The t(7;11)(p15;p15) translocation in acute myeloid leukaemia fuses the genes for nucleoporin NUP98 and class I homeoprotein HOXA9.

The t(7;11)(p15;p15) translocation is a recurrent chromosomal abnormality associated primarily with acute myeloid leukaemia (FAB M2 and M4). We present here the molecular definition of this translocation. On chromosome 7 positional cloning revealed the consistent rearrangement of the HOXA9 gene, which encodes a class I homeodomain protein potentially involved in myeloid differentiation. On chromosome 11 the translocation targets the human homologue of NUP98, a member of the GLFG nucleoporin family. Chimaeric messages spliced over the breakpoint fuse the GLFG repeat domains of NUP98 in-frame to the HOXA9 homeobox. The predicted NUP98-HOXA9 fusion protein may promote leukaemogenesis through inhibition of HOXA9-mediated terminal differentiation and/or aberrant nucleocytoplasmic transport.

Functional and morphological assessment of rat aorta stored in University of Wisconsin and Eurocollins solutions.

University of Wisconsin (UW) and Eurocollins (EC) solutions are widely used for preservation of organs before transplantation. However, effect of storage solutions on vascular interface for transplant success is not known. In this study, we have used rat aorta as a model and assessed the effects of cold storage in UW and EC solutions on smooth muscle and endothelial function and the morphology. Smooth muscle and endothelial functions of the rat aorta were assessed using in vitro isometric tension measurement. Morphologic studies were done with scanning and transmission electron microscopy. No significant difference in contractile response to either norepinephrine (NE) or potassium chloride was observed between control aorta and aorta stored in UW solution for 1 hr or 24 hr. In contrast, sensitivity, but not the reactivity to NE and KCl, was increased in aorta stored in EC solution for 1 hr. If the tissues were stored in EC solution for 24 hr, both sensitivity and reactivity to NE and KCl were significantly reduced. Relaxatory response to acetylcholine, in endothelium-intact vessels were reduced in aortas stored in EC solution, but not in UW solution. The magnitude of relaxations observed in tissues stored in the EC solution for 24 hr was less than in tissues stored for 1 hr. Sodium nitroprusside elicited similar relaxatory response in endothelium-denuded control tissue and in tissues stored in UW and EC solution. Electron microscopy data revealed marked swelling of the cell, loss of mitochondria and other intracellular organelles, and striking calcium deposits after preservation of the vessels in EC for 1 or 24 hr. In aorta stored in UW solution for 24 hr, endothelial and smooth muscle cells were intact, with moderate-size vacuoles in the cytoplasm. These results suggest that the UW solution is more suitable than EC solution for short-term preoperative storage of blood vessels.

Identification of breakpoints in t(8;21) acute myelogenous leukemia and isolation of a fusion transcript, AML1/ETO, with similarity to Drosophila segmentation gene, runt.

We have developed a restriction map of the chromosome 21 breakpoint region involved in t(8;21)(q22;q22.3) acute myelogenous leukemia (AML) and have isolated a genomic junction clone containing chromosome 8 and 21 material. Using probes from these regions, rearrangements have been identified in each of nine cases of t(8;21) AML examined. In addition, we have isolated cDNA clones from a t(8;21) AML cDNA library that contain fused sequences from chromosome 8 and 21. The chromosome 8 component, referred to as ETO (for eight twenty-one), is encoded over a large genomic region, as suggested by the analysis of corresponding yeast artificial chromosomes (YACs). The DNA sequence of the chromosome 21 portion of the fusion transcript is derived from the normal AML1 gene. A striking similarity (67% identity over 387 bp, with a corresponding 69% amino acid identity) was detected between AML1 and the Drosophila segmentation gene, runt. The critical consequence of the translocation is the juxtaposition of 5' sequences of AML1 to 3' sequences of ETO, oriented telomere to centromere on the der(8) chromosome.

Transcutaneous electrical nerve stimulation for pain control after spinal fusion with Harrington rods. A clinical report.

During the past two years, we have been using transcutaneous electrical nerve stimulation (TENS) for pain control after spinal fusion with Harrington rods. This report describes our postoperative pain management program and our evaluation of its effectiveness. We examined postoperative pain medication usage and found that patients using TENS received fewer doses of several commonly prescribed pain drugs than did patients not using TENS devices. Difficulties we encountered in evaluating our program are described along with recommendations for other researchers interested in assessing TENS in the management of postoperative pain.

Morphologic and cytochemical observations on the overt leukemic phase of therapy-related leukemia.

Comparison of morphologic and cytochemical features of the leukemic cells of patients with overt acute nonlymphocytic leukemia following therapy for primary malignancies (2 degrees-ANLL) to those of patients with ANLL de novo revealed several differences. No Auer rods were seen in cells of patients with 2 degrees-ANLL, but they were seen in 54 (42%) of 129 cases of ANLL de novo. Only one (4.7%) of 21 patients with 2 degrees-ANLL was classified as M-4 by FAB criteria, compared to 46 (36%) of 129 patients with ANLL de novo classified as M-4. Only one (10%) of 10 patients with 2 degrees-ANLL had more than 10% peroxidase-positive leukemic cells, whereas 19 (95%) of 20 of a control group of patients with ANLL de novo of similar FAB subtypes had more than 10% positive cells. Two of 11 (18%) 2 degrees-ANLL patients had 10% or more naphthol-AS-D chloroacetate esterase-positive cells, in contrast to nine (47%) of 19 patients with ANLL de novo. Nonspecific esterase activity was present in 10% or more of cells in only one (10%) of 10 patients with 2 degrees-ANLL studied, as compared to seven (35%) of 20 of the control group. Previous reports indicate that patients with 2 degrees-ANLL have cytogenetic abnormalities and therapeutic responses that differ from those in ANLL de novo; our findings indicate that there are morphologic and cytochemical differences as well.
