[The intracellular multiplication of "Brucella" The theory for an experimental model (author's transl)].

Brucella melitensis can penetrate and multiply in cells of the BHK21 line. The intracellular bacteria can be counted either by culture after destruction of the cells or by direct examination after staining; in the latter case, the cells are distributed into various classes according to the number of bacteria which they contain. The number of intracellular bacteria increased exponentially and the division time is 12 hours. The intracellular division of the bacteria is independent of the cellular invasion. The infection by bacteria is not a factor related to mortality nor to survival of the cells. Whatever the number of bacteria initially placed on the BHK21 cells (up to 1,000 bacteria per cell), the number of infected cells never increased about 20%, and this indicates that all the cells do not have the same aptitude for the phagocytosis of Brucella. A mathematical model is suggested. Taking all factors into account, the model shows the probability for a certain number of Brucella per cell and, on the other hand, it shows the mean number of Brucella per cell at any particular moment. This model can be used in the case of other bacteria and of other cells, and can be used whenever a precise study of the intracellular multiplication of bacteria is required.

[Value of a papain-treated homologous microfilarial antigen in the diagnosis of filariasis due to Wuchereria bancrofti var. pacifica. Analysis of 237 cases].

An indirect immunofluorescence assay using papaïn treated microfilariae (IFI-WBp) has been performed to attempt diagnosis of bancroftian filariasis. The authors have analysed 237 bed-rests of patients consulting for specific and non specific lymphangitis, chyluria, elephantiasis, hydrocele or just asymptomatic hypereosinophilia. Taking into account the clinical aspects, the parasitological and the serological results, they have shown the efficiency of IFI-WBp (titre greater than 80) to diagnose filariasis in French Polynesia where only one species of filaria is present (Wuchereria bancrofti var. pacifica.)

[An important endemic in the Marquesas: neural angiostrongylosis].

The authors report on 37 new cases of eosinophilic meningitis collected in two years in an island with a population of 1,800. Recalling first the epidemiology of the disease, they describe the most frequent clinical signs met and the results of rachiocentesis which confirm the diagnosis. The disease strikes more than 1% of the population. It might not rapidly disappear as the only efficient prophylaxis would be to deprive the population, very attached to its traditions, of one traditional dish.

[Integration of malaria control in primary health care in rural environment. General considerations].

Only 20 to 40% of the rural populations in Africa south of Sahara have the possibility to benefit from the classical health care. This situation interferes in the campaign against the main endemic diseases. Thus, in the efforts to control malaria, it appears necessary to get the population participation through the development of the primary health care services. Systematic chemotherapy of febrile attacks seems to be the easiest field strategy on a large way. The authors believe that such a strategy is possible through the development of primary health care services within the progressive decentralization of the health service network.

[Skin fibrosis in hyperthyroidism treated by sotalol and radioactive iodine (author's transl)].

The authors present detailed data about skin fibrosis appearing in hyperthyroidism treated by Sotalol and radioactive iodine. Cutaneous thickening is discovered quite rapidly when the patient is monitored daily (as in case 4). It is asymptomatic and no other features of scleroderma are found. Regression occurs within 4-10 months. Histologically, fibrosis is located in the entire dermis. Dermal appendages are normal and no inflammatory changes occur. No anomalies of collagen structure and fibroblasts have been observed ultrastructurally. Immunological studies (direct immunofluorescence of the skin, lymphocyte transformation and leucocyte migration tests with Sotalol) were normal. The mechanism is unknown, but an immunological or a toxic one is excluded; however a pharmacological action is possible. The role of other betablockers must be assessed by a randomised study.
