Changing patterns of breast cancer.

Changes in treatment of breast cancer are high-lighted as follows: 1) Outpatient breast biopsy offers advantages in planning treatment and also saves money, 2) Mammograms are increasing the numbers of breast tumors found early, 3) For many reasons modified mastectomy is finding increasing use, 4) Breast cancer appears to be multifocal at least one-half of the time. Multifocal breast cancer is more aggressive, 5) Bilateral breast cancer appears to have a very favorable prognosis, especially when non-simultaneous.

Evolutionary changes in the electrocardiogram of severe progressive hypothermia.

A patient is reported who developed progressive hypothermia during therapy for adult respiratory distress syndrome. Electrocardiographic changes (sinus bradycardia, prolonged PR interval, prolonged QTc interval, "Osborn waves") were documented and correlated with body temperature. The significance of these changes is discussed and the relationship between the degree of hypothermia and the presence of "Osborn waves" is noted.

The structure-action relationship and kinetics of some naloxone and naltrexone derivatives.

The isolated longitudinal muscle preparation (with attached Auerbach plexus) of the guinea pig ileum was used to investigate the structure activity relationship and kinetics of some naloxone and naltrexone derivatives and that of cyclazocine. The agonist used for the investigation of the antagonistic effect of these compounds was 6-azidomorphine (AM). AM was found to be an about 20 times more potent agonist than morphine. In contrast to cyclazocine, which also was found to be approximately 15 times more potent agonist than morphine, naloxone had no demonstrable agonistic activity and naltrexone and the various naloxone and naltrexone derivatives had only insignificant agonistic activity with ED50/Ke ratios ranging from 2,000 to about 120,000. All compounds tested were competitive reversible antagonists of AM. 6-Methylene substitution caused an approximate 50 and 100% increase of the antagonistic activity of naloxone and naltrexone, respectively, and decreased the duration of action of naloxone. 3-Acetate or 3-nicotinate substitution decreased potency and had no effect on the duration of naloxone action. There is a correlation between tachyphylaxis observed on the inhibition of longitudinal muscle contraction and antagonist activity of narcotic agonists.

Serial coronary arteriography with 105mm photofluorography: 24-month experience, 2,200 patients.

A 105 mm camera has been used for over 24 mo in evaluation of the coronary circulation for the most part and occasionally the proximal peripheral arteries in 2,200 patients. This photofluorographic technique provides sufficient diagnostic image detail with improved operating convenience and reduced radiation exposure compared to the large film changer. 105 mm serialography is a good adjunct to cine arteriography as a complementary film recording system, and in many cases may supplement large film serialography.

Rotation of spin-cast hydrogel lenses.

This study was designed to determine if spin-cast hydrogel contact lenses rotate on the eye, and if so, to investigate the parameters which influence rotation. If these lenses do rotate, correction of residual astigmatism may be difficult, if not impossible. We monitored the lens rotation associated with 12 specially marked B and L Soflens TM contact lenses on six corneas after insertion and 30 minutes later by use of high speed motion pictures. To analyze lens rotation, the film was projected with a stop action projector and the amount of rotation measured with a protractor. Thirty-one percent of the 144 observations made showed some lens rotation, the majority of which was encyclorotation. Most of the lenses showed little or no change in rotation between the two time periods. Corneal curvature was the only lens or eye parameter associated with rotation. Why lenses were more likely to rotate on steeper corneas is not fully understood. Our findings indicate that Softlenses TM may rotate, but there is no definite way of determining in advance if a given lens will rotate on a given eye. This emphasizes the need for individualized lens fitting and proper patient monitoring.

Pulmonary invasive aspergillosis in patients infected with the human immunodeficiency virus: report of two cases.

Patients infected with the human immunodeficiency virus (HIV) have an increased frequency of invasive aspergillosis. We report two cases of invasive pulmonary aspergillosis in patients infected with HIV. Patients had less than 10 CD4 (helper cell) cells/mm3 (2/2), presented with fever (2/2), dyspnea (1/2), and abnormal chest radiograph (2/2). Diagnosis was established by transbronchial biopsy (1/2) and autopsy (1/2). Patients died in spite of treatment with intravenous deoxycholate amphotericin B. Prognosis of invasive pulmonary aspergillosis among patients infected with HIV remains dismal, even with aggressive antifungal treatment.
