[Intraoperative endoscopic surveillance in laparoscopic surgical treatment of achalasia].

We study the utility of intraoperative endoscopic control, in laparoscopic surgical treatment of the typical Achalasia of Esophagus. 81 patients were intervened surgically in Centro de Cirugia Endoscopica. Hospital Universitario Calixto Garcia in a five year-old period. All patients received intraoperative endoscopic evaluation to guarantee the effectiveness and integrity of the esophago-cardiomyotomy. The average time of surgical procedure was of 144,7 minutes. The transendoscopic complication in relation with endoscopic procedure was seen in one case but it didn't modify the morbi-mortality of the study. According to Vantrappen classification, 79 patients were considered with a evolution between excellent and good. The use of endoscopic intraoperative control is recommended in the surgical interventions of the patients with achalasia.

[Modified EMJH medium for cultivation of Leptospira interrogans serogroup ballum].

Strains within the Ballum serogroup of spirochete Leptospira show fastidious growth with more exigent nutritional requirements than those of other Leptospira pathogenic strains. The influence of 37 nutritional compounds on the growth of Leptospira interrogans serogroup Ballum was investigated employing the synthetic EMJH medium as the base for the study. Microbial growth was estimated spectrophotometrically and direct counts were performed with a Petroff-Hausser counting chamber. Virulence stability was evaluated by calculating the mean lethal dose in hamsters. Antigenicity stability was evaluated by Western blotting using a specific antiserum. Cell yields commonly obtained in EMJH were triplicated without virulence or antigenicity depletions after culturing in a modified EMJH medium with an increased concentration of Tween 80, and the incorporation of sodium acetate and beef extract. Neither the increased concentration of at least 6 components of EMJH nor the incorporation of a variety of new nutrients stimulated cell yields or the growth rate of the microorganism. The results allow us to make use of an enriched culture medium that promotes high cell yields of this fastidious serogroup most prevalent in humans in Cuba.

Sporadic amplification of the c-fms proto-oncogene in human musculoskeletal sarcomas.

Fresh samples of bone and soft tissue sarcoma from 31 patients were analyzed by the Southern technique for amplification or other structural abnormalities of the c-fms oncogene. As a sole finding, amplification of the c-fms (4- and 10-fold) was detected in 2 of 3 cases with a histologic diagnosis of liposarcoma. RFLP analysis disclosed no gene rearrangements associated with the c-fms amplification. The case with the highest amplification of the c-fms gene was disease-free at review. The 2 fms-amplified liposarcomas were aneuploid according to DNA flow cytometry. This is the first study demonstrating amplification of the c-fms proto-oncogene in human liposarcoma. The fact that this structural abnormality of the c-fms gene was only detected in liposarcomas might point to an uncommon but tissue-specific phenomenon.

[Study of spontaneous cytotoxic activity in laryngeal carcinoma: prognostic value].

The status of natural killer (NK) cell activity in peripheral blood, based on number and functional state, was studied in relation to the clinical and histopathologic stage of 52 patients with laryngeal carcinoma and in 23 healthy controls. The number of NK cells, estimated using CD16 and CD56 monoclonal antibodies, was similar in patients and controls and showed no relation to tumor size and nodal involvement. NK cell function did not show significant differences in spontaneous cytotoxic activity either overall or in relation to tumor size and the presence of palpable lymph nodes. However, cytotoxic activity was significantly lower in patients who had histologically confirmed nodal involvement. NK activity under 36% (the percentage of specific lysis at an effector:target dilution of 50:1) was suggested the probable presence of nodal metastases and was a highly sensitive and specific test. In patients with laryngeal carcinoma, NK-cell cytotoxic activity may be an independent prognostic parameter for evaluating cervical lymph node involvement.

Middle term intestinal adaptation after massive distal small bowel resection in oral feeding dogs.

Intestinal adaptation (IA) consists mainly in a increase of the normal cell proliferative rate in the crypts of the intestinal mucosa. Histidine- and ornithine-decarboxylase (HDC and ODC, respectively) are always involved in the rapid growing tissue process. The present experiment studies the relation of HDC and ODC, the morpho-functional changes of the jejunal mucosa remnant and the nutritive state 2, 3 and 4 weeks after performing a 75% distal small bowel resection (DMSBR) in dogs fed with a standard chow. Each animal was its own control measure in a healthy state measured before provoking the DMSBR. The results demonstrated that 14 days after DMSBR, the mucosa of the jejunal remnant showed a loose of the normal characteristics of the villi and enterocyte morphology ("mucosal microinjuries"), accompanied by an increase in the depth of the crypt and in the HDC and ODC levels, while the D-glucose and L-phenylalanine absorptive capacity did not vary compared with that of the mucosa of the same animals in a healthy state. This correspond with a statistically significant altered nutritive state parameters. However, 21 days after DMSBR, the intestinal remnant mucosa initiates a structural recovery process. It was also observed a significant increase in the HDC and ODC enzymatic levels accompanied by an increased absorptive capacity and an improvement in the nutritive state parameters. Twenty eight days after DMSBR, the findings revealed a similar trend. In conclusion, in our model, between the second and fourth week the IA process is accompanied by a progressively increased HDC and ODC activities and an improvement of the nutritive state. The significance of the "mucosal microinjuries" described needs further investigation.

[Antinuclear antibodies in multiple sclerosis].

The prevalence of circulating antinuclear antibodies (ANA) was studied in a group of 147 patients with multiple sclerosis (MS). Also analyzed was the evolution of ANA over time in those patients for whom the first analysis was positive. ANA titers > or = 1/50 were considered positive. Twenty-two of the MS patients (15%) were positive for ANA upon the first analysis. In a control group of patients with other non-autoimmune neurological diseases the prevalence of ANA positivity was 6.1%. ANA titers became normal in 94.7% of the MS patients over a mean maximum period of 22.5 +/- 19.2 months. We known of no other study of the evolution of ANA titers in MS patients. Because ANA fluctuates in MS, the titers of other circulating autoantibodies should be interpreted cautiously both when discussing etiopathogenetic hypotheses and when diagnosing associated diseases. The diagnostic usefulness of ANA determination in MS is low.
