Effects of fasting on neuroendocrine function and follicle development in lean women.

A 72-h fast in normal weight women during the follicular phase results in transient alterations in neuroendocrine function, but follicle development and follicular phase length remain unaltered. In this study we evaluated neuroendocrine and ovulatory function in lean women (body fat, < or = 20%) undergoing a similar 72-h fast. Compared to fed controls, fasted lean women experienced significant weight loss, blunting of the diurnal variation of cortisol, suppression of the nocturnal TSH rise, and a decrease in T3 levels after a 72-h fast. In contrast to similarly fasted, normal weight women, lean women have significantly higher evening cortisol levels and do not exhibit a normal nocturnal TSH rise after the fast. Lean fasted women exhibited a 19% decrease in the number of LH pulses over 24 h compared to fed women (12.9 +/- 1.3 vs. 16.0 +/- 1.9; P < 0.05). Fasting did not result in significant differences in mean LH, LH amplitude, LH area under the curve, and mean FSH levels in these lean women. Of the seven fasted cycles, two were anovulatory. In the five women studied in fed and fasted cycles, one had interrupted lead follicle development with anovulation, and four had significant lengthening of the follicular phase compared to those during their fed cycles (14.4 +/- 1.2 vs. 13.2 +/- 1.0 days; P = 0.01). The clinical observations made in this small sample of lean women showing more profound changes in neuroendocrine function, anovulation, and lengthened follicular phase after fasting suggest that lean women may be more vulnerable to fasting stress than normal weight women.

A family-based analysis of whether the functional promoter alleles of the serotonin transporter gene HTT affect the risk for alcohol dependence.

A population association between a regulatory variation in the promoter of the serotonin transporter gene (HTT) and severe alcohol dependence was recently reported. We analyzed this potential association in a large number of systematically ascertained families in the United States; these families had at least three first-degree relatives who were alcohol-dependent. Analyses focused on individuals defined as alcohol-dependent by criteria from ICD-10 and on subsets of these individuals reporting withdrawal-related symptoms. Application of the transmission disequilibrium test did not provide support for either linkage or association between this functional polymorphism and alcohol dependence; there was no significant bias in the transmission of either allele to the alcohol-dependent offspring. We also report that African Americans differ from Caucasians in allele frequencies for this polymorphism.

The collection of oocytes from bovine ovaries.

Various needle sizes (17- and 20-g) and aspiration pressures (25, 50, 75 and 100 mmHg) were used to aspirate a total of 5,827 ovarian follicles from bovine ovaries from a slaughterhouse source to assess the impact on the quantity and quality of recovered immature oocytes. The cumulus oocyte complexes (COC's) were graded according to the presence and consistency of cumulus cells surrounding the oocyte and the data analyzed using general linear models. Overall recovery rates and the recovery of oocytes considered viable for IVM/IVF procedures (Classes A, B and C) were both significantly higher using a 17-g needle than a 20-g needle (P < 0.01). As the vacuum pressure increased so did the recovery rate of the total number of oocytes, although the number of viable oocytes reached a maximum at a calculated vacuum pressure of 55 mmHg for the 17-g needle and 77 mmHg for the 20-g needle, with an increased incidence of denuded oocytes at higher vacuum pressures. In a second experiment conducted on 1, 473 follicles, no significant difference was found between 17-g double (flushing) and 17-g single lumen needles in the recovery rate of either the total number or number of viable oocytes when using a vacuum pressure of 50 mmHg.

The human cornea has a high incidence of acquired chromosome abnormalities.

Structurally and functionally, the human cornea is a highly specialized tissue. The corneal stromal collagen matrix is uniquely transparent and yet maintains a mechanically tough and chemically impermeable barrier between the eye and environment. We report for the first time that stromal keratocytes of the human cornea show cytogenetic abnormalities with a frequency that is unprecedented among normal tissues. The abnormalities are acquired, clonal and nonclonal, primarily aneuploid in nature, and present in normal as well as diseased corneas.

Measuring fruit and vegetable intake: is five-a-day enough?

OBJECTIVE: Validation of a self-monitoring "portions' measurement of fruit and vegetable (F&V) consumption against a standard of weighed intakes. DESIGN: Component of a randomized controlled trial. SETTING: Subjects attended research centres in Reading and Glasgow for instruction and monitoring but undertook free-living dietary changes at home. SUBJECTS: A study sample of 42 adult men and women fulfilling the main recruitment criterion of eating less than five F&V portions/day but contemplating increasing intakes and providing weighted baseline reported energy intakes exceeding (estimated basal metabolic rate x 1.1). INTERVENTIONS: Subjects attended an intensive group advice session which included the specific relationship of high F&V intake with reduced risk of disease; practicalities; portion definition and measurement recording. The target was to exceed five F&V portions/day for 8 weeks. MAIN OUTCOME MEASURES: Self-recorded simultaneous weighed inventories and F&V portion measures. RESULTS: Data from subjects who were not evident under-recorders showed correlations between portion and weighed intakes of r = 0.73, (P < 0.000), although the portions measure tended to under-estimate intakes. Using 80 g/portion the "5-a-day' concept tends to create false negatives (namely consumption could be greater than 400 g whilst recording fewer than five discrete portions) but rarely false positives (namely recorded consumption of less than 400 g did not give measures of more than five discrete portions). CONCLUSIONS: The data suggest that the five portions F&V/day health message, if used in conjunction with defined discrete portions, would encourage desirable consumption exceeding 400 g.

Determination of penicillin susceptibility of Streptococcus pneumoniae using the polymerase chain reaction.

AIM: To develop a polymerase chain reaction (PCR) based method to detect penicillin susceptibility in isolates of Streptococcus pneumoniae (SP). METHOD: PCR primers were designed to amplify differential nucleotide sequences of the penicillin-binding protein (PBP) genes 2b, 2x, and 1a in penicillin susceptible and resistant strains of SP. Primers derived from the PBP 2x and 2b genes were designed to amplify products from penicillin susceptible S pneumoniae (PSSP), whereas primers derived from the PBP 1a gene were designed to amplify gene sequences of penicillin resistant S pneumoniae (PRSP). RESULTS: Two hundred and thirty clinical isolates of SP from the USA, UK, Kenya, Romania, and the Kingdom of Saudi Arabia were tested. Of the isolates, 116 were penicillin susceptible, 65 were intermediately resistant, and 49 were highly resistant. PCR identified 108 (93%) of 116 of PSSP isolates, 55 (85%) of 65 intermediately resistant isolates, and all of the 49 highly resistant isolates of SP. The susceptibility of 16 (7%) isolates could not be determined using PCR. All of these 16 isolates had a minimum inhibitory concentration (MIC) of penicillin < 1 mg/l. None of the highly resistant isolates was identified as penicillin susceptible by PCR, although two of the isolates with intermediate resistance (MIC = 0.125 mg/l) were. CONCLUSION: Using primers that differentially identify the genotypes of susceptible and resistant strains of SP, PCR provides a rapid method for determining the penicillin susceptibility of SP isolates and could potentially be used for testing clinical samples.

The applicability of selected rational-emotive therapy principles for pastoral counseling.

Examines the compatibility and possibility of communication between pastoral counseling and Rational-Emotive Therapy (RET). Uses a Delphi technique to determine whether a randomly selected group of pastoral counselors (N = 15) believe that the basic principles of RET can be used without surrendering theological tenets foundational to pastoral counseling. Concludes that some RET principles may be applicable for pastoral counseling.

The genesis, tutelage and exodus of B cells in the ileal Peyer's patch of sheep.

The ileal Peyer's patch (PP) is a prominent lymphoid organ that extends 1-2 meters along the terminal small intestine of sheep. It is comprised of rapidly proliferating B cells that make major contributions to the animals total B cell system. The characteristics of this tissue in sheep have enabled a variety of novel approaches to studying both the B cell system and the contribution of PP to the mucosal immunity. The sheep ileal PP has characteristics that place it in a category similar to that of the thymus, bone marrow and the avian bursa of Fabricius. The ileal PP develops before birth and involutes while the sheep is still young. It produces B cells that populate the immune system but most of the large numbers of newly-formed B cells are rapidly destroyed by apoptosis. It has been concluded that this death is related to a selection event that examines each newly-formed cell. Antibody diversity in sheep is a post-rearrangement event, generated by the process of somatic hypermutation. A comparison of the lambda light chain gene from surviving and dying B cells indicates that when the PP is at its greatest size the dying cells have the characteristics of cells with high affinity receptors for a selecting ligand/antigen. It is proposed that if B cells proliferate in the continual presence of selecting ligand/antigen that the B cell receptor might develop a sufficient affinity to trigger apoptosis. This process might contribute to elimination of clones with high affinity for either self antigens or continually present environmental antigens.

Patients' reactions to the suicide of a psychotherapist.

This descriptive exploratory study examines the behavioral and affective reactions of clients to the suicide of their psychotherapist 1 year after the event, including remembered responses upon first hearing of the suicide. Initial grief reactions of participants were found to be similar to other suicide survivors with strong attachments to the deceased, including depression, numbness, anger, hopelessness, and panic. One year after the event, several participants continued to discuss the event with friends, had persistent feelings of depression and abandonment, and continued to deny the death as a suicide. A reluctance to reenter therapy was also reported. Almost all participants stated that suicide was an acceptable solution to some problems. These results suggest the potential existence of pathological grief for many participants in this study. Implications for the mental health community and intervention services for patient survivors are discussed.

Bone mineral density in women with depression.

BACKGROUND: Depression is associated with alterations in behavior and neuroendocrine systems that are risk factors for decreased bone mineral density. This study was undertaken to determine whether women with past or current major depression have demonstrable decreases in bone density. METHODS: We measured bone mineral density at the hip, spine, and radius in 24 women with past or current major depression and 24 normal women matched for age, body-mass index, menopausal status, and race, using dual-energy x-ray absorptiometry. We also evaluated cortisol and growth hormone secretion, bone metabolism, and vitamin D-receptor alleles. RESULTS: As compared with the normal women, the mean (+/-SD) bone density in the women with past or current depression was 6.5 percent lower at the spine (1.00+/-0.15 vs. 1.07+/-0.09 g per square centimeter, P=0.02), 13.6 percent lower at the femoral neck (0.76+/-0.11 vs. 0.88+/-0.11 g per square centimeter, P<0.001), 13.6 percent lower at Ward's triangle (0.70+/-0.14 vs. 0.81+/-0.13 g per square centimeter, P<0.001), and 10.8 percent lower at the trochanter (0.66+/-0.11 vs. 0.74+/-0.08 g per square centimeter, P<0.001). In addition, women with past or current depression had higher urinary cortisol excretion (71+/-29 vs. 51+/-19 micrograms per day [196+/-80 vs. 141+/-52 nmol per day], P=0.006), lower serum osteocalcin concentration (P=0.04), and lower urinary excretion of deoxypyridinoline (P=0.02). CONCLUSIONS: Past or current depression in women is associated with decreased bone mineral density.

Partial retinal detachment at 3 months after threshold retinopathy of prematurity. Long-term structural and functional outcome. Multicenter Trial of Cryotherapy for Retinopathy of Prematurity Cooperative Group.

OBJECTIVE: To investigate the structural and functional outcome at age 4 1/2 years of eyes that had partial retinal detachment (RD) at 3 months after the occurrence of threshold retinopathy of prematurity; these eyes were involved in the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity study. METHODS: Of the 531 eyes in the randomized portion of the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity study, 61 had partial RD at the 3-month examination. The extent of retinal involvement was recorded, and the presence of a foveal detachment was noted. At the examination conducted at age 4 1/2 years, the fundus structure was graded into cicatricial retinopathy of prematurity outcome categories by study ophthalmologists, and the visual acuity was measured by masked examiners using the Teller Acuity Card procedure and the crowded HOTV recognition acuity test. RESULTS: Of the 61 eyes, 7 eyes continued to have partial RD at age 4 1/2 years. Of the remaining eyes, 20 eyes had structural outcomes classified as favorable in the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity study, and 27 eyes had unfavorable structural outcomes. Data were unrecordable for 3 eyes and missing for 4 eyes. No difference in outcome was found for eyes with partial RD at 3 months that had undergone cryotherapy vs eyes that had served as controls. Only 6 eyes had a visual acuity better than 20/200. When partial RDs did not involve the fovea at 3 months, structural and functional outcomes at 4 1/2 years were better than when RDs involved the fovea. The best predictor of outcome was the extent of RD at 3 months. CONCLUSIONS: Partial RD present 3 months after threshold retinopathy of prematurity is unstable anatomically, and the visual outcome is generally poor. Structure and function at 4 1/2 years are related to the extent of RD and the involvement of the fovea at 3 months.

Sexual contact tracing outcome in adolescent chlamydial and gonococcal cervicitis cases.

BACKGROUND AND OBJECTIVE: Treatment of sex partners is an essential part of sexually transmitted diseases (STD) control. This study examined the efficacy of contact tracing via patient self-referral in gonococcal and chlamydial cervicitis cases among adolescents, compared with the effectiveness of provider-referral. STUDY DESIGN: Adolescent females with culture-proven chlamydial or gonococcal cervicitis were the study subjects. This cohort study was done in an urban non-STD clinic setting. The subjects chose either provider-notification or self-notification method to inform their sex partner(s) in 2 months preceding the interview date. RESULTS: Two hundred and sixty-five eligible subjects (91% African-American, 9% white) were identified. One hundred and ninety-eight sex contacts were reported by 165 (62%) cases; no contact was elicited in the remaining 100 (38%). The follow-up data revealed that 129/198 (66%) contacts were informed: 63 contacts by 61 index cases, 54 contacts of 47 cases by the case manager, 9 by both methods, and 3 by unspecified means. History of treatment was obtained in 54 contacts, including 37% (23/63) of patient-notified contacts and 50% (27/54) of provider-notified contacts; these 54 contacts constituted 42% of informed contacts, or 27% of all named contacts. The mean number of sexual contacts treated per index case was 0.58 (27/47) for the provider-referral groups and 0.38 (23/61) for the self-referral groups. Successful contact tracing was documented in 19.3% (51/265) of all index cases, resulting in treatment of 54 contacts. CONCLUSION: This study demonstrates the need for more effective partner treatment strategies in adolescent STD cases.

A scale for predicting nonphysiological contributions to pain.

BACKGROUND: Waddell and colleagues have identified a series of nonorganic signs which may appear in low back pain patients. The occurrence of these signs helps predict nonphysiological factors in the etiology of the pain. This study describes the development of the Waddell Equivalency Scale (WES) which elicits comparable information from patients with pain at other sites. METHODS: One hundred fifty patients presenting to a comprehensive pain clinic with low back pain were evaluated by physicians for Waddell signs and by psychologists using the Pain Presentation Inventory (PPI) developed by Psychometric Designs Inc. From three scales of the PPI, correlation, analysis of variance and multiple regression identified a formula which predicted Waddell signs from the PPI. The study was replicated on a new sample of 150 patients with almost identical results. The two samples were combined, again with highly similar results. An interpretive scheme was devised for use of the WES scale. RESULTS: The WES score derived from three scales of the PPI predicts the number of Waddell signs elicited from low back pain patients. Since it comes from a paper-and-pencil test rather than a physical examination, it can be used with patients whose pain locus is other than the back. CONCLUSIONS: Behavioral expectations and treatment plans, à la Waddell, can now be applied to patients with other than low back pain. Clinical application of the WES information over a variety of pain populations will be required to determine the parameters of its usefulness.

Phase I trial of iodine 131-labeled COL-1 in patients with gastrointestinal malignancies: influence of serum carcinoembryonic antigen and tumor bulk on pharmacokinetics.

PURPOSE: COL-1 is a high-affinity murine monoclonal antibody (MAb) specific for carcinoembryonic antigen (CEA). A phase I trial was conducted in which a uniform quantity of antibody labeled with escalating doses of iodine 131 (131I) was administered to patients with advanced gastrointestinal (GI) malignancies to evaluate tolerance and pharmacokinetics. PATIENTS AND METHODS: Eighteen patients with advanced, assessable GI malignancies (16 colon, one pancreas, and one gastric) previously treated with conventional chemotherapy (but no pelvic radiation) received 20 mg of COL-1 labeled with 131I, with doses from 10 mCi/m2 to 75 mCi/m2. In this cohort, the baseline serum CEA level ranged from 6 to 2,739 ng/mL (mean +/- SD, 500 +/- 639). RESULTS: Nuclear imaging detected at least one tumor site in all 18 patients; 82% of all tumor involved organs were positive and 58% of all lesions > or = 1.0 cm were detected. Immune complexes were detected in 89% of patients 5 minutes after completion of infusion, and levels correlated with CEA levels (r = .71). Elevated CEA (> 500 ng/mL) and tumor bulk (total tumor area > 150 cm2) correlated directly with clearance of serum radioactivity and inversely with serum half-life and cumulative serum radioactivity parameters. Nonhematologic toxicity was mild and non-dose-limiting. Hematologic toxicity, particularly thrombocytopenia, was both dose-related and dose-limiting. The maximal-tolerated dose is 65 mCi/m2. The correlation between dose (millicuries per square meter) and thrombocytopenia was made stronger, by accounting for either variation in pharmacokinetics, or variation in serum CEA and tumor bulk. CONCLUSION: 131I-COL-1 is well tolerated, except for hematologic toxicity. These data suggest that patients with highly elevated circulating CEA levels and/or increased tumor bulk may clear 131I-labeled COL-1 more rapidly from the circulation and experience less myelosuppression.
