Acute gastric volvulus is an infrequent entity. Management is surgical. Diagnosis frequently is delayed because of the non-specific nature of presenting symptoms. The authors report a case of acute gastric volvulus diagnosed by computed tomography in a 92 year old woman that was confirmed at surgery. The pathophysiology, classification and different presentations of this entity are briefly reviewed. CT findings suggesting gastric volvulus are reviewed as well.
Internal recognition is a primordial concept in the idiotypic network hypothesis proposed by N. Jerne. The analysis of idiotypic recurrence in rabbit antiallotypic antibodies led us to propose the existence of antiidiotypic molecules Ab2 beta carrying idiotopes which are internal images of the original immunizing allotype. Using mouse monoclonal antibodies, we have shown that the internal image of b6 rabbit allotopes is associated with the internal image of kappa 1 isotypic determinants. The influence of interactions within the idiotypic network on the evolution of the immunological repertoire is discussed.
Review of the literature shows that numerous authors admit the existence during various inflammatory reactions, and especially in granulation and cicatricial tissues, of peculiar connective tissue cells e.g. modified fibroblast or myofibroblast, with intermediate features between a fibroblast and a smooth muscle cell. Some of the physical and structural characteristics of these tissues would depend on these cells. The ultrastructural features of the cells in human and experimental pathology as well as their connection with the extracellular matrix are recalled.

Two cases of pancreatic exocrine tumors with endocrine component are reported. The first case concerned a microcystic cystadenoma including an endocrine tumor identified by Grimelius staining and positive immunostaining for neurone specific enolase and the second a tubular adenocarcinoma including cells with positive immunostaining for glucagon and neurone specific enolase. The malignant nature of the endocrine component of both tumors which was certain for the first one and probable for the second whose endocrine component was distinct from the hyperplastic endocrine reaction observed at the periphery of the tumor, seems to support the hypothesis of exocrine and endocrine differentiation from a common precursor cell. The precise value of such a sub-group of pancreatic tumors with double component need to be clarified.

Jugulo-tympanic chemodectomas are usually treated by means of surgical ablation and radiotherapy, these two techniques being used either in isolation or association. The recent advent of embolization, which has undergone considerable development in the field of vasular neuro-radiology within the last few years, open up new therapeutic possibilities for these vascular tumours. A study of three personal cases, followed up after a substantial lapse of time, gives a good idea of the importance of embolization in the therapeutic armoury. In fact, although embolization used alone can cause a reduction in functional symptomatology and even and objective clinical improvement (disappearance of some lesions of the cranial pairs, for exemple), it is regularly followed by repermeabilization of the various pediculi. Embolization used in isolation should therefore be reserved for patients who are inoperable for reasons for extension or associated deficiencies. Embolization appears to be of value before the operation as it results in less haemorrhage and may also be of value before radiotherapy.
70 fetuses (10 to 37 weeks) and 30 full term infants were examined by ultrasound (U.S.). Anatomic correlations were made with frontal, axial and sagittal sections of 43 fetal and 3 neonatal brains. At 12 weeks gestational age (w.g.a.) only the inter hemispheric fissure is seen by U.S. The sylvian fissure (21 w.g.a.), the callosal sulcus (21 w.g.a.), the parieto-occipital sulcus (25 w.g.a.), the calcarine fissure (25 w.g.a.), the cingulate sulcus (26 w.g.a.) and the collateral sulcus (25-27 w.g.a.) are visualized rather late with in-utero U.S. The other sulci are more difficult to see. The peripheral location of these sulci contributes to the difficulty encountered in their visualization. The morphology of the sylvian fissure is quite characteristic on U.S. imaging and can be used to estimate the gestational age of the fetus. Compared to the embryological development there is a 2-4 weeks delay between the first infolding of the brain and the visualization of a sulcus by U.S. Abnormal sulcal patterns can be recognized based on the normal appearance for each gestational age. Sulcal anomalies are quite specific in holoprosencephaly, lissencephaly, micropolygyry, schizencephaly, agenesis of the corpus callosum. Silhouetting of the sulci may occur if the parenchymal echogenicity is sufficiently increased that the sulci no longer stand out (ischemia, tumors, encephalitis). Thickening of the sulci occurs in subdural hematomas, external hydrocephalus, meningitis and toxoplasmosis.
