In 37 patients and 143 control patients we estimated tear fluid lysozyme content by the Micrococcus lysodeikticus agar diffusion assay. We found no correlation between the titer of lysozyme in tear fluid and the rate of tear flow. Decrease in lysozyme production was found to be a sensitive indicator of the involvement of the lacrimal system in Sjögren's syndrome.
The dissection of the facial musculature in the elderly made on 2 bodies, confirmed the mode of evolution noted earlier in the Primates. The reduction in the m. platysma myoides was confirmed; this muscle showed only two parts: a rudimentary pars buccalis and a relatively developed pars mentalis. This reduction was accompanied by fundamental transformations since the m. quadratus of the chin appeared at the anterior part of the pars mentalis of the platysma myoides. The derivatives of the deep m. sphincter coli showed an arrangement very similar to that of the Simians. However some modifications already observed became stronger, particularly the verticilisation of the m. caninus and the separation of the two m. zygomaticus. Curiously, the extrinsic ear musculature persisted and remained well developed despite its absence of activity.

A case of progressive multifocal leukoencephalopathy (PML) with a classic clinical presentation but with unusual pathological and radiographic findings is reported. The pathology revealed evidence of prior hemorrhage, and imaging studies revealed focal cerebral atrophy as well as contrast enhancement on MR scans. The contrast enhancement was visible only by utilizing magnetization transfer pulses on T1-weighted scans. The case report indicates that image criteria for PML may need to be redefined in the future.
A novel technique that can be used for reacting toxic carbon dioxide (CO2) emissions from power plants and other combustion wastes with sea water is described. A chemical interaction between CO2 and the cations in sea water, with the pH electrolytically regulated, can precipitate almost all the calcium and magnesium ions, as well as some sodium and potassium ions, as carbonates and bicarbonates. The carbonates and bicarbonates thus prepared can then be mixed with ash to yield a building material. Sulfur ions will be neutralized with calcium and magnesium, and the remaining ions can be removed using reverse osmosis or some other method. The technology and equipment for purification are based on modules that can be used for industrial waste-water, sea water, solutions, and otherwise. The module for separation of sand and suspended coarse substances consists of a tank for flocculation, coagulation, and precipitation of solid particles; and a low-pressure hydrocyclone. The module for purification from oil and fine suspensions is based on column flotation, flotation with a special ejector, and adhesion flotation. The module for ions and colloids consists of an absorbing filter with zeolite, fly ash, and other absorbing materials. Using a laboratory model consisting of a special mini-plant, we processed 10 L of factory-waste water containing more than 20 g/L organic content (compare with the upper limit of 0.02 g/L allowed by the Ministry of Environmental Protection in Israel). After the experimental solution was treated and evaporated to a small bulk, the water obtained was almost clear. On the basis of the results in the model, we present a scaled-up process for the design, development, and production of equipment for and the assembly of a large installation for drainage and water purification.

A duplicated longitudinal bracketed epiphysis or the "kissing delta" phalanx is not well recognized in the world literature. This represents complex double-delta bones with a longitudinal epiphyseal bracket. This complex is composed of a back-to-back fusion of adjacent delta bones enclosed in a bracket. The convex side of each delta phalanx faces the other; hence the term kissing-delta bone. These delta bones have a very bizarre appearance and are associated with the Rubinstein-Taybi syndrome. We have seen a total of 36 kissing-delta phalanges in the foot. We discuss the natural history and treatment options of this very rare congenital anomaly. This is not a surgical-outcome study, but surgical treatment is observed.
SPECT has a high sensitivity for the diagnosis of coronary artery disease. Dual isotope protocol using rest thallium and stress MIBI was introduced in Mexico 4 years ago. This protocol reunite the advantages of both radiotracers for the study of myocardial perfusion. We present our experience of the first three years. One thousand six hundred patients were studied with suspected myocardial ischemia; 288 were excluded because of an absence of a proper follow up. In 895 of the 1312 patients a coronariography was performed. Images were evaluated by dividing the heart in 20 segments using a 5 points scale (0 = normal to 4 = absence of perfusion). It was considered a perfusion defect when a segment had a score greater or equal to 2 and the SPECT study was considered abnormal if two or more segments had a MIBI stress score equal or greater than 2. The global sensitivity for diagnosis of ischemia was 96.28%. Dual isotope method is appropriate for the diagnosis of ischemic heart disease. It has a high sensitivity and specificity for the recognition of global coronary disease and for specific coronary territories. This work constitutes the greatest series in Latin America that uses this diagnosis method.
