Non-invasive assessment of the Ionescu-Shiley pericardial xenograft heart valve.

This study was undertaken to describe the auscultatory, phonocardiographic, M-mode and two-dimensional echocardiographic features of the Ionescu-Shiley prosthesis in a group of unselected subjects. Twenty-seven valves were studied - 15 aortic, 10 mitral and 2 tricuspid. An opening sound was noted by phonocardiography and auscultation from the majority of valves in the three positions. A mid-systolic murmur was heard from 14 aortic valves and two had regurgitant murmurs. Three mitral valves had apical diastolic murmurs and two of these had mitral regurgitation. Two-dimensional echocardiography provided important spatial information but M-mode allowed superior resolution of valve leaflets. Calcification was seen in one four year old aortic valve and dehiscence of the sewing ring in another leading to severe aortic regurgitation. These data indicate that an opening sound is a frequent normal feature of an Ionescu-Shiley prosthetic valve and a mid systolic murmur is normal for aortic valves. In the three valve positions, the normal M-mode pattern resembles a native aortic valve. Abnormal features which can be recognized include calcification of valve cusps and dehiscence of the sewing ring.

A Western Australian Delphi survey of staff development research priorities.

A modified Delphi survey conducted during 1989-1990 identifies problems that Western Australian nurses consider research priorities. Participants were employed in hospital and health-care agencies throughout metropolitan and rural regions of the state. The large number of topics generated were analyzed and categorized into three areas of nursing: staff development, clinical practice, and management. Participants rated each topic on its importance to nurses and patients. The results are based on the 10 priority items identified in the staff development field.

Pediatric pain assessment: validation of a multidimensional tool.

Prior to introducing a locally developed pediatric pain assessment tool into the ward setting, testing was done to establish its content and construct validity. Results indicated that the PMHPAT is a valid measure of pain for children between 7 and 14 years old with two limitations: (a) it is unsuitable for assessing spasmodic pain, and (b) some children are unable to distinguish between "hurting" and "feeling."

Use of in-line filters in pediatric intravenous therapy.

This study arose from a need to justify the extensive use of in-line intravenous filters in an Australian pediatric teaching hospital. The incidence of septicemia was observed for two 12-month periods before and after the withdrawal of in-line i.v. filters from patients with central venous access devices. A total of 19,221 i.v. days were monitored in 88 children in two 1-year periods. No significant differences were found in the incidence of septicemia between the children who had filters fitted and those who did not. As a result, the routine fitting of filters to i.v. lines infusing fluids filtered during manufacture under a laminar flow has been discontinued with significant cost savings.
