Therapeutic doses of cyproheptadine do not inhibit monoamine oxidase in man.

Cypro, a serotonin and histamine antagonist, has been shown to be a moderately potent reversible inhibitor of tissue monoamine oxidase (MAO) obtained from hamsters and rabbits. In the present study, Cypro inhibits MAO obtained from human platelets with the same potency as harmine (Ki = 5 x 10(-5) M). However, when ten normal volunteers received conventional therapeutic doses of Cypro (16 mg/day) for two days, there was no alteration in their urinary excretion of tryptamine, 5-hydroxyindoleacetic acid, 3-methoxy-4-hydroxymandelic acid, epinephrine, or norepinephrine. We conclude that, when used in conventional clinical doses, Cypro does not inhibit MAO in man.

Relaxivity of Gd-EOB-DTPA in the normal and biliary obstructed guinea pig.

After injection of Gd-EOB-DTPA, T1 and T2 were determined on a clinical MR scanner (1.5 T) in the liver and kidneys of sacrificed but intact guinea pigs with normal and obstructed biliary systems and in bile, urine, and blood collected postmortem. Tissue [Gd] was determined by radioassay of 153Gd and relaxivities (R1 and R2; units of s(-1) x mmol(-1) x kg) of Gd-EOB-DTPA calculated. Compared with R1 in 2% agarose gel (4.49 +/- 0.03), in normal animals R1 was increased in liver (9.3 +/- 0.5), similar in kidney cortex (4.1 +/- 0.5), but reduced in kidney medulla (2.5 +/- 0.4) and papilla (2.7 +/- 0.4). Chronic biliary obstruction did not change R1 in liver (9.7 +/- 4.3) but reduced R1 further in kidney tissues (1.0-0.4). In normal animals, R2 values of all tissues (9.5-18.4) were greater than R2 in gel (5.72 +/- 0.12). Biliary obstruction possibly elevated R2 in liver (40.1 +/- 63.5), severely depressed R2 in kidney cortex (-4.2 +/- 6.2) and medulla (-2.3 +/- 5.4), and reduced R2 in papilla (5.4 +/- 4.6). Obstruction had little effect on R1 and R2 in bile and urine. Water content, macromolecular binding, microviscosity, compartmentalization, and susceptibility effects can readily account for the R1 and R2 observed in liver and kidney. Negative R2 could be a result of several factors, including reduced endogenous magnetic field gradients due to "susceptibility matching" as [Gd] increased, changes in tissue T2 with period of ligation, or a physiological effect of EOB-DTPA. These results show that disease can alter both R1 and R2 from their values in normal tissues.

[Computerized management of anticancer chemotherapy].

Control of anti-cancer chemotherapeutic management is facilitated by the use of micro-computers. The authors have devised a program in which are entered, as separate records, all data required concerning the patient, the drugs administered, the therapeutic regimens and the controls to be applied during treatment. The computer commands appointments, examinations to be performed, prescriptions and protocols, making all the calculations needed and taking into account those data which restrict the use of drugs. At the end of each course, each record is updated by entering control parameters and new events, so that a detailed history of all the treatments received by all patients is obtained. This last record can be used for statistical purposes and to test compliance with, and response to treatment. The program, produced and distributed by a data-processing service firm, can be used with various micro-computers. It is available to medical groups interested in the treatment of cancer.

Association of histone H1(0) with a gene repressed during liver development.

Histone H1(0) has a number of unusual properties that set it apart from other H1 subtypes (for review see ref. 1). For example, H1(0) synthesis is not strictly coupled to DNA synthesis, it is absent from the embryonic liver of mice (but present shortly after birth) and its synthesis is hormone-dependent in some of the glands of adult rodents. All the H1 subtypes differ in their DNA binding properties, and H1(0) has been shown to be preferentially associated with nuclease-resistant chromatin. These features suggest that the H1(0) may have a role in developmental gene control. To investigate this further, we have fractionated the H1(0)-containing nucleosomes of chromatin from adult mouse liver. We report here that the gene for alpha-fetoprotein, which is expressed in embryonic tissue but repressed soon after birth, is preferentially associated with the H1(0)-containing nucleosomes. The related gene for albumin, which is expressed in both embryonic and adult tissues, is absent from the H1(0)-containing nucleosome fraction. These results support a role for histone H1(0) in the control of gene expression.

Circular countercurrent-distribution behavioral genetic maze.

An inexpensive, easily constructed apparatus for mass screening of taxes of small animals with a rapid and accurate assignment of individual behavior scores has been described that should be of utility in hastening a behavioral geneticist's chores and allow him to handle more populations with fewer mazes. Also, the apparatus is well suited for use within the constraints of the undergraduate laboratory.

The future of home and hospice care. Linking interventions to outcomes.

The pressure on home care and hospice nurses to reach successful outcomes in a cost-effective manner mandates that they become well versed in how to analyze data. The goal of outcome research is to relate patient outcomes to the interventions provided. This article discusses the challenges of outcomes research and how it differs from and relates to performance improvement, and it explores solutions to common problems.

The story: a primary spiritual tool. Stories can be a healing power in the dying process.

Recounting the events of one's life is therapeutic, particularly if the storyteller has the opportunity to share feelings, as well as factual content. Inherent in each person's story is the key to healing. Sensitive listening and dialogue can help dying persons discover connections between their own stories and those in the Scripture. In addition, when a person is in denial regarding a life-threatening illness, dreams can often provide a means to unlocking the unconscious. As Jesus used images from nature and symbols from everyday life, so, too, can care givers use analogies to get to the heart of the matter. Another source of healing is the original story, which can help a patient and family see their situation in a new light. Humor, used appropriately, can be a redemptive gift. Finally, when unfinished business has been completed and issues of forgiveness resolved, there is little left but to celebrate the life of the dying person. The story can thus be therapeutic, diagnostic, a source of healing, a means of fostering reconciliation, and an occasion for celebration.

[Epidemiology of human trypanosomiasis in the Luba focus, in Equatorial Guinea].

In Equatorial Guinea, the human trypanosomiasis, after an intensive surveillance beginning in the 30's, seemed to be controlled by the end of the 60's. A lack of surveillance provoked a surge of different foci, confirmed in 1982 by the spontaneous presentations of sick in the hospitals of the old foci. These processes have been observed in the Luba focus (ex San Carlos) on the island of Bioko (ex Fernando Poo). The situation was grave in 1985 and a survey was organized by the "Centro de Control de la Tripanosomiasis" (Ministerio de Sanidad de Guinea Ecuatorial--Agencia Española de Cooperación Internacional) to delimit the focus, to evaluate the presence of the disease in the different villages, to know the characteristics and the distribution of the vector, and evaluate the impact of the mono-pyramidal trap as the method of anti-vectorial control. The results have been obtained and presented in the conclusion.
