Glomerular hyperfiltration in type I, type II, and secondary diabetes.
Glomerular hyperfiltration, a risk factor for diabetic nephropathy, has been reported in type I insulin-dependent diabetics, but it is not clear if it occurs in other types of diabetes. To ascertain the prevalence of glomerular hyperfiltration in various types of diabetes, we measured glomerular filtration rate (GFR) in 158 diabetics (91 type I, 36 type II without insulin treatment, 20 type II with insulin treatment, and 11 subjects with diabetes secondary to chronic pancreatitis), and classified them as hyper-, normo-, or hypofiltration according to values measured in 36 age-match controls. After elimination of subjects with overt renal disease or hypertension, glomerular hyperfiltration was detected in 35% of the type I diabetics, 32% of the type II diabetics without insulin treatment, one subject with chronic pancreatitis, and one type II diabetics with insulin treatment. Glomerular hyperfiltration was associated with high blood glucose in type I, insulin-dependent diabetics, and with a high apolipoprotein B/A1 ratio in type II, non-insulin-dependent diabetics without insulin treatment. In all subjects with glomerular hyperfiltration, GFR values and urinary albumin excretion were positively related (r = 0.33; n = 34; p = 0.05). Glomerular hyperfiltration is detectable among all types of diabetics.

Magnetic resonance sounding: new method for ground water assessment.
The advantage of magnetic resonance sounding (MRS) as compared to other classical geophysical methods is in its water selective approach and reduced ambiguity in determination of subsurface free water content and hydraulic properties of the media due to the nuclear magnetic resonance (NMR) principle applied. Two case examples are used to explain how hydrogeological parameters are obtained from an MRS survey. The first case example in Delft (the Netherlands) is a multiaquifer system characterized by large signal to noise ratio (S/N = 73), with a 24 m thick, shallow sand aquifer, confined by a 15 m thick clay layer. For the shallow aquifer, a very good match between MRS and borehole data was obtained with regard to effective porosity n(e) approximately 28% and specific drainage S(d) approximately 20%. The MRS interpretation at the level deeper than 39 m was disturbed by signal attenuation in the low resistivity (approximately 10 omega(m)) media. The second case of Serowe (Botswana) shows a fractured sandstone aquifer where hydrogeological parameters are well defined at depth > 74 m below ground surface despite quite a low S/N = 0.9 ratio, thanks to the negligible signal attenuation in the resistive environment. Finally, capabilities and limitations of the MRS technology are reviewed and discussed. MRS can contribute to subsurface hydrostratigraphy description, hydrogeological system parameterization, and improvement of well siting. The main limitations are survey dependence upon the value of the S/N ratio, signal attenuation in electrically conductive environments, nonuniformity of magnetic field, and some instrumental limitations. At locations sufficiently resistive to disregard the signal attenuation problems, the MRS S/N ratio determines how successfully MRS data can be acquired. Both signal and noise vary spatially; therefore, world scale maps providing guidelines on spatial variability of signal and noise are presented and their importance with respect to the MRS survey results is discussed. The noise varies also temporally; therefore, its diurnal and seasonal variability impact upon the MRS survey is covered as well.

[On preclotting of woven Dacron vascular prostheses (author's transl)].
The role and the clinical value of common protocols of preclotting for a woven Dacron vascular prosthesis have been investigated. A comparative study of blood loss was carried out for Standard Woven De Bakey protheses inserted in a specially constructed extracorporeal blood circuit with canine models. The system detected transport of blood across the wall of the prostheses. These measurements were supplemented by gravimetric assessment of blood clot density and scanning electronmicroscopy examination of the internal surfaces of the prostheses. The pre-operative protocols investigated included : absence of pre-treatment, precoagulation with anticoagulant-free blood and various techniques using heparinized blood subsequent to an initial blood contact (modified method of Sauvage). The effect of using heparinized animals was also investigated. Leakages in unclotted prostheses could be reduced almost five fold by any precoagulation technique. Heparinization did not cause a significant increase in leakage and considerably improved the quality of the newly deposited blood product lining on the prostheses which consisted almost entirely of fibrin and eliminated any embolization.
