest that I/D and Pst I RFLP ACE gene polymorphism and angiotensinogen gene m235t polymorphism do not play any significant role in the pathogenesis of H-gestosis.

[Carcinoma of the vulva in women: clinical and morphopathological problems].

Surgery is the mainstay of the treatment of vulvar cancer. Radical vulvectomy together with some form of lymphadenectomy is the usual treatment today, but the desire to minimize morbidity have led to a selective decrease in the extent of the surgery in recent years. We recommended retroperitoneal pelvic lymphadenectomy be used only if the inguinal nodes were positive. The approach has become widely accepted. Improvements in perioperative care and anesthetic techniques have allowed the surgical approach to be used in most patients, even in older women seen so commonly with this cancer.

[Surgical procedure connected complications in women with cervical cancer operated with ovarian transposition (transpositio ovariorum)].

OBJECTIVES: Approximately one-half of women with cervical cancer are premenopausal. Preservation of ovarian function in these women is highly desirable. The safety and efficacy of ovarian retention include risk of ovarian metastasis, risk of surgery, psychological impact of surgical castration, expense of hormone supplementation. DESIGN: This report compares intra- and postoperative surgical complications in 2 groups of women with cancer in stage Ib and IIa treated by radical hysterectomy (Wertheim-Meigs operation). MATERIALS AND METHODS: Two groups were compared: 72 patients with ovarian transposition and 55 women without transposition, but after brachy-HDR therapy. The surgery time was mean 30 minute longer in case of ovarian transposition. RESULTS: There was no difference in mean time of hospital stay after the surgery, the frequency of relaparotomy, deep vein thrombosis, urological complications and blood transfusion between the groups. There was confirmed the higher number of lymphatic pseudocysts (p < 0.0005) and fever (p < 0.001) in women without transposition, but after brachy-HDR therapy. We linked this with the previously applied radiotherapy. CONCLUSIONS: We conclude that ovarian transposition is the procedure which can safe the endogenous ovarian function in young women with cervical cancer without the higher risk of complications.

[Analysis of indications for 3528 Cesarean sections based on sixteen years of clinical material (177-1992)].

Analysis was subjected on 3528 cesarean sections made in I Obstetric and Gynecology Clinic Academy of Medicine in Bytom between 1977-1992. At the drop in general number of deliveries there were noticed upward tendencies in percentage of cesarean sections but during last two years this increase was inhibited. The ratio of sudden and planned indications determined by us as index of indications (N/P) oscillated from 0.16 to 0.31. Among sudden indications imminent intrauterine fetus asphyxia was noticed most often and among planned indications state after cesarean section and loss of progress of delivery.

[Radical vulvectomy using Way's method with bilateral removal of inguinal, thigh, iliac and veil lymph nodes in the clinic].

Thirty-one cards of women after radical vulvectomy in Way's mode were analysed. Metastases in inguinal, thigh, iliac and veil lymph nodes were found only in 19.35%. Authors conclude that removal of iliac and veil lymph nodes can be given up after previously negative outcome of intra-operative examination of inguinal and thigh lymph nodes. Mean period of patient's staying in hospital was long--about 40 days. In 51.6% of cases the blood group was "A", with the mean for the whole country 40.5%.

[Laparoscopic hysterectomy with vaginal extraction].

In the paper authors present 30 laparoscopic hysterectomy made during 5 month (november 1992 till march 1993). Indications to laparoscopic hysterectomy were: myomata uteri, recurrent metrorrhagia and CIN III. The middle duration time of operation was 153 min. and the middle blood loss during operation was 122 ml. 28 patients during first control after operation felt well. In 1 patient intraoperative injury of urine bladder was noted and in next injury of left ureter.

Some parameters of lipid metabolism in women 6 years after surgery on account of uterine leiomyoma.

The examinations were carried out in 44 women (age between 40 and 60 years) six years after surgery on account of a uterine leiomyoma. The total cholesterol (TC) level, HDL-cholesterol (HDL-C), LDL-cholesterol (LDL-C), triglycerides (TG) and 17 beta-estradiol (E2) as well as the values of the atherogenic coefficient HDL-C/TC- and HDL-C/LDL-C were assessed. The subjects were divided into 3 age groups as well as groups with regard to the extent of surgery. In younger women with partial oophorectomy no changes of the lipid metabolism were found. In older women, however, with hysterectomy and oophorectomy, the risk of atherosclerosis reached the threshold of compensatory abilities and was marked by a decrease of mean values of HDL-C/LDL-C (p < 0.05), with an increase of HDL-C/TC- (p < 0.001).

Risk of atherosclerosis in women six years after uterine leiomyoma surgery as compared with healthy women.

44 women at the age between 40 and 60, who underwent uterine leiomyoma surgery six years previously and 45 healthy women were examined for total cholesterol (TC), HDL-cholesterol (HDL-C), LDL-cholesterol (LDL-C), triglycerides (TG) and 17 beta-estradiol (E2). The value of the atherogenic coefficient [sequence: see text] and [sequence: see text] were determined. The clinical condition of the women was also evaluated. It was shown that independently on the extent of surgery, significantly (p < 0.01 and p < 0.001) higher values of LDL-C and lower values (p < 0.05) of atherogenic coefficients of [sequence: see text] were recorded, which is evidence of risk atherosclerosis and coronary heart disease (CHD) in all groups of women. A greater intensity of atherosclerotic changes in women with bilateral oophorectomy (group B'beta) may be connected both with older age and a higher body weight inducing these processes and is evidence of pathogenetic disorders caused by laparotomy. We confirmed that oophorectomy in women with leiomyoma during the perimenopausal period does not result in atherosclerosis and CHD risk when preserving the ovaries during laparotomy.

[Evaluation of the influence of certain epidemiologic factors on development of uterine myomas].

Age of menarche, fertility, number of labours, body weight, physical and mental activity, and incidence of stress in everyday life were evaluated in 72 women with uterine myomas, aged 34-54 years. The control group consisted of 45 healthy women at similar age. It was demonstrated that overweight (p < 0.05, p < 0.001 in most groups studied) in combination with low physical activity and higher mental (stress producing) was a risk factor in the aetiology and pathogenesis of this disease. Fertility and number of labours as well as the age of menarche were unchanged (p > 0.05).

[Laparoscopy in clinical practice. I. Indications].

In the paper the authors analyzed indications to 1527 laparoscopies. The most frequent indications were: sterility (72.2%) and suspicions of adnexal tumours or extrauterine gravidity (14.1%). In 173 diagnostically difficult cases laparotomy was avoided. In these cases laparoscopy allowed to exclude the pelvic pathology or to quality to laparoscopic operation of ovarian cysts and intact tubal pregnancy. Diagnostic and operative laparoscopy give the possibility of quick diagnosis in diagnostically difficult cases and enables avoidance of laparotomy in many cases.

[Laparoscopy in clinical practice. II. Usefulness in diagnosis of sterility].

The paper presents results of 1099 laparoscopies performed in sterility. Laparoscopy was made in the second phase of the menstrual cycle, 5 to 7 days after probable ovulation, as a last stage in diagnosis of sterility. Normal state of genital organs and minor pelvis was noted in 20.9% of investigated patients. In the remaining more than one etiologic factor was found, which may influence the sterility. The authors concluded, laparoscopy is valuable, accessory method in the diagnosis of sterility, which enables sometimes to find the etiologic factor in patients with "unexplained infertility".
