Prevalence and incidence of hepatitis C virus infection in the US military: a seroepidemiologic survey of 21,000 troops.

Because of a high prevalence of hepatitis C virus (HCV) infection (10-20%) among veterans seeking care in Department of Veterans Affairs (VA) hospitals, current US military forces were evaluated for HCV infection. Banked serum samples were randomly selected from military personnel serving in 1997 and were tested for antibody to HCV (anti-HCV). Overall prevalence of anti-HCV among 10,000 active-duty personnel was 0.48% (5/1,000 troops); prevalence increased with age from 0.1% among military recruits and active-duty personnel aged <30 years to 3.0% among troops aged >/=40 years. Prevalence among 2,000 Reservists and active-duty troops was similar. Based on sequential serum samples from 7,368 active-duty personnel (34,020 person-years of observation), annual incidence of infection was 2/10,000. Of 81 HCV RNA-positive troops for whom genotype was determined, genotypes 1a (63%) and 1b (22%) predominated, as in the civilian population. These data indicate that HCV infection risk among current military forces is lower than in VA studies and the general civilian population aged <40 years. The low level of HCV infection may be attributed to infrequent injection drug use in the military due to mandatory testing for illicit drugs prior to induction and throughout military service.

Endemic infectious diseases and biological warfare during the Gulf War: a decade of analysis and final concerns.

Infectious diseases were one of the first health threats confronted by Coalition troops deployed to the Arabian desert in August 1990. On the basis of experiences in World War II, the major endemic infectious disease risks were thought to be sandfly fever, cutaneous leishmaniasis, diarrheal disease, and malaria. Although there was active surveillance, no case of sandfly fever and few other endemic infectious diseases were identified among over 500,000 U.S., British, and Canadian ground troops. In addition, there was no diagnosis of biological warfare (BW) exposure, and BW agents were not detected in clinical, environmental, or veterinary samples. The most common infectious disease problems were those associated with crowding (acute upper respiratory infections) and reduced levels of sanitation (travelers-type diarrhea). Only one endemic infectious disease has been confirmed as causing chronic health problems: visceral Leishmania tropica infection (viscerotropic leishmaniasis). However, this protozoan infection was diagnosed in only 12 U.S. veterans, and no new cases have been identified during the last 8 years. Infectious diseases were not a serious problem for Gulf War troops because of extensive preventive medicine efforts and favorable weather and geographic factors. Moreover, it is unlikely that an endemic infectious disease or a BW agent could cause chronic health problems and remain undetected over a 10-year period.

Viral hepatitis in the U.S. military: a study of hospitalization records from 1974 to 1999.

Viral hepatitis remains a health threat for military forces. Most recently, there has been concern about hepatitis C virus transmission during military service because a high prevalence of hepatitis C virus infection has been found in some U.S. veteran populations. In this study, hospitalizations of active duty U.S. military personnel for hepatitis were evaluated using standardized computer records. Only the first hospitalization was assessed during the period January 1, 1989, to December 31, 1999. Among active duty forces, the rate of hospitalization for all types of acute hepatitis declined from 13 to 1.1 per 100,000 personnel from 1989 to 1999. Males, nonwhite racial/ethnic groups, and older troops were more likely to be hospitalized for acute hepatitis. This study's finding of declining rates of acute hepatitis is a continuation of a trend observed since 1974. The decreasing risk of viral hepatitis in the U.S. military is attributable to several factors, including reduced levels of injection drug use because of routine, randomized drug testing.

Molecular/clinical correlations in females with fragile X.

Females who are affected by fragile X syndrome (FXS) can have significant physical, neuropsychological and emotional involvement. This study was designed to explore the relationships between these three domains and to learn how the degree of involvement in each of these phenotypic areas relates to molecular parameters including CGG repeat length and activation ratio (the proportion of normal FMR1 alleles on the active X chromosome). Three groups of females were studied: 35 women who grew up in a fragile X family but do not carry an FMR1 mutation, 92 women with a premutation, and 29 women with a full mutation. Correlations between neurocognitive, physical and emotional traits were calculated for each of the three groups. Within the full mutation group significant correlations were seen between schizotypal traits and full scale IQ. The Lie scale was significantly correlated with the physical findings index. The activation ratio correlated significantly with the measure of executive function (r = .50, P = .01). There was a trend toward correlations of activation ratio with the physical index score, outer ear prominence and IQ. CGG repeat number significantly correlated only with the physical index (r = .44, P = .01). Thus, activation ratio may be the more pertinent molecular parameter in full mutation women in determining the degree of cognitive and physical phenotypic involvement.

Molecular-clinical correlations in children and adults with fragile X syndrome.

INTRODUCTION: Fragile X syndrome is the most commonly known inherited form of mental retardation. The intellectual abilities range from a normal IQ with learning disabilities to severe mental retardation. In males, there is a tendency for IQ decline in childhood. The purpose of this study was to correlate variations of the molecular cytosine guanine guanine (CGG) amplification in the fragile X mental retardation-1 (FMR-1) gene with the clinical findings, including IQ and physical features. METHODS: Full-scale IQ and cytogenetic results in 116 individuals with the FMR-1 mutation were studied. The IQ testing was performed with age-appropriate standardized tests. Physical features were summarized in a physical index score for each patient. The FMR-1 results were determined with the OX1.9 probe and the following system was used: P1 indicates premutation; P2, large premutation to small full mutation; P3, full mutation; and P4, mosaic. RESULTS/CONCLUSIONS: The findings showed that those females with a small insert in the P1 range had a significantly higher IQ than other heterozygotes (P2, P3, and P4 categories). P4 males had a significantly higher IQ than P2 or P3 males. In cross-sectional age comparisons, the slope of the IQ decline was greater in P2 males than in P4 or P3 males.

Blood and urine responses to ingesting fluids of various salt and glucose concentrations.

Several hours before returning to Earth, Space Shuttle astronauts consume fluid and salt tablets equivalent to a liter of 0.9% saline as a countermeasure to postflight orthostatic intolerance. This countermeasure is not completely successful. Therefore, in search of a countermeasure that would protect against orthostatic intolerance better and for a longer duration, the authors compared the blood and urine responses of five men (21-41 yr) after they drank 1 L of 0.9% saline to their responses after drinking five other solutions: distilled water, 1% glucose, 0.74% saline with 1% glucose, 0.9% saline with 1% glucose, and 1.07% saline. Each subject ingested a different solution on 6 different days and remained seated for the ensuing 4 hours. Heart rate, blood pressures, and urine variables were measured before ingestion of the fluids and every 30 minutes thereafter; blood samples were drawn before, immediately after, and every 60 minutes after ingestion. Change in plasma volume, which was estimated from hemoglobin and hematocrit, was considered the most critical variable. Data for all solutions were compared by analysis of variance. Since plasma volume was increased most after ingestion of 1.07% saline, all variables (at 2 hours, at 3 hours and at 4 hours) were compared between 1.07% saline and 0.9% saline, the current countermeasure. Plasma volume was increased more after 1.07% saline than after 0.9% saline, and this difference was most significant at 4 hours after ingestion (P = .056). Diuresis occurred promptly after ingestion of the two saline-free solutions, water and 1% glucose.(ABSTRACT TRUNCATED AT 250 WORDS)

Platelet function and structure in myeloproliferative disease, myelodysplastic syndrome, and secondary thrombocytosis.

Platelet function and morphologic characteristics were evaluated in 43 patients with myeloproliferative disease (MPD), 5 patients with myelodysplastic syndrome (MDS), and 7 patients with secondary thrombocytosis (ST). Platelet Factor IV (PF4) and B-thromboglobulin (BTG) showed slight elevation in ST but significant elevation in all MPDs. They were either normal or slightly elevated in MDS. Defective platelet aggregation with one or more inducers was seen in 62% of all patients. An epinephrine-induced defect was the most consistent aggregation abnormality. Hyperaggregation and spontaneous aggregation were seen in 15% of patients. Of the eight patients who showed increased bleeding tendency, all eight showed defective aggregation with two or more inducers, five showed decreased surface activation response, as well as decreased or abnormal granules and dense tubular disarray in the transmission electron microscope (TEM) study. Seven patients had clinical evidence of recurrent arterial and venous thromboses. Five of these patients showed hyperaggregation response to adenosine diphosphate and collagen and abnormal Wu and Hoak platelet aggregate ratio. Four patients showed spontaneous aggregation on aggregometer. Surface activation response was significantly increased in five patients, and an increase in platelet granules by TEM study was seen in four patients. Primary thrombocythemia could be differentiated from secondary thrombocytosis (ST) by the presence of abnormal aggregation response and significantly increased PF4 and BTG in the former, and greatly elevated plasma fibrinogen and Factor VIII, as part of acute phase reactant response, in the latter.

[Asymmetrical changes in electrical activity of Purkinje fibers and muscle cells after administration of BAY K 8644. A new model of cardiac arrhythmia].

The effects of calcium current agonist BAY K 8644 on transmembrane potential of guinea pig papillary muscle (PM) and sheep Purkinje fibers (PF) were studied. The action potentials (AP) and contractions (C) were recorded. C both of PM and PF increased (two-four times) in response to BAY K 8644 (0.5-5 microM) addition. The AP duration of PM increased slightly (20%) while PF AP duration increased progressively with the time even at low BAY K 8644 dose (0.5 microM). At 5th min, early afterdepolarizations (EAD) occurred. After 10 min drug application PF became inexcitable at high plateau level. TTX and ethmozin (1 microM) restored AP. Thus, changes in ionic currents balance in PF towards the increase in inward calcium current by BAY K 8644 resulted in additional steady state potential at plateau level. The latter might induce EAD and re-entry around functionally inexcitable PF. The similarity of mechanisms of arrhythmias induced by ischemia with the presented model is discussed.

Incidence of hepatitis A virus (HAV) infection in rural Liberia.

To provide background for future hepatitis A vaccine trials, sera were collected from 0- to 4-year-old Liberian infants and their mothers on two occasions an average of 14.75 months apart and tested for antibody to hepatitis A virus (anti-HAV). The prevalence of anti-HAV rose from 2.5% in infants 0-6 months of age to 70% in children 3-4 years of age and did not differ between male and female infants. The annual incidence of new infections was slightly lower in the first year of life (35%) than in the subsequent 3 years, when it averaged 45%. The presence of HBV infection did not affect the incidence of HAV seroconversion. No clinical hepatitis was recognized in the subjects who seroconverted. Dual hepatitis A and B virus infection were observed; these were all clinically inapparent. The extraordinary incidence of HAV infection documented in the present study offers an opportunity for vaccine efficacy trials requiring minimal numbers of subjects.

Epidemiology of hepatitis B infection in Liberian infants.

To provide background for a hepatitis B vaccine efficacy trial, sera were collected from 0- to 4-year-old Liberian infants and their mothers, on two occasions an average of 14.75 months apart, and tested for serological markers of hepatitis B virus infection. The prevalence of the hepatitis B surface antigen (HBsAg) was 2.9% in the 0- to 6-month age group and 23% in infants 3 to 4 years of age. HBsAg persisted for the 14.75-month average follow-up period in 80.8% of the infants tested. The annual incidence of development of HBsAg was 18.9% for infants less than 1 year of age and 13.6% in infants 3 to 4 years of age. Infants born to HBsAg carrier mothers had significantly higher age-specific prevalence and incidence of hepatitis B virus infection. However, it was estimated that only a minor proportion of hepatitis B infections in Liberia are derived by vertical transmission from carrier mothers.

The "spindle-shaped" body in fibroblasts: intracellular collagen fibrils.

The "spindle-shaped" or "fusiform" bodies of fibroblasts are membrane-bound collagen fibrils. These cellular organelles, also described in the literature as rods or cigars, can be induced massively in chick embryo fibroblasts by treatment with colchicine in vinblastine. These alkaloids also induce massively the presence of non-striated fibrils, segment long-spacing (SLS) and fibrous long-spacing (FLS) crystallites in cytoplasmic vacuoles. The intracellular fibrillar structures described within the cytoplasmic vacuoles suggest the occurrence of tropocollagen interconversions, indication that under the experimental conditions used in this work some procollagen is processed intracellularly. Our findings may represent the first step of autophagocytosis of collagenous products when the secretory process is blocked. It is possible that these agents, by blocking secretion, magnify a normal process, and that under normal conditions a population of procollagen molecules may be processed intracellularly, thus contributing to the process of fibrillogenesis.

Intracellular processing of procollagen induced by the action of colchicine.

Corium fragments incubated in vitro with colchicine or obtained from chick embryos 11 days old after in vivo injection of this alkaloid were studied with the electron microscope. The well known effect of colchicine in producing an accumulation of procollagen was demonstrated by electrophoretic separation of the collagenous products in polyacrylamide gels after incubation of corium fragments in vitro with labelled amino acids. Electron microscopical studies showed various types of cytoplasmic vacuoles containing collagenous fibrillar structures. Some of these vacuoles contained non-striated filaments, fibres with the periodicity of native collagen, and segment long spacing (SLS) crystallites, embedded in an electrondense substance. The central aspect of this communication is the finding of intracellular SLS in cytoplasmic vacuoles. These crystallites were morphologically intact, and had no additional bands at the NH2 or COOH ends, suggesting that they were formed by the aggregation of tropocollagen molecules. Since the presence of intact SLS in cytoplasmic vacuoles suggests that these are secretory, we believe that inhibition of the transcellular transport system by colchicine permitted the visualization of vacuoles containing the products of tropocollagen interconversions. We interpret these findings as an example of autophagocytosis, with intracellular processing of procollagen, occurring in chick embryo fibroblasts when the secretion of procollagen is inhibited by colchicine.

Fat embolism prophylaxis: a study of four treatment modalities.

Significant coagulation and blood gas changes may occur with uncomplicated extremity fractures. To more accurately define the effect of therapy on these changes, a prospective study of 58 patients with uncomplicated fractures was undertaken. An initial group of 10 selected patients was studied to determine the changes with "standard therapy." The remaining 48 patients were then randomized into five groups: control, increased fluid intake, increased glucose intake, aspirin, and massive steroids. The patients were studied on 5 consecutive days. Treatment of patients in the latter four groups with aspirin or steroids resulted in significant normalization of blood gases, coagulation proteins, and platelet numbers when compared to controls. These measurements in patients treated with fluid loading or increased glucose intake were not significantly different from controls.
