Follicular lymphoid hyperplasia of the hard palate is a slowly growing, soft, nontender swelling that may grow to involve the entire hard palate. The overlying mucosa is normal. This appearance naturally prompts biopsy, and both clinically and microscopically might be confused with lymphoma. Four case histories are presented with histologic description: normal palatal submucosal structures are replaced with benign reactive lymphoid tissue replete with well-developed germinal centers. Surrounding these centers are dense populations of small, regular, bland lymphocytes. Minor salivary glands, except for some atrophied residue, are notably absent; also absent are the epimyoepithelial islands characteristic of the benign lymphoepithelial lesion (Mikulicz's disease). Etiologic factors remain obscure. One of our patients had two recurrences following local excision; in another patient nodules of benign lymphoid hyperplasia developed in the cheek and upper neck. These four patients are alive and free of any malignant process 4, 7, 9, and 12 years after the onset of their palatal swellings. We urge caution in distinguishing these lesions from palatal lymphoma, and recommend local excision as the treatment of choice.

Agricultural workers acquire pesticide exposures as a function of the properties of the product, crop, and work task. Mixer/Loaders and Applicators have greater exposure potential than other workers since concentrated formulations are handled during preparation and application of spray mixes. Exposures can be estimated using passive dosimetry and biological monitoring. Measured exposures are invariably less than estimates derived from surrogate data due to a series of conservative "worst case" assumptions and exposure estimates derived from small portions of the work day. Data have also been collected to estimate the exposure potentials of harvesters. We are establishing a set of empirical transfer factors (cm2/h) that can be used to estimate daily harvester exposure (transfer factor x dislodgeable foliar residue x hours). These estimates are important to current efforts to establish exposure-based reentry intervals to protect against acute and chronic toxicity.
Corneal grafts enjoy the highest success rate of any form of organ transplantation. The low incidence of graft rejection is particularly impressive considering that human leukocyte antigen matching of donor and recipient is not normally performed. Although corticosteroids are applied topically, systemic immunosuppressive drugs are not routinely implemented for keratoplasty. The most widely accepted explanation to account for corneal allograft success suggests that the avascularity of the graft bed prevents corneal alloantigens from reaching the regional lymphoid tissues and therefore results in an "afferent blockade" of the immune response. However, recent findings suggest that the unique immunologic characteristics of the corneal graft itself may play a crucial role in determining the fate of the transplant. In particular, the presence and distribution of donor-derived Ia+ Langerhans cells can have a profound impact on graft immunogenicity and thus, graft survival even if the graft bed is initially free of lymphatic and blood vascular drainage channels. Thus, the immunologically unique characteristics of the corneal graft conspire with the avascular graft bed to produce an "immunologically privileged" environment that promotes graft survival.

We observed four cases of chemosis associated with otherwise uneventful four-lid blepharoplasty. These patients experienced postoperative conjunctival chemosis (without associated globe injection), varied types of congestion, subconjunctival hemorrhage, retrobulbar hematoma, or corneal involvement. Postoperatively, the patients were treated with some combination of patching, topical and/or systemic steroids, decongestant eye drops, and observation. Regardless of the treatment, the chemosis resolved by 5 months, without permanent complication. Probable causes were blockage of orbital or eyelid lymphatics and excessive cautery during surgery.
