Massive thyroxine intoxication: evaluation of plasma extraction.

Six patients were admitted after erroneous massive intake of levothyroxine (70-1200 mg over an interval of 2-12 days). All patients developed classical symptoms of thyrotoxicosis within 3 days of the first dose; five patients presented grade II-III coma and one became stuporous (days 7-10). Two patients developed left ventricular failure and three had arrhythmias (days 8-11). Total thyroid hormone levels in serum on admission ranged 935-7728 nmol/l for T4 (TT4) and 23-399 nmol/l for T3 (TT3). All patients received treatment with hydrocortisone and Propranolol. Propylthiouracil was also given in 3 cases. Extractive techniques (charcoal haemoperfusion and/or plasmapheresis) were initiated 8-14 days after the first dose of L-T4. The plasma disappearance rate (K) of TT4 with plasmapheresis was 30 times higher, on average, than under standard medical treatment (M). Also, K of TT4 under haemoperfusion was about five times higher than K under M. K changes for TT3 were higher under haemoperfusion than under plasmapheresis. Furthermore, extractive procedures shortened the average half life of TT4, (from 106.5 +/- 44.6 to 59.7 +/- 20.2 h, p less than 0.05).

Phosphorus supplementation of Nellore steers fed Yaragua/Stylo mixture in the Peruvian tropics.

Twenty-four yearling 11-month old Nellore steers initially weighing 180 kg were assigned to three treatments in a randomised complete block experiment lasting 392 days. Animals grazed continuously either Yaragua grass (Hyparrhenia rufa) or Yaragua/Stylo legume (Stylosanthes guianensis) and received the following treatments: A) Control, Yaragua plus common salt, B) Yaragua/Stylo plus common salt, and C) Yaragua/Stylo plus dicalcium phosphate/common salt (1:1). Mineral consumption in the three groups averaged 52 g per animal day-1. At 13 months daily liveweight gains/steer were 0.182, 0.409 and 0.483 kg for treatments of Yaragua, Yaragua/Stylo, and Yaragua/Stylo plus dicalcium phosphate/common salt mixture. Daily liveweight gains per ha were 0.488, 1.289, and 1.522 kg steer-1 for Yaragua, Yaragua/Stylo plus common salt and Yaragua/Stylo plus dicalcium phosphate/common salt. Steers receiving a legume (stylo) in the diet more than doubled weight gains (P less than 0.05) compared to controls with dicalcium phosphate further (P less than 0.05) increasing these gains.

En bloc kidney and bladder transplantation from an anencephalic donor into an adult recipient.

The visceral block, composed of both kidneys and their vascular pedicles, inferior vena cava, ureters and bladder, was removed from a 3-day-old male anencephalic donor and transplanted into a 32-year-old adult in chronic renal failure. The urinary tract was reconstructed by vesicovesicostomy for technical ease, and prevention of reflux and ureteral stenosis. Maximum urine output was 150 ml. per day and the kidney grafts were lost owing to rejection 5 months after transplantation.

Treatment of metastatic neck nodes secondary to an occult epidermoid carcinoma of the head and neck.

One hundred thirty-eight patients, who were followed for a minimum of 5 years, had either surgery and postoperative radiotherapy (48 patients) or radiotherapy only (90 patients) for metastatic epidermoid carcinoma in cervical nodes from an unknown head and neck primary. All received radiotherapy to the presumed occult sites. Forty-five percent presented with a single unilateral adenopathy. Those who were initially operable had a neck recurrence rate of 17% and a survival rate of 53%. Forty-three percent of initially inoperable patients recurred and only 25% survived. Patients with adenopathy which completely regressed or became resectable after irradiation had an 80% locoregional control. Only 4% developed an overt cancer at an occult site within 5 years.

Use of monoclonal antibodies for the differential detection of Trypanosoma cruzi and T. rangeli in epidemiological studies and xenodiagnosis.

T. cruzi and T. rangeli have the same insect and mammalian hosts, including man, and in addition share approximately half the antigenic determinants recognised by the humoral response. Thus serodiagnosis of T. cruzi infection in areas where T. rangeli is endemic may include an unknown rate of false positives due to this antigenic cross-reactivity. Similarly, the results of xenodiagnostic procedures and epidemiological surveys of insect vectors are prone to distortion because of the close morphological resemblance of the epimastigote stages. The description of a T. cruzi epimastigote specific monoclonal antibody, 2A2, which reacts with both culture and insect derived epimastigotes provides a more reliable basis for differential diagnosis of these two parasites.

[Cancers of the soft palate. Results of repeat surgery].

Between 1960 and 1983, 270 patients were treated at the CURIE Institute for a carcinoma of the soft palate, including 260 by radiotherapy (mean dose: 69 Gy). Most of these tumours were T1 T2 (201 cases, 76%). There was a high frequency of a past history of carcinoma (38 cases, 14%) as well as of simultaneous carcinomas (44 cases, 16%). Two months after radiotherapy, a velar tumor persisted in 29 patients whilst 1 patient had deep necrosis. 15 of these 30 patients underwent secondary surgical excision. This surgical series included 25 of 27 recurrences seen secondarily and 6 patients operated on for second velar carcinoma appearing in the irradiated zone (mean dose: 68 Gy). 45 of the 46 excisions were carried out by a buccal approach. Overall survival after surgery was 32% at 3 years. The 11 patients operated on for an extensive T3 T4 tumour died before 12 months. These poor results were related to the frequency of local recurrences (12 loco-regional failures amongst 36 deaths) as well as the number of deaths due to second carcinomas (11 deaths amongst the surgical patients). Later carcinomas were particularly frequent during the course of tumours of this site (69 cases out of 270), the problem of multiple malignant tumours finally affecting 122 patients out of 270, i.e. 45%.

[Results of operations of salvage laryngectomy].

The surgery of the larynx in an irradiated neck changed since 30 years by an evolution of the radiotherapeutic technics and the surgical procedures. At the Curie Institute, from 1960 to 1986, 230 patients were operated with laryngectomy for failure of the first treatment (average doses: 70 Gy). 29 had partial laryngectomy. The feeding tube was removed before 6 weeks in 78% of the patients. 19 patients (8%) died in the 3 months after surgery (8 carotid blow-up). The crude survival rates are 70% at 1 yr, 42% at 3 tr, 37% at 5 yr. These rates are identical in patients with persistent disease two months after irradiation (90), in patients with recurrence (110) and for those with second primary in the larynx (21). 54% of T1 and T2 patients are NED at 3 yr and 47% at 5 yr. For T3 and T4 rates are only 22% and 17%. Best results are in glottic cancers (74% at 3 yr); poorest in advanced supra-glottic (17%). Salvage procedure is effective in larynx and neck in 62% of the patients. Early detection of a failure after irradiation is a major condition for the application of a protocol of definitive radiotherapy in laryngeal cancer.

[Cancers of the hypopharynx. Course of surgical salvage].

Recovery surgery on irradiated hypopharyngeal zones, and the indications for this operation, have been transformed by currently available possibilities for pharyngeal reconstruction and protection of carotid axis by musculocutaneous pedicle flaps. Of 117 patients operated upon in the Curie Institute between 1960 and 1986 for hypopharyngeal cancer, after radiotherapy (mean dose 70 Gy), deglutition was restored in 100 cases (85%) after a mean healing period of 30 days. Fatal postoperative complications occurred in 23 patients (19%), including 12 carotid hemorrhages, but since 1980 the percentage of mortal bleeding has dropped from 15 to 5%. Postoperative survival was 50% at 1 year, 29% at 3 years and 15% at 5 years, results being similar whatever the indication (46 patients operated upon at end of radiotherapy, 36 local recurrences and 37 second cancers in irradiated zone). Local and regional recovery surgery was effective in only one patient out of two. The musculocutaneous flap allows wider and more certain surgery and improved functional results, but in no way affects the particularly serious nature of cancer of this localization.

Cross-reactivity among five major pollen allergens.

We have studied the cross-reactivity between L. perenne, S. cereale, P. pratense, C. dactylon, and Ph. communis. The results obtained demonstrate that L. perenne, S. cereale, and P. pratense have strong cross-reactivity. C. dactylon has a number of allergens that cross-react with the rest of the grasses studied but they are minor allergens. Ph. communis possesses a moderate cross-reactivity with the species of the group but it has, as well, individual allergens.

Albendazole therapy for neurocysticercosis.

Seven patients with chronic parenchymal brain cysticercosis were treated with albendazole for one month at daily doses of 15 mg/kg of body weight. Computed tomographic follow-up studies were made at the following times: a mean of 16 months before the trial; the beginning of treatment; the end of treatment; and three months afterward. Computed tomographic studies at the beginning of treatment showed a total of 157 cysts, the last day of treatment the number had decreased to 39 cysts, and three months later, the total number of lesions was 22; this represents 86% improvement in the number of cystic lesions. Control studies in the same group of patients had shown chronic persistence of lesions without spontaneous improvement in most cases. Two patients had been treated with praziquantel previously with partial response; in both cases albendazole therapy was effective with 100% and 77% improvement. We conclude that albendazole is highly effective for the treatment of parenchymal brain cysticercosis. It is also effective in patients who had shown poor therapeutic response to praziquantel.

Differential effects of tobacco and alcohol in cancer of the larynx, pharynx, and mouth.

Two thousand five hundred forty male patients with cancer of larynx, pharynx, and mouth were examined in the same hospital between 1975 and 1982. Different sites were compared according to alcohol and tobacco consumption: oropharynx, hypopharynx, larynx supraglottis, larynxglottis, epilarynx, lip, tongue-tip, and tongue lateral border, other tongue sites, gum, floor of the mouth, and buccal mucosa. For tobacco, the mean daily consumption of smokers and exsmokers did not differ according to location (except in patients with cancer of the lip, in whom the consumption was lowest). Cancer locations differed significantly according to the daily alcohol consumption. The consumption was the lowest for patients with cancer of the lips and the glottis, the highest for patients with cancer of the epilarynx, hypopharynx, and the floor of the mouth. Significant differences were observed in the percentage of nonsmokers or nondrinkers, even between adjacent locations: supraglottis versus glottis; tongue, tip, and lateral border versus tongue, other sites; floor of the mouth versus buccal mucosa. Some of the results might be particular to elements of the French life-style (e.g., consumption of brown tobacco, high consumption of wine).

Role of heterochromatin during preferential 9q;22q translocation in chronic myelogenous leukemia.

The secondary constriction region (h) of human chromosome 9 was evaluated in 55 chronic myelogenous leukemia (CML) patients with respect to its size and position. Each case was examined by C-banding and distamycin A-4,6-diamidino-2-phenylindole techniques for the expression of the h regions. When one h region of chromosome 9 was larger, it was more frequently involved in the reciprocal translocation with chromosome 22. In addition, there was a higher incidence of pericentric inversions in the h regions in the translocated chromosome 9 when compared with normal homologues. The role of the constitutive heterochromatin of chromosome 9 as a possible influencing factor during 9q;22q translocation in CML is suggested.
